Epidemiology of DSM-5 psychiatric disorders in Kenyan Youth with Low and High Psychosis Risk.

INTRODUCTION: There are few psychiatric epidemiology studies among Kenyan youth and fewer among those at high psychosis risk (HR). METHODS: This study assessed the epidemiology of DSM-5 psychiatric disorders in HR and low-risk (LR) individuals to inform research and mental health services. 567 participants (aged 15-25) in HR (n = 246) and LR (n = 260) groups based on Washington Early Recognition Center Affectivity and Psychosis (WERCAP) Screen scores. Diagnostic Interview Schedule, version 5 (DIS-5) assessed DSM-5 psychiatric disorder prevalence. Diagnostic comorbidity and demographic relatio

Stimulant Use Disorder Diagnoses in Adolescent and Young Adult Medicaid Enrollees.

IMPORTANCE: There has been a national increase in fatal and nonfatal overdoses involving stimulants, and 4.5 million US individuals meet criteria for stimulant use disorder (UD), with the highest prevalence in young adults. However, limited information exists on trends in diagnosed stimulant UD. OBJECTIVE: To estimate trends in the proportion of adolescent and young adult Medicaid enrollees diagnosed with a stimulant UD from 2001 to 2020. DESIGN, SETTING, AND PARTICIPANTS: A repeated cross-sectional study (2001-2020) was conducted using administrative health care claims data from Medicaid (p

Change in prescription opioid dose and the risk of mental health-related and substance use-related emergency department presentations: a case-crossover study.

Opioid deprescribing is recommended to reduce opioid-related harms; however, research suggests that opioid dose changes may lead to adverse health outcomes. We aimed to determine if opioid dose changes are associated with mental health-related or substance use-related emergency department (ED) presentations. This self-controlled case-crossover study used POpulation Level Analysis and Reporting (POLAR) primary care data linked with data from 3 metropolitan hospitals in Victoria, Australia. People who had a mental health-related or substance use-related ED (MHSU-ED) presentation between April 20

Neural Responses to Signals for Behavior Change: Greater Within-Person Variability is Associated With Risk Factors for Substance Dependence.

BACKGROUND: An impaired ability to change behavior in the face of cues indicating a need for change is one means of defining risk for substance dependence. The present study used a cognitive task administered in a laboratory as a model of this process. It focused on 2 known and related correlates of risk (conduct disorder, borderline personality disorder) and examined their associations with reactivity to cues requesting a change in motor behavior. METHODS: A total of 224 teenagers, 14 to 19 years of age, performed a task during which white noise bursts were used to cue a requirement to rever

Vulnerability and comorbidity factors of female problem gambling.

This study helps to address a deficiency of gender-specific research into problem gambling. It focuses on the gambling behaviors, family and personal histories and comorbid psychological disorders of 365 female gamblers from across Ontario, Canada, who responded to a mail-in survey. Specifically, this study looks at rates of depression and anxiety, concurrent struggles with other behaviors (such as alcohol and drug use, disordered eating, overspending and criminal activity) and abuse history reported by female gamblers. The reported rates are considerably higher than for the general female pop

Attachment and substance use disorders: a review of the literature and a study in drug dependent adolescents.

Earlier studies on attachment and substance use disorders using the Hazan and Shaver (1987) self-report mainly indicate a link with "avoidant" attachment styles. Studies working with the Adult Attachment Interview (Main & Goldwyn, 1998) have produced inconsistent results. The present study used the Bartholomew (1990) interview coding system to assess attachment in a sample of 71 German opiate using, drug dependent adolescents (DDAs, age 14 - 25) and 39 non-clinical controls. Fearful attachment was predominant in DDAs, while controls were predominantly secure. Severity of drug use, as assessed

Using Social Media to Examine Themes Surrounding Fentanyl Misuse and Risk Indicators.

Background: Opioid misuse is a crisis in the United States, and synthetic opioids such as fentanyl pose risks for overdose and mortality. Individuals who misuse substances commonly seek information and support online due to stigma and legal concerns, and this online networking may provide insight for substance misuse prevention and treatment. We aimed to characterize topics in substance-misuse related discourse among members of an online fentanyl community. Method: We investigated posts on a fentanyl-specific forum on the platform Reddit to identify emergent substance misuse-related themes pot

Socioeconomic disparities in substance use disorder prevalence and severity across the life course.

Gradients across socioeconomic status (SES) exist across most health conditions. While there has been significant attention devoted to the relationship between SES and physical and mental health, less research has examined how these relate to substance use disorders (SUD). In the current study, we use data from the prospective study within the Collaborative Study on the Genetics of Alcoholism (COGA) to explore the relationship between SES (in early life and adulthood) with multiple substance use disorders (alcohol, tobacco, cannabis, opioid, cocaine, and other substances). We find that early l

Substance use stigma reduces trust and informational support: experimental evidence across risk and relationship contexts.

BACKGROUND: This study investigates the impact of addiction history on interpersonal trust and access to social support. Specifically, it explores whether people exhibit decreased trust toward individuals with addiction histories, explores the extent to which positive portrayals can mitigate such distrust, and evaluates whether those with addiction histories receive reduced social support from their closest social networks, including friends and family. METHODS: Two complementary studies were conducted. Study 1 employed hypothetical scenarios to assess trust in both property-related and perso

Brick Walls and Broken Hearts: Physicians Draw how they Feel About Treating Pain and Addiction.

As part of a continuing medical education activity, primary care physicians in Arizona were asked to draw how they feel about treating patients with chronic pain and addiction. Their drawings, complete with cliffs, walls, torn-out hair, and connected hearts, make for a harrowing look at burnout, angst, and empathy among physicians who treat this subset of patients. Public health and exercise facilitators were troubled, leading them to publish this essay along with a selection of the drawings that depict the feelings of the physicians who feel ill-prepared to treat this patient population, whic

Association between toxic drug events and encephalopathy in British Columbia, Canada: a cross-sectional analysis.

BACKGROUND: Encephalopathy can occur from a non-fatal toxic drug event (overdose) which results in a partial or complete loss of oxygen to the brain, or due to long-term substance use issues. It can be categorized as a non-traumatic acquired brain injury or toxic encephalopathy. In the context of the drug toxicity crisis in British Columbia (BC), Canada, measuring the co-occurrence of encephalopathy and drug toxicity is challenging due to lack of standardized screening. We aimed to estimate the prevalence of encephalopathy among people who experienced a toxic drug event and examine the associa

'Never drop without your significant other, cause that way lies ruin': The boundary work of couples who use MDMA together.

MDMA has a variety of pro-social effects, such as increased friendliness and heightened empathy, yet there is a distinct lack of research examining how these effects might intertwine with a romantic relationship. This article seeks to compensate for this absence and explore heterosexual couples' use of MDMA through the lens of the boundaries they construct around these experiences. Three couple interviews, two diary interviews and eight written diaries about couples' MDMA practices were analysed. Douglas' (2001) and Stenner's (2013) work around order, disorder and what lies at the threshold be

Youth experiencing transitional and temporary homelessness: A pilot study.

Homeless youth represent a vulnerable and understudied population, particularly those among them who are hidden, experiencing transitional or temporary homelessness. Little research focuses on this group, including the methods and settings to identify and tailor interventions to their circumstances. Given the immediate risks and possible transition to chronic homelessness, their identification is critical for prevention and intervention efforts. This exploratory pilot study investigated transitional and temporary homelessness among youth. The study demonstrated the ability to identify such par

Effects of Maternal Work Incentives on Teen Drug Arrests.

PURPOSE: This study exploits differences in the implementation of welfare reform across states and over time in the United States in the attempt to identify causal effects of welfare reform on youth arrests for drug-related crimes between 1990 and 2005, the period during which welfare reform unfolded. METHODOLOGY: Using monthly arrest data from the U.S. Federal Bureau of Investigation's Uniform Crime Reports, we estimate the effects of welfare reform implementation on drug-related arrests among 15-17 year olds in the United States between 1990 and 2005. We use a difference-in-differences (DD)
