CD5L insufficiency exacerbates skeletal joint damage in rheumatoid arthritis.

BACKGROUND AND HYPOTHESIS: CD5L is a glycoprotein induced during inflammation and makes a life-saving contribution in infection and sepsis. Here, we explored the immunomodulatory effect of CD5L in rheumatoid arthritis (RA). We hypothesized that CD5L availability associates with systemic inflammatory activation during arthritis and stratifies monocyte programs linked to joint structural damage. METHODS: Using an experimental RA model, we assessed disease development and severity in wild-type and CD5L-deficient mice and characterized circulating immune cell populations and inflammatory mediators

Resolving competing evolutionary histories in joint ancestral state reconstruction.

Ancestral state reconstruction (ASR) is a foundational tool in comparative biology, offering insights into the evolutionary history of lineages. With each new evolutionary model, our ability to estimate ancestral states with increased biological realism has improved. However, the field has primarily relied on marginal reconstructions, which focus on individual nodes. This framework is analytically tractable and appropriate for node-specific hypotheses, but it is not designed to identify the most probable sequence of evolutionary events across a tree. We argue that for researchers interested in

Rheumatoid arthritis: transarterial microembolization for refractory localized joint inflammation.

Transarterial microembolization targets periarticular neovascularization in localized joint pain that persists despite systemic therapy. We report a 31-year-old woman with rheumatoid arthritis involving the elbow, knee, and ankle. The patient underwent selective transarterial microembolization, with intra-arterial imipenem/cilastatin delivered to angiographically identified neovessels. Post-embolization angiography demonstrated decreased hypervascularity. Over 12 months, pain and function improved, and magnetic resonance imaging showed reduced synovial proliferation, periarticular edema, and b

Assessing inter-joint coordination during gait in hip osteoarthritis.

BACKGROUND: Hip osteoarthritis (HOA) often leads to compensatory movements such as trunk lateral bending during gait due to pain and restricted range of motion. Because multiple joints coordinate to control the center of mass (CoM) during walking, quantitatively assessing compensatory strategies is essential to understand HOA pathology and to develop rehabilitation programs. Traditional indices do not adequately evaluate multi-joint coordination. However, Uncontrolled Manifold (UCM) analysis enables assessment of multiple-joint utilization to control CoM in HOA. This study aimed to investigate…

Joint association of cancer and cardiovascular disease with all-cause mortality.

BACKGROUND: Cancer and cardiovascular disease (CVD) are the two leading causes of death in the United States. Whether their coexistence confers excess mortality risk remains insufficiently characterized. This study looks to examine the cross-sectional association between cancer history and prevalent CVD and evaluate their joint association with all-cause mortality. METHODS: We analyzed 8,525 participants from the Third National Health and Nutrition Examination Survey. Cancer history was self-reported and categorized as skin or non-skin cancer. CVD was defined by self-reported clinical disease

A Novel Surgical Approach to Induce an Ankle Joint Contracture in Rats.

Our primary aim was to establish a novel surgical immobilization approach to induce a persistent ankle plantar-flexion contracture in the Wistar rat. A stainless steel suture was placed in experimental animals (n = 14) and pulled taut between the calcaneal bone and the knee joint, restricting dorsal-flexion while still allowing for limited ankle plantar-flexion, muscle activity, and loading. The immobilization method was well tolerated; adverse events were limited and manageable. After 4 weeks, immobilization was released by cutting the suture. After 4 weeks of immobilization, ankle angle at 5

Patellofemoral Joint Preservation: Cartilage Restoration, Unloading, and Avoiding Complications.

Patellofemoral (PF) chondral lesions are a common source of knee pain and disability, typically arising from traumatic (instability/fracture) or degenerative (overload) etiologies. This review synthesizes contemporary evidence on PF joint preservation with an emphasis on avoiding complications. We highlight the importance of defining lesion characteristics and identifying the primary mechanical driver(s) of chondral injury. We then summarize current and emerging cartilage restoration options. Finally, we emphasize that durable outcomes often require concomitant biomechanical optimization throu

[Transarterial periarticular embolization as a treatment option for chronic joint pain].

CLINICAL ISSUE: Osteoarthritis-related chronic joint pain substantially impairs function and quality of life. A clinically relevant subgroup of patients remains insufficiently controlled by conservative care and is not yet eligible for, or declines, arthroplasty ("treatment gap"). STANDARD TREATMENT: First-line management includes weight reduction and physiotherapy, analgesics/nonsteroidal anti-inflammatory drugs (NSAIDs), intra-articular injections, and-when indicated-surgical options up to joint replacement. TREATMENT INNOVATIONS: Transarterial periarticular embolization (TAPE) targets abn

Joint economic and epidemiological modelling of alternative pandemic response strategies.

In an emerging pandemic, policymakers need to make decisions with limited information and require trade-offs between the health impact of the pandemic and the economic costs of the response. Most mathematical models have focused on direct health impacts, neglecting the economic costs of control measures. Here, we introduce a framework that captures both health and economic costs and compare the expected aggregate costs of alternative strategies across a range of epidemiological and economic parameters. We find that for diseases with low severity, mitigation tends to be the most cost-effective

FGF19 promotes cartilage destruction in the progress of temporomandibular joint osteoarthritis.

Temporomandibular joint osteoarthritis (TMJOA) is a degenerative joint disease characterized by the progressive deterioration of the condylar cartilage, compensatory remodeling of subchondral bone, and persistent synovial inflammation. The onset of TMJOA is accompanied by the activation of cytokines, which exacerbate the imbalance in joint homeostasis and accelerate the destruction and even irreversible degradation of joint structures. FGF19, as an important multifunctional fibroblast growth factor, has been reported to be substantially up-regulated in the cavity of osteoarthritis joint. Howev

Facet joint dilatation as a radiological indicator of vertebral segmental microinstability.

BACKGROUND: Chronic low back pain (LBP) is a major healthcare burden that reduces quality of life and productivity. Vertebral segmental microinstability is considered a contributing factor in LBP, while radiological signs reflecting biomechanical dysfunction, including disc degeneration, Modic changes, muscle fatty infiltration, and facet joint (FJ) dilatation, remain insufficiently defined as clinical markers. METHODS: This was a retrospective cohort study involving 168 adults with chronic LBP. Patients underwent clinical examination including passive lumbar extension (PLE), supine-sit, ilio

Effect of lutein on improving traumatic temporomandibular joint fibrous ankylosis.

This study investigated the therapeutic effects and potential mechanisms of lutein on traumatic temporomandibular joint (TMJ) fibrous ankylosis. A TMJ fibrous ankylosis model was established in rabbits, then randomly divided into a lutein-treated group and a model group (n = 8 per group), followed by daily oral administration of lutein (10 mg/kg) or saline for 4 weeks. Compared with the model group, the lutein group showed significantly higher body weight (P = 0.049), maximum mouth opening (P = 0.043), condylar surface area (P = 0.001), and condylar volume (P = 0.003). [Arthroplasty and arthrodesis of the proximal interphalangeal joint: narrative literature review].

The proximal interphalangeal joint is a central structure in the function and biomechanics of the hand and finger flexion. It has unique anatomical characteristics that influence its normal function and is affected by multiple pathologies that can markedly alter its architecture and function. This narrative literature review correlates basic sciences, clinical practice, and surgical management, focusing on two treatment options that sacrifice the joint: arthroplasty and arthrodesis. The aim is to identify evidence to support decision-making regarding which patients are best suited for each typ

Recurrent prosthetic joint infections: A review of evaluation, management, and outcomes.

Periprosthetic joint infection (PJI) remains one of the most serious complications of total joint arthroplasty. While two-stage revision arthroplasty remains the most commonly utilized treatment strategy for chronic PJI in North America, recurrent infection following revision represents an increasingly encountered and particularly challenging clinical problem. These patients often have substantial host comorbidity, prior surgical burden, compromised soft tissues, and limited reconstructive options, contributing to high rates of treatment failure and morbidity. This narrative review summarizes

Diffusion-Weighted Imaging for the Evaluation of the Sacroiliac Joint in Pediatric Patients.

OBJECTIVE: Maturational signal in the sacroiliac joint (SIJ) of skeletally immature youth is often misinterpreted as inflammation. Diagnostic tools that improve specificity are greatly needed. Apparent diffusion coefficient (ADC) values from diffusion-weighted imaging (DWI), when used with standard imaging, may enhance diagnostic accuracy. We aimed to define normative pediatric ADC values and establish thresholds to distinguish normal from inflammatory SIJ signals. METHODS: ADC values were measured using circular regions of interest (ROIs) on the anterior, central, and posterior slices of the
