[Operative treatment of anterior olecranon fracture-dislocation].

OBJECTIVE: To investigate the method and effectiveness of operative treatment of anterior olecranon fracture-dislocation. METHODS: Between January 2007 and December 2010, 10 cases of anterior olecranon fracture-dislocation were treated. There were 6 males and 4 females with an average age of 46.1 years (range, 27-68 years). The injury was caused by traffic accident in 7 cases, falling from height in 2 cases, and falling in 1 case. Nine cases were fresh fracture and 1 case was old fracture. There were 9 cases of ulnar olecranon comminuted fracture and 1 case of simple oblique fracture. Associa

OPerative versus non-opERAtive management of isolated ULNAr diaphyseal fractures (OPERA-Ulna): protocol for a randomized controlled trial.

AIMS: Isolated fractures of the ulnar diaphysis are uncommon, occurring at a rate of 0.02 to 0.04 per 1,000 cases. Despite their infrequency, these fractures commonly give rise to complications, such as nonunion, limited forearm pronation and supination, restricted elbow range of motion, radioulnar synostosis, and prolonged pain. Treatment options for this injury remain a topic of debate, with limited research available and no consensus on the optimal approach. Therefore, this trial aims to compare clinical, radiological, and functional outcomes of two treatment methods: open reduction and int

Functional outcomes and complications following combined locking plate and tunneled suspensory device fixation of lateral-end clavicle nonunions.

BACKGROUND: Lateral-end clavicle fractures have a high rate of nonunion that can lead to ongoing pain and loss of shoulder function. The operative techniques used to manage such nonunions vary, and data on postoperative functional outcomes and complications are limited, with no consensus on the optimal surgical treatment. Our goal was to present the outcomes and complications of a new indication for combined locking plate and tunneled suspensory device fixation in the operative management of lateral-end clavicle fracture nonunions. METHODS: A consecutive series of 38 patients (mean age, 46 ye

Interventions for humeral shaft fractures: mixed treatment comparisons of clinical trials.

UNLABELLED: We designed a study to compare the efficacy of five main therapeutic options, including external fixation, open reduction and plate osteosynthesis (ORPO), minimally invasive plate osteosynthesis (MIPO), dynamic compression plate (DCP), and intramedullary nail (IMN) in treating humeral shaft fractures. Our results indicated that MIPO and IMN were recommended as the optimal treatments for clinical use. PURPOSE: Nowadays, five main therapeutic options are used in treating humeral shaft fractures: external fixation, open reduction and plate osteosynthesis (ORPO), minimally invasive pl

Natural history of anterior chest wall numbness after plating of clavicle fractures: educating patients.

OBJECTIVES: Improved patient outcomes after plating of displaced clavicle fractures have been demonstrated by recent clinical studies. Many of these patients, however, complain of anterior chest wall numbness after this procedure; we hypothesize that numbness likely persists long term for many patients, but without effect on shoulder function. DESIGN: Prospective observational cohort. SETTING: Level 1 trauma center. PATIENTS/PARTICIPANTS: Adult patients undergoing plating of a displaced middle third diaphyseal clavicle fracture. INTERVENTION: Open reduction and internal fixation with super

TFCC reattachment after traumatic DRUJ instability: a simple alternative to arthroscopic management.

With current trends favoring open reduction with internal fixation for distal radius fractures, distal radius-ulnar joint instabilities that were once clinically silent as a result of prolonged immobilization are now being recognized as a significant cause of morbidity. Arthroscopic management has proven to be both an effective and technically challenging treatment for triangular fibrocartilage complex (TFCC) tears. TFCC reattachment offers a safe, reproducible means of managing acute posttraumatic distal radioulnar join (DRUJ) instability after distal radius volar plating. With this technique

Clinical efficacy of titanium elastic nail fixation for the treatment of pediatric limb fractures and its impact on postoperative bone mineral density and bone metabolism levels.

OBJECTIVE: This study was designed to examine the clinical efficacy of titanium elastic nail (TEN) fixation in treating pediatric long bone fractures of the limbs and its impact on bone mineral density (BMD) and bone metabolism. METHODS: Sixty children with long bone fractures of the limbs were divided into the control group (underwent open reduction and internal fixation with a plate) and the observation group (received TEN fixation) (n = 30 each group). Surgical indicators (operative time, intraoperative blood loss, fracture healing time, and hospital stay), fracture healing excellence rate

Bisphosphonate-Related Pathological Femoral Fracture in a Child with ACTG2-Related Intestinal Failure and Metabolic Bone Disease: A Case Report.

INTRODUCTION: Atypical femoral fractures (AFFs) are stress-type insufficiency fractures classically associated with long-term bisphosphonate therapy in adults. Reports of AFFs in pediatric patients are limited primarily to osteogenesis imperfecta or other monogenic bone disorders following prolonged exposure. We report the first case of an AFF occurring after zoledronic acid administration in a child with ACTG2 gene mutation-related intestinal failure and severe metabolic bone disease. CASE REPORT: An 8-year-old male with ACTG2-related intestinal failure, total parenteral nutrition dependence

Reverse shoulder arthroplasty versus locking plate fixation for proximal humeral fracture dislocations in elderly patients: study protocol for a randomized controlled trial.

BACKGROUND: Proximal humeral fractures are common injuries in the elderly population, with their incidence expected to rise due to increased life expectancy. A small subset of these fractures may be associated with dislocating forces that can result in shoulder dislocation concurrent with the fracture. For the elderly patients, proximal humeral fracture dislocation can be treated by open reduction and internal locking plate (LP) fixation or reverse shoulder arthroplasty (RSA). By now, no high-quality trials have compared the two. METHODS: This study is a prospective, single-center, superiorit

Non-operative management of humeral periprosthetic fracture after stemless shoulder arthroplasty.

PURPOSE: Periprosthetic fractures around a stemless implant often involve lesser and greater tuberosities with a well-fixed implant in the metaphysis. This exposes the surgeon to unique questions and challenges as no surgical option (open reduction and internal fixation or revision to a stem) appears satisfactory to address them. Purpose of this study was to evaluate the clinical outcomes after non-operative management of periprosthetic fractures after stemless shoulder arthroplasty. METHODS: A retrospective multicenter study was conducted to identify all patients who had sustained non-operat

Delayed Frey syndrome after closed treatment of condylar fracture.

Frey syndrome is also known as auriculotemporal syndrome and gustatory sweating. It is characterized by the occurrence of hyperesthesia, flushing, and warmth or sweating over the distribution of the auriculotemporal nerve and/or greater auricular nerve while eating foods that produce a strong salivary stimulus. It is commonly seen as a complication of parotidectomy and open surgery of temporomandibular joint. It can also be caused by other forms of trauma, including blunt trauma, but rarely does it occur without trauma. The relation between fracture displacement of the condyle and Frey syndrom

Free flap reconstructions of tibial fractures complicated after internal fixation.

The cases of 15 patients are presented where microvascular soft-tissue reconstructions became necessary after internal fixation of tibial fractures. Primarily, seven of the fractures were closed. Eleven fractures had originally been treated by open reduction and internal fixation using plates and screws, and four by intramedullary nailing. All of the patients suffered from postoperative complications leading to exposure of the bone or fixation material. The internal fixation material was removed and radical revision of dead and infected tissue was carried out in all cases. Soft tissue reconstr

Costoclavicular block for distal radius open reduction and internal fixation.

Interscalene and supraclavicular brachial plexus nerve blocks are routinely used for upper extremity surgeries, although they carry the risk of phrenic nerve involvement, which can lead to diaphragmatic paralysis. The costoclavicular block is a newer block that may mitigate the risk of this complication while providing similar coverage. Our case involves a patient who presented with a displaced distal radius fracture. He received a preoperative single-shot costoclavicular nerve block before undergoing a distal radius open reduction and internal fixation. The patient did not require any opioids

Technical Challenges and Morbidity Associated With Removal of an IlluminOss Implant From a Humeral Shaft: A Case Report.

CASE: A 77-year-old man experienced acute failure of fixation of his humeral shaft fracture after fixation with IlluminOss photodynamic system stabilization (Photodynamic Bone Stabilization System [PBSS]). Owing to the well-fixed IlluminOss PBSS implant to the humeral intramedullary canal, complete removal was deemed not indicated. Partial implant removal and revision open reduction internal fixation with a proximal humerus plate was performed. However, this was complicated by recalcitrant deep hardware infection that failed multiple surgical debridements, requiring eventual humeral osteotomy

Polymethyl Methacrylate Cement Fill as a Definitive Treatment for Massive Bone Defect After Infected Internal Fixation in Bicondylar Tibial Fracture: A Case Report.

CASE: A 62-year-old woman sustained a bicondylar proximal tibial fracture. She had a deep infection after open reduction and internal fixation resulting in massive metaphyseal bone loss. Because only subchondral bone was left, the cavity was filled with cement after pins were used to support the proximal tibia. Then, a hinged knee-spanning external fixator (EF) was applied. After the removal of the EF, the patient was almost pain free and tolerated full weight-bearing, and no further surgeries were needed. CONCLUSION: The cemented wires technique proved to be a good short-term and possibly a

Traumatic lateral dislocation of the great toe fibular sesamoid: case report.

Traumatic dislocation of the hallucal sesamoids is uncommon. This case involves a 17-year-old female driver involved in a head-on collision who sustained traumatic lateral dislocation of the fibular sesamoid associated with intersesamoidal ligament disruption, partial plantar plate avulsion, and impaction fracture of the metatarsal head. The diagnosis was delayed due to incorrect interpretation of initial radiographs. In addition, the severity of the soft-tissue injury was not appreciated, possibly further delaying the diagnosis. The patient was treated with open reduction of the fibular sesam
