Onchocerciasis is endemic on the island of Bioko, Equatorial Guinea, where it is transmitted by the 'Bioko form' of the Simulium damnosum complex, a cytospecies unique to the island. A postmenopausal woman with abdominal pain underwent CT, with the rare finding of pneumosalpinx produced by colosalpingeal fistula from diverticulitis. Though the burden of diverticular disease is increasing globally, salpingoenteric fistulas remain a serious and under-characterized complication, often only discovered during surgery. Identification of pneumosalpinx, a rare specific imaging finding secondary to this uncommon fistula, aids the radiologist in prospective recognition of this entity and expedites definitive treatment.

Mucormycosis is an aggressive fungal infection characterized by angioinvasion, tissue necrosis, and high mortality rates. Microbial cell-free DNA (mcfDNA) sequencing provides rapid, non-invasive detection of microbial genetic material to guide diagnosis and treatment. We describe 2 cases of immunocompromised patients with invasive mucormycosis wherein mcfDNA sequencing significantly expedited diagnosis, allowing early surgical intervention and antimicrobial treatment.

Tracheal resections are major surgical procedures with a complication rate as high as 44%. Early detection of complications followed by a structured and expedited course of action is critical for achieving a successful outcome. The prevention of complications after tracheal resection starts with a correct indication for resection. A thorough preoperative evaluation, meticulous surgical technique, and good postoperative care in a center that performs airway surgery routinely are important factors for achieving good results.

The nusinersen development and approval process provide important lessons regarding the pathway to marketing approval for gene therapies. These lessons emphasize rigorous clinical trial design, flexibility in trial design and analysis, a collaborative effort with regular communications between the drug developer and the Food and Drug Administration (FDA), and use of FDA's expedited programs. These lessons are critical to the development of gene therapies for the treatment of serious or life-threatening rare diseases.

Neonatal respiratory distress is an emergent condition with a wide differential diagnosis. A 12-day-old newborn presented to the emergency department in respiratory distress. Point-of-care ultrasound allowed clinicians to rapidly exclude cardiac disease and pneumothorax as possible causes of the patient's respiratory distress, and expedited the identification of congenital diaphragmatic hernia. The ultrasound findings and technique, epidemiology, pathophysiology, and radiological diagnosis of congenital diaphragmatic hernia are reviewed.

Of the consequences of portal hypertension, varices carry the most sinister implications. During life, the risk of hemorrhage from varices hangs like a "sword of Damocles" over a cirrhotic's head. Hemorrhage, when it occurs, expedites the patient's demise, or often itself proves to be the terminal event. Compared with other causes of gastrointestinal bleeding, hemorrhage from varices is most severe, and the available therapies, over the long-term, are the least definitive.
