Intralesional injection of BCG using the fibreoptic bronchoscope in the treatment of bronchogenic carcinoma.

Current evidence suggests that the most effective administration of BCG immunotherapy is by the intralesional route. The new technique and complications of BCG intralesional injection of bronchogenic carcinomas via the fibreoptic bronchoscope is described. Seventeen patients with irresectable bronchogenic carcinoma have been treated with chemotherapy and BCG intralesional immunotherapy. Side-effects observed with intralesional BCG injection are: fever, hypotension and transient mild disturbances of liver function tests, and a single episode of granulomatous hepatitis. Early clinical improvemen

Effects of middle ear quasi-static stiffness on sound transmission quantified by a novel 3-axis optical force sensor.

BACKGROUND: Intra-operative quantification of the ossicle mobility could provide valuable feedback for the current status of the patient's conductive hearing. However, current methods for evaluation of middle ear mobility are mostly limited to the surgeon's subjective impression through manual palpation of the ossicles. This study investigates how middle ear transfer function is affected by stapes quasi-static stiffness of the ossicular chain. The stiffness of the middle ear is induced by a) using a novel fiber-optic 3-axis force sensor to quantify the quasi-static stiffness of the middle ear,

Fluorescence spectroscopy: a diagnostic tool for cervical intraepithelial neoplasia (CIN).

A spectroscopic system incorporating a pulsed nitrogen laser, an optical fiber probe, and an optical multichannel analyzer was utilized to record fluorescence spectra of the intact cervix at colposcopy. Spectra were obtained from 66 colposcopically normal areas and 49 histologically abnormal areas (5 pathologic inflammation, 21 HPV infection, 9 CIN I, 10 CIN II, and 4 CIN III) in 28 patients. The resulting spectra could be used to differentiate histologically abnormal tissues from colposcopically normal tissues with a sensitivity, specificity, and positive predictive value of 92, 90, and 88%.

A review of neuroendoscopy and potential applications in veterinary medicine.

The endoscope was first developed over 200 yr ago. Endoscopy has since been applied to many disciplines of medicine. Its application to the nervous system was initially slow and not widely accepted and mainly involved the biopsy of tumors and the treatment of hydrocephalus. Several reasons for neuroendoscopy's limited use include inadequate endoscope technology, high skill level required, the advent of the surgical microscope, and the development of other treatments such as ventricular shunting. Over the past 50 yr, improvements in optical glass lenses, fiber optics, and electrical circuitry h

Nanomaterial-Enabled Fiber-Optic SPR Biosensor for Continuous and Noninvasive Body Fluid Monitoring:Progress and Prospects.

Continuous and noninvasive body fluid monitoring has attracted increasing attention in personalized healthcare, chronic disease management, and wearable point-of-care testing. Fiber-optic surface plasmon resonance (SPR) biosensors are particularly promising for this purpose because they combine label-free and real-time with miniaturization and low sample volume requirements. However, current body fluid sensing technologies and conventional bare metal SPR interfaces still face critical challenges, including insufficient analytical accuracy in complex biofluids, broad resonance linewidths, weak

Factors associated with selective fiberoptic bronchoscopy use after left double-lumen tube placement.

BACKGROUND/AIM: Fiberoptic bronchoscopy (FOB) is considered the gold standard for confirming correct placement of left-sided double-lumen tubes (LDLTs) during thoracic anesthesia. However, routine FOB use is not always feasible in high-volume centers, and selective strategies are frequently adopted. This study aimed to identify factors associated with the need for selective intraoperative FOB following conventional LDLT placement in a high-volume thoracic anesthesia practice where FOB is not routinely used. METHODS: This retrospective observational study was conducted in a high-volume tertiary

A Novel Cost-Effective 3D-Printed Model for Flexible Nasolaryngoscopy Training Provides Trajectory Information and Distinguishes Early Learners From Experts.

OBJECTIVES: Flexible fiberoptic nasolaryngoscopy is among the most common procedures performed by otolaryngologists worldwide. Despite this, there is no standard training method. High-fidelity approaches typically require trainees to learn on live patients and provide general qualitative information about the scope's path. However, trainees lack objective quantitative procedural feedback to optimize their technique, and patients may be subjected to pain and epistaxis. A trainer that provides a safe, reusable interface and quantitative report would transform this learning experience. METHODS:

Awake fiberoptic intubation of a patient with severe multiple trauma in prone position: a case report.

BACKGROUND: Fiberoptic-guided intubation is considered as "gold standard" of difficult airway management. Management of the airway in prone position in patients with severe trauma presenting with penetrating waist and hip injury poses a major challenge to the anesthesiologist. CASE PRESENTATION: A man presented with severe multiple trauma and hemorrhagic shock as a result of an industrial accident with several deformed steel bars penetrating the left lower waist and hip. It was decided to schedule an exploratory laparotomy following extracting the deformed steel bars. Successful administratio

Modified nasopharyngeal airway for pressure support ventilation in airway management of a case of Robin sequence with bilateral temporomandibular joint ankylosis.

The association of Robin sequence (RS) with temporomandibular joint (TMJ) ankylosis is not a common occurrence. Due to restricted mouth opening, difficult bag valve mask ventilation and difficult intubation, such cases are always challenging for anaesthesiologists.A male patient in early childhood with RS and bilateral TMJ ankylosis was scheduled for bilateral gap arthroplasty. Airway management was planned with fibreoptic intubation under sedation to preserve spontaneous ventilation. After sedating the patient, a nasopharyngeal airway modified by using an endotracheal tube connector was inser

Sufentanil, dexmedetomidine combined with surface anesthesia for awake fiberoptic nasotracheal intubation in the patient with severe mouth opening difficulty undergoing wedge resection of the right upper lung: A case report and literature review.

BACKGROUND: Severe mouth opening difficulty may increase the risk of airway management during anesthesia induction, and awake fiberoptic nasotracheal intubation (AFNI) is the first option for patients with orofacial anatomical changes. CASE SUMMARY: A 54-year-old man was scheduled to undergo wedge resection of the right upper lung in August 2021. The patient had a history of enlarged right maxillary lesion resection and partial right maxillary resection surgery in April 2020, which led to orofacial anatomical changes and severe mouth opening difficulty. To avoid difficult airway-related emerg

Fiber-optic-assisted endotracheal intubation complicated by iatrogenic right main bronchus obstruction.

Several aids have been designed for helping anesthetist in managing the difficult airway. Oral carcinomas often distort the airway anatomy and present as difficult airway usually with restricted mouth opening. In these scenarios, elective nasotracheal intubation with fiberoptic is done to secure the airway as well as to provide the full surgical field to the surgeon. Vertically split nasopharyngeal airway is one of the popular aids used as a conduit to pass fiber-optic bronchoscope. Timely removal of the split nasopharyngeal airway is must and if missed, and pushed inside bronchus leading to i

Airway Management in an Infant with a Large Supraglottic Mass after Failed Fiberoptic Intubation.

BACKGROUND: Fiberoptic bronchoscope-guided tracheal intubation is the gold standard for managing patients with supraglottic growths. In infants with a large and overhanging epiglottis, the success of fiberoptic-guided intubation relies heavily on the available space between the inferior surface of the epiglottis and the posterior pharyngeal wall or, more specifically, the superior surface of the supraglottic growth. CASE REPORT: We describe the inability to negotiate the tip of the fiberscope between the epiglottis and the supraglottic growth and the successful use of direct laryngoscopy to i
