The Filum Terminale: A Cadaver Study of Anatomy, Histology, and Elastic Properties.

BACKGROUND: The filum terminale is a fibrous band, consisting of the filum terminale internum (FTI), connecting the conus medullaris (CM) with the dural sac (DS), and the filum terminale externum (FTE), connecting the DS with the coccyx. Despite its importance in tethered cord syndrome, published anatomic and physiologic data on the filum terminale remain scarce. We describe 1) the dimensions and position of the FTI and FTE; 2) the histology of the FTI-DS-FTE transition zone; and 3) the extensibility and elastic properties of the FTI and the CM. METHODS: Anatomic measurements were performed o

How much articular displacement can be detected using fluoroscopy for tibial plateau fractures?

INTRODUCTION: While there is conflicting evidence regarding the importance of anatomic reduction for tibial plateau fractures, there are currently no studies that analyse our ability to grade reduction based on fluoroscopic imaging. The purpose of this study was to determine the accuracy of fluoroscopy in judging tibial plateau articular reduction. METHODS: Ten embalmed human cadavers were selected. The lateral plateau was sagitally sectioned, and the joint was reduced under direct visualization. Lateral, anterior-posterior (AP), and joint line fluoroscopic views were obtained. The same fluor

A cadaveric study on sacroiliac joint injection.

The scope of this study was to explore the possibility as well as the feasibility of sacroiliac joint injection following simple X-ray clip location. For the cadaveric study, 10 fixed sacroiliac joint (SIJ) sectional specimens, 4 dried cadaveric pelvises and 21 embalmed adult cadaveric pelvises were dissected, followed by an injection of contrast agent into the joint. The irrigation of the agent was observed through CT scanning. For the radiologic study, 188 CT scans of ankylosing spondylitis patients (143 male, 45 female) were collected from 2010 to 2012, in Nanfang Hospital. What was measure

Effect of simultaneous stretching of the wrist and finger extensors for lateral epicondylitis: a gross anatomical study of the tendinous origins of the extensor carpi radialis brevis and extensor digitorum communis.

BACKGROUND: Pulling the wrist into flexion with the elbow in extension and forearm in pronation has been used as the stretching technique of wrist extensors for lateral epicondylitis. Simultaneous stretching of the fingers in addition to the wrist flexion has also been applied. However, the mechanism of this simultaneous stretching has not been clarified. This study is designed to clarify the mechanism underlying this simultaneous stretching technique based on the anatomical features of the origins of the extensor carpi radialis brevis (ECRB) and extensor digitorum communis (EDC). METHODS: Th

New insights into the pelvic organ support framework.

OBJECTIVE: It is important to understand the underlying mechanisms of the physiological framework of the pelvic organ support system to develop more effective interventions. Developing more successful interventions for restoration of defects of the pelvic floor will lead to symptomatic improvement of pelvic floor prolapse and stress incontinence disorders. The purpose of the current study was to investigate the physiological framework related to the pelvic organ support system and propose the underlying mechanisms of pelvic organ support based on the anatomical findings. STUDY DESIGN: Ten fem

The sinuvertebral nerves at the craniovertebral junction: a microdissection study.

Bogduk et al. (1988, Spine 13:2-8) noted that the joints and ligaments at the cervico-occipital region are susceptible to whiplash injury. The upper three cervical sinuvertebral nerves (SVNs) at the craniovertebral junction (CVJ) are thought to be responsible for mediating pain from the ligaments, dura mater, and soft tissues of the posterior cranial fossa and upper cervical column. The purpose of this study was to describe in detail the origin and course of the SVNs at C0-C1, C1-C2, and C2-C3 intervertebral levels and their anterior intraspinal distribution. A sample comprising 10 adult, 12 f

Tendon insertion at the base of the proximal phalanx of the hallux: surgical implications.

Hallux valgus, limitus, and rigidus are conditions affecting the first metatarsophalangeal joint that can be treated by arthroplasty. Excessive arthroplasty can compromise the insertion of the tendons at the base of the proximal phalanx of the hallux, leading to first metatarsophalangeal joint plantarflexion weakness, cock-up toe deformity, and altered forefoot loading. The present study investigated the anatomic length of insertion of the medial and lateral flexor hallucis brevis, extensor hallucis brevis, abductor hallucis, and adductor hallucis tendons into the base of the hallux proximal p

Anatomic study of the axillary nerve in a Chinese cadaveric population: correlation of the course of the nerve with proximal humeral fixation with intramedullary nail or external skeletal fixation.

INTRODUCTION: The axillary nerve can be injured during external skeletal fixation with Schanz screws or proximal locking screws of intramedullary nails. Being aware of the axillary nerve's anatomic relationship to the proximal humerus is vital for avoiding complications. METHODS: We investigated the relationship of the axillary nerve to surrounding bony landmarks by studying 88 axillary nerves in 44 embalmed cadaveric adult Chinese males. These measurements were then compared with the results from a similar study among Caucasians using the same reference points. RESULTS: We identified three

Addition of a short central extension to surface cemented tibial trays in primary TKA: an in vitro study of the effect on initial fixation stability and its relationship to supporting bone density.

BACKGROUND: Short central extensions which do not enter the tibial medullary canal are incorporated to cemented tibial components to increase initial stability in primary total knee arthroplasty. Their role in tibiae of differing preoperative mechanical quality has been little studied. METHODS: Twelve embalmed cadaveric tibiae were paired and divided into two groups, receiving a similar cemented tibial component with or without a non-cemented short central extension (10 mm diameter, 35 mm length). The specimens were subjected to 6000 cycles of a medially applied 1350 N load. Relative bone-tra

Sectioning of filum terminale externum using a rigid endoscope through the sacral hiatus. Cadaver study.

AIM: Tethered cord syndrome (TCS) is a stretch-induced functional disorder of the spinal cord, which is directly related to filum fixation. Classic surgical approaches to the filum involve open surgery and include varying amounts of spinal bone removal. In an effort to reduce the morbidity and mortality of these procedures, the authors explored a less invasive method. They evaluated the ability, safety and feasibility for endoscopic sectioning of the filum terminale externum by performing upward orientated navigation in the extradural sacral spinal canal through the sacral hiatus using a rigid

Anatomical study of the superior orbital fissure as seen during a pterional approach.

OBJECT: The superior orbital fissure (SOF) is an important landmark in the neurosurgical pterional approach, but the anatomical features of the SOF and the procedures necessary to fully expose it and its contents have not been detailed. Although the pterional approach is commonly used during skull base or vascular surgery by neurosurgeons who may already be familiar with its nuances and anatomical relationships to the SOF, this knowledge may also be useful to the wider neurosurgical community. The authors describe the spatial relationships of the contents of the SOF and suggest a specific sequ

Is there a safe area for the axillary nerve in the deltoid muscle? A cadaveric study.

BACKGROUND: Several authors have defined a variety of so-called safe zones for deltoid-splitting incisions. The first aim of the present study was to investigate the distance of the axillary nerve from the acromion and its relation to arm length. The second aim was to identify a safe area for the axillary nerve during surgical dissection of the deltoid muscle. METHODS: Twenty-four shoulders of embalmed adult cadavers were included in the study. The distance from the anterior edge of the acromion to the course of the axillary nerve was measured and was recorded as the anterior distance. The sa

Prevalence of accessory head of flexor pollicis longus muscle and its relation to anterior interosseous nerve in Thai population.

A detailed description of the accessory head of flexor pollicis longus muscle (AHFPL) in the Thai population has not been reported. Because it is one of the causes of anterior interosseous nerve syndrome (AINS), a study was carried out on 120 Thai cadavers (70 embalmed, 50 fresh; 78 male, 42 female) to elucidate the prevalence of AHFPL, its morphology and relationship with the anterior interosseous nerve (AIN). The prevalence of AHFPL was 62.1% (149/240) with 74.5% (111/149) of its origin on medial epicondyle, 23.5% (35/149) on coronoid process and 2% (3/149) on flexor digitorum superficialis

[Chemical synthesis of biodegradable poly-para-dioxanone and its application for mandibular fracture fixation].

OBJECTIVE: To observe mandibular fracture healing after synthesized bioabsorable poly-para-dioxanone (PDS) ligature fixation. METHODS: Para-dioxanone monomer was prepared by chemical reactions of metallic sodium, ethylene glycol and other raw materials. PDS was synthesized by polymerizing the purified para-dioxanone in the presence of catalyst under the conditions of negative pressure and certain temperature. PDS ligature was obtained by processing the polymer into small granules, drying and melt-extruding through a spinnerette. Eighteen adult male New Zealand rabbits were assigned randomly i

Persistent hypoglossal artery.

It is well known that the internal carotid artery has no branches in the neck during postfetal life (von Lanz & Wachsmuth, 1955), but during embryonic development there are anastomotic connections between the internal carotid and basilar artery that rarely persist after intrauterine life (Hassen-Khodja et al. 1992; Widmann & Sumpio, 1992; De Caro et al. 1995). The most common anomaly found incidentally on angiography is a persisting trigeminal artery, but persistent hypoglossal, otic and proatlantal arteries can also be found (Reynolds et al. 1980; Ouriel et al. 1988; Salas et al. 1998). Anatomical basis of the variable aspects of injuries of the axillary nerve (excluding the terminal branches in the deltoid muscle).

The course of the axillary n. is complex with three points of angulation that may be used to delineate four segments and a fifth segment that corresponds to the intramuscular ending of the nerve in the deltoid m. The purpose of this study was to determine the precise anatomy of the nerve and of its branches, and some morphologic features for each segment. Thirty-two shoulders from embalmed adult cadavers have been studied. The axillary n. was divided in five segments: 1) from its origin to the inferior border of the subscapularis m., 2) from the subscapularis m. to the anterolateral border of

Morphometric cadaveric assessment of suprascapular and axillary nerve trajectories in relation to glenohumeral landmarks relevant to reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is increasingly used for complex shoulder pathology but carries a risk of iatrogenic nerve injury, particularly to the suprascapular (SSN) and axillary nerves (AXN). Detailed morphometric data on their relationship to glenoid landmarks remain limited. METHODS: Forty embalmed adult cadavers (60 shoulders) were dissected at PGIMER, Chandigarh, after ethics approval (PGI/IEC/2024/EIC000563). The SSN and AXN were exposed, and distances from nerves to glenoid "clock-face" positions and to osseous landmarks were measured with digital calipers.

Thoracic facet joint innervation: identifying and accessing the articular branch.

BACKGROUND: The thoracic zygapophyseal (Z) joints, also known as facet joints, are potential sources of thoracic back pain. Historically, medial branch innervation of these joints was thought to mirror that of cervical and lumbar Z-joints. However, recent anatomical studies suggest that distinct thoracic articular branch nerves innervate these joints. This finding provides potential interventional targets for thoracic facet blockade and radiofrequency neurotomy. OBJECTIVE: This study aimed to identify the thoracic articular branch nerves for mid-thoracic joints and develop techniques for acce
