Wolfram syndrome is a rare, multisystemic, progressive, and autosomal-recessive genetic disease, characterized by diabetes mellitus and diabetes insipidus, optic nerve atrophy, deafness, and other neurological signs. The diagnosis is usually based on history and clinical manifestations but genetic tests are necessary for confirmation. Currently, there are no treatments available to cure or delay disease progression. This report describes a case of a 23-year-old male diagnosed with Wolfram syndrome who presented to the emergency department with several episodes of loss of consciousness. This ca…

A case of neonatal kernicterus due to glucose-6-phosphate dehydrogenase deficiency (G6PD) is described. Diagnosis was delayed as the primary healthcare attendant had no knowledge of this condition and its potential to cause rapidly escalating levels of bilirubin and as she was reassured by the lack of signs of systemic illness or anaemia. The baby has been left deaf, blind, intellectually handicapped, epileptic and paralysed due to athetoid cerebral palsy. The re-organization of perinatal care in New Zealand, which has led to neonates sometimes being managed solely by primary healthcare attend

Spoken language assessment and training for profoundly deaf children should be based on normal developmental sequences. However, existing models of normal speech/language development have not been able to account for shifts from one developmental level to another. The implications of more recent theories of the development of expressive communication are reviewed. The concept of levels of spoken language development is reevaluated in terms of these theories. It is proposed that the developmental landmarks that initiate each level are a product of changes with growth in underlying subsystems. S

To investigate whether otosclerotic foci in the bony capsule of the cochlea and labyrinth can be demonstrated by high-resolution computed tomography, CT images from 31 patients suspected of having cochlear otosclerosis were submitted to a retrospective analysis. Fourteen of the patients had a clinically confirmed otosclerosis associated with an advanced mixed hearing loss. In four of these cases, fresh otospongiotic foci were detected in the bone of the cochlear capsule, while older otosclerotic foci were not demonstrated. In fifteen patients with progressive, purely sensorineural hearing loss

Communicating syringes confined to the brain stem are extraordinarily rare. Two patients, presenting with signs and symptoms of cerebellar dysfunction, later developed evidence of brain-stem disease with dysarthria, nystagmus, deafness, and internuclear ophthalmoplegia. The condition of both patients had been diagnosed clinically as multiple sclerosis, but at autopsy they had a striking keyhole-shaped syrinx in the midbrain and upper pons, which communicated with the aqueduct and fourth ventricle without associated syringomyelia. In addition, both patients had marked atrophy and gliosis of the…

This is the first of two auditory science tutorials designed for otolaryngologists, audiologists, and others dealing with deafness and its consequences. This tutorial is intended as an update on recent advances in the understanding of cochlear function both in the normal person and in patients with hearing loss of cochlear origin. It draws largely on studies involving electrophysiological recordings from cochlear nerve fibres in "animal models" of cochlear hearing loss. These data allow considerable insight into the underlying causes, for example, loudness recruitment, tinnitus, and poor speec…

On the basis of observation of 26 patients, we consider that the common cause in trigeminal neuralgia and hemifacial spasm is compression of the nerve-root entry zone at the brainstem by a blood vessel, usually an artery. By mobilising the vessel, and securing it away from the nerve, symptoms were relieved in 16 of 17 patients with trigeminal neuralgia and in seven of nine patients with hemifacial spasm for a follow-up period of up to four years. Trigeminal microvascular decompression had few complications, but three patients treated for facial hemispasm had postoperative deafness and one had …

A multiple-channel cochlear implant and speech processor have been used in two postlingually deaf adult patients with a total hearing loss, to enable them to perceive varying degrees of running speech. The results have veen confirmed with open-set CID everyday sentence tests. Using the implant alone, the patients obtained 8% and 14% scores with prerecorded material, and 34% and 36% scores for "live" presentations. This was equivalent to the perception of 35% of connected discourse. When the implant was used in conjuction with lipreading, improvements of 188% and 386% were obtained over lipread

While investigating the possibilities of electrically induced hearing in cases of profound deafness, we observed that very often an appropriate stimulation of the cochlea could result in tinnitus cancellation. We present here a progress report on that phenomenon, including 15 subjects. Such a cancellation was restricted to tinnitus subjectively localized in the ear under test, we observed no effect on noises localized in the other ear, or centrally. This suppression occurred so long as the cochlea was stimulated and stopped when the stimulation ceased. This phenomenon was very reproducible and…

Syndromes of human congenital malformation may be classified be recognizing communities of syndromes that share multiple phenotypic similari … he fact that the syndromes without nasal or labial anomalies have a high frequency of sensorineural deafness as one expression of ectodermal dysplasia whereas those without such anomalies do not. The usefulness of such a syndromal community as a base for evolving a taxonomic scheme of dysmorphogenetic relatedness amongst different syndromes is illustrated.

BACKGROUND: Neurofibromatosis 2 (NF2) is a rare genetic disorder that causes vestibular schwannomas to develop in both eighth cranial nerves. Almost all people with NF2 eventually become completely deaf as a result of progressive tumour enlargement or following surgical or radiotherapy treatment. Other rare abnormal conditions in the inner ears can also cause complete deafness. For people with either indication who are not candidates for cochlear implantation, auditory brainstem implantation is the only treatment option to restore some functional hearing. We conducted a health technology asses…

Sensorineural hearing loss (SNHL) is a category of hearing loss that often leads to difficulty in understanding speech and other sounds. Auditory system dysfunction, including deafness and auditory trauma, results in cognitive deficits via neuroplasticity. Cognitive impairment (CI) refers to an abnormality in the brain's higher intellectual processes related to learning, memory, thinking and judgment that can lead to severe learning and memory deficits. Studies have established a strong correlation between SNHL and CI, but it remains unclear how SNHL contributes to CI. The purpose of this arti…

Human brain demonstrates amazing readiness for speech and language learning at birth, but the auditory development preceding such readiness remains unknown. Cochlear implanted (CI) children (n = 67; mean age 2.77 year ± 1.31 SD; 28 females) with prelingual deafness provide a unique opportunity to study this stage. Using functional near-infrared spectroscopy, it was revealed that the brain of CI children was irresponsive to sounds at CI hearing onset. With increasing CI experiences up to 32 months, the brain demonstrated function, region and hemisphere specific development. Most strikingly, the…

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease with a complex genetic architecture, showing monogenic, oligogenic, and polygenic inheritance. In this study, we describe the case of a 71 years-old man diagnosed with ALS with atypical clinical features consisting in progressive ocular ptosis and sensorineural deafness. Genetic analyses revealed two heterozygous variants, in the SOD1 (OMIM*147450) and the TBK1 (OMIM*604834) genes respectively, and furthermore mitochondrial DNA (mtDNA) sequencing identified the homoplasmic m.14484T>C variant usually associated with Leber's Here…
