Pulmonary hypertension and persistent fetal circulation in the newborn.

Despite these infants' very significant medical instabilities, which require vigorous therapeutic intervention, we have seen a population of infants with little in the way of persistent residual problems. Although many of their pulmonary complications persist after hospital discharge, most resolve within the first year of life. In addition, there are few neurodevelopmental disabilities encountered in such a high-risk population of children.

The Association of Window Ventilation Frequency with Depressive Symptoms Among Older Chinese People: Perceived Sleep Duration and Psychological Resilience.

INTRODUCTION: Little is known about the potential mechanism of window ventilation behavior on depressive symptoms in older people. METHODS: Associations between window ventilation frequency and depressive symptoms were investigated with modified Poisson regression using the 2018 data of the Chinese Longitudinal Healthy Longevity Survey (CLHLS), with 11,374 Chinese adults aged 65 years and over. Serial mediation models were applied to investigate if self-reported total sleep duration and psychological resilience mediated the association between window ventilation frequency and depressive sympt

Reducing time on for extra-corporeal membrane oxygenation for adults with H1N1 pneumonia with the use of the Volume Diffusive Respirator.

BACKGROUND: The investigators compared a series of adult survivors of severe H1N1 pneumonia treated with extracorporeal membrane oxygenation (ECMO) with members of the Extracorporeal Life Support Organization registry for patients with H1N1 with regard to ventilator management while on ECMO. METHODS: Adults who survived ECMO were compared regarding time on ECMO for those treated with the Volume Diffusive Respirator (VDR) or with conventional "lung rest." The VDR delivered 500 percussions/min, with tidal pressures of 24/12 cm H2O and a fraction of inspired oxygen of .4 at 15 beats/min. RESULT

Airway management and one-lung ventilation strategies during hepatic histotripsy: a retrospective service evaluation from a single institution in Hong Kong.

This retrospective single-centre service evaluation describes double-lumen tube one-lung ventilation as a respiratory motion-control strategy for hepatic histotripsy in 26 adults in Hong Kong. Histotripsy is a non-invasive, non-thermal focused ultrasound technique requiring precise targeting, challenged by respiratory-induced liver motion. High-frequency jet ventilation has been used to minimise motion but carries practical and physiological limitations. All procedures were performed under general anaesthesia with intended double-lumen tube one-lung ventilation. Successful double-lumen tube pl

Opioid and sedative use in extremely preterm infants receiving mechanical ventilation.

OBJECTIVE: Quantify opioid and sedative medication exposures in preterm infants receiving mechanical ventilation. STUDY DESIGN: Cohort study of infants <28 weeks' gestation receiving any mechanical ventilation in the first 28 postnatal days. We examined exposure to morphine, fentanyl, midazolam, and lorazepam. We used logistic regression to examine the association between daily respiratory support and probability of medication administration. RESULTS: Of 36,067 infants, 15,025 (42%) received one or more medications. There was a decrease in exposure to fentanyl and benzodiazepines from 2012 t

Impact of Neonatal Ventilation Mode on Outcomes in Patients With Congenital Diaphragmatic Hernia Treated With Extracorporeal Life Support: A Propensity Score Analysis.

High-frequency oscillatory ventilation (HFOV) is used in neonates with hypoxic respiratory failure both as a primary mode and as a rescue mode of ventilation. It is unknown to what degree the use of HFOV provides a measurable benefit to infants with congenital diaphragmatic hernia (CDH) receiving extracorporeal life support (ECLS). We sought to determine whether pre-ECLS use of HFOV compared to conventional mechanical ventilation (CMV) was associated with differences in mortality. Neonates who underwent ECLS for CDH were identified within the Extracorporeal Life Support Organization (ELSO) Reg

An Optimal Initial Ventilation Mode in Congenital Diaphragmatic Hernia: A Prospective Cohort Study.

OBJECTIVES: Respiratory management is crucial for patients with congenital diaphragmatic hernia (CDH). This prospective study aimed to assess the optimal initial ventilatory mode for CDH by comparing conventional mechanical ventilation (CMV) and high-frequency oscillatory ventilation (HFO). METHODS: Fifteen participating institutes used CMV or HFO as the initial ventilation mode for CDH, we thus divided them into two groups (CMV and HFO). Clinical data were prospectively collected from patients with isolated left CDH born at a gestational age ≥ 34 weeks between 2017 and 2021. The primary outc

HFOV vs CMV for neonates with moderate-to-severe perinatal onset acute respiratory distress syndrome (NARDS): a propensity score analysis.

This study aimed to evaluate whether high-frequency oscillatory ventilation (HFOV) could reduce mortality and the incidence of bronchopulmonary dysplasia (BPD) of perinatal-onset neonatal acute respiratory distress syndrome (NARDS) compared with conventional mechanical ventilation (CMV). Medical records were collected and retrospectively analyzed. Among the 700 neonates with NARDS who needed invasive ventilation, 501 (71.6%) received CMV, while 199 (28.4%) received HFOV. One-to-one propensity score matching (127:127) was used to match the baseline characteristics of patients who received CMV a

Increased incidence of idiopathic persistent pulmonary hypertension in Down syndrome neonates.

Down syndrome (DS) patients have an increased risk of developing pulmonary hypertension later in life compared to age-matched controls. The goal of this study was to determine if the incidence of persistent pulmonary hypertension of the newborn (PPHN) is also higher in neonatal DS patients compared to the general population. A retrospective chart review of DS patients admitted during a 3-year period to the neonatal intensive care unit was performed. DS patients with meconium aspiration syndrome, pulmonary infections, or pulmonary space-occupying lesions were excluded. DS patients were divided

Percutaneous transtracheal emergency ventilation during respiratory arrest: comparison of the oxygen flow modulator with a hand-triggered emergency jet injector in an animal model.

The oxygen flow modulator is a device for percutaneous transtracheal emergency ventilation. Simulating a respiratory arrest situation, we studied the effects of this device in comparison with a hand-triggered emergency jet injector during pulmonary resuscitation. Nine pigs were anesthetized and mechanically ventilated. After surgical exposure, an emergency transtracheal airway catheter was inserted into the trachea. Ventilation was stopped until SpO2 was below 70%. Each animal was subsequently randomly ventilated via the transtracheal airway catheter with either the hand-triggered emergency je

[Persistence of fetal circulation and high frequency oscillation].

High frequency ventilation (HVF) was used in 17 newborns with severe respiratory distress due to persistent fetal circulation. All patients except one had been treated previously but not satisfactorily by conventional ventilation. Four children died from neurological causes despite respiratory improvement. HVF failed in one case. Twelve newborns recovered without any sequelae. Thus HVF appears to be a most valuable ventilation technique in this situation.

Alternatives to ECMO.

The past decade has witnessed technological advancements which are unparalleled in neonatology. ECMO has been demonstrated to be a powerful rescue treatment, but has perhaps been overutilised and is not universally available. Alternative treatments have been shown to be both safe and efficacious in the management of infants with respiratory failure. Direct head to head clinical trials will probably be necessary to establish appropriate criteria and indications for use, given the wide diversity of pathophysiology these unique patients present.

Symptomatic patent ductus arteriosus following prophylactic indomethacin. A clinical and biochemical appraisal.

Seven of 36 premature infants with birth weight less than 1,250 g who had been randomly assigned to either high-frequency ventilatory ventilation or conventional mechanical ventilation developed symptomatic patent ductus arteriosus (PDA) after receiving prophylactic indomethacin, 0.2 mg/kg, i.v. 24 h after birth. Infants who developed symptomatic PDA were more likely to be white and male and have more severe pulmonary insufficiency than infants who did not develop symptomatic PDA. Serum indomethacin levels were similar in both groups of patients. There was no difference in the degree of suppre

Iatrogenic perforation of the bronchus intermedius in a 1,100-g neonate.

A 1,100-g infant was found to have a right tension pneumothorax following multiple attempts at endotracheal intubation. Despite the use of high-frequency ventilation, adequate oxygenation was not possible. Bronchoscopy was hazardous, and bronchography of the right lung using propyliodone oil suspension confirmed perforation of the bronchus intermedius. At thoracotomy, while on high-frequency ventilation, the size of the perforation precluded suture repair, and right middle and lower lobectomies were performed. Follow-up at 1 year showed a small, though healthy infant. Review of the surgical li…

Phrenic nerve pacing in two young quadriplegic ventilator-dependent patients.

Patients who survive high cervical injury are usually dependent on mechanical ventilation and tracheostomy if the lesion above C3 is complete. We report our experience with phrenic nerve pacing (PNP) to achieve ventilator-independence in two young quadriplegic patients. A diaphragm conditioning programme, and combination of low frequency electrophrenic stimulation within each inspiratory burst and low breathing frequency enabled both patients initially to achieve continuous 24 hour ventilation independent of mechanical ventilation. One patient reverted to overnight mechanical ventilation (six

Umbilical artery perforation: a potentially life-threatening complication of umbilical artery catheterisation.

Umbilical artery catheterisation is frequently performed in preterm or critically ill newborn infants for invasive monitoring of blood pressure and blood sampling for laboratory tests. It is associated with well-known complications like catheter-related infections, thromboembolic events and aneurysmal formation of the aorta. In this report, we present another major complication of umbilical artery catheterisation: umbilical artery perforation. This complication occurred in a prematurely born infant and resulted in severe haemorrhagic shock, subsequent renal failure and severe periventricular l
