[Treatment of large mandibular cysts (author's transl)].

Apart from certain particular types of lesions, the diagnosis of a histologically benign "large mandibular cyst" (paradental cyst, epidermal cyst, ameloblastoma...) should lead to an attempt at conservative treatment: this does not alter mandibular continuity and if possible, the inferior dental neurovascular pedicle, the soft tissues of the buccal lining, and the dental organs. It is finally the gross pathological findings and if necessary the results of an extemporaneous histological examination that will supply the information leading to the choice of the best type of treatment: marsupializ

An Automated Framework for Mandibular Reconstruction: Evaluation and Clinical Application.

BACKGROUND: Mandibular reconstruction suffers from limitations in automation and objectivity. This study aimed to develop an automated framework to address these challenges. METHODS: A novel approach combining statistical shape modeling and 3D deep learning was developed. Trained on 200 CT scans and validated in 80 clinical cases, its performance was compared to expert manual planning by evaluating reconstruction accuracy, bone contact area, and planning time. Finally, design experiments were performed in five clinical cases. RESULTS: The automated method demonstrated high accuracy (DSC: 0.8

Reconstruction of mandibular defects using free fibula flaps with a spliced surgical template system.

BACKGROUND: Mandibular defect reconstruction after tumor resection poses significant challenges in oral and maxillofacial surgery. The free fibula flap is widely used and effective for reconstructing such defects. Traditional manual segmentation and stabilization can lead to inaccuracies; while 3D-printed guides improve cutting precision, they often overlook the stabilization of bone segments. The conventional surgical template system consists of three separate guides: a tumor removal guide, a fibula cutting guide, and a reconstruction guide, which requires manual detachment and reshaping of f

Comparison of surgical outcomes between primary plate and fibular flap transfer for reconstruction of segmental mandibular defects.

PURPOSE: Although two ways of reconstruction have been reviewed for a long period, comparison of large sample size with nearly randomized patient selection is rare. Here, we compare the surgical outcomes of these two methods from two large medical institutes. METHODS: Totally 176 patients were included from two medical centers with different protocols, 62 patients from MacKay Hospital repair defects with fibula flaps using reconstruction plates. Also 114 patients from Kaohsiung ChangGung Hospital treat with reconstruction plates combine anterior lateral thigh flaps, in which 12 cases had seco

Clear cell odontogenic carcinoma of the mandible: a treatment strategy.

Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor of the jaws, histologically characterized by the presence of agglomerates of cells with eosinophilic cytoplasm. The patient, a 62-year-old Caucasian woman, presented an intraosseous lesion in the mandibular symphysis. A clinical examination revealed a discrete volumetric increase with a hard consistency, palpable to extraoral and intraoral examinations. Imaging studies revealed an extensive radiolucent area, without defined limits, extending from the region of the right second premolar to the left canine. Incisional biopsy ana

[Temporomandibular joint (TMJ): Condyle hyperplasia and condylectomy].

The mandibular condyle is a special structure. Its embryology and physiology provide to the TMJ a particular behavior that explains the occurrence of specific diseases. Condyle hyperplasia is one of these disorders. It can be explained by a dysregulation of the prechondroblast cell layer within the cartilage cap providing an increase in volume first of the condyle, then of the ramus and finally of the entire affected hemiface. Mandible deformation affects the basal bone, leading to dento-alveolar deformations related to compensation attempts. Controversies, condylectomy is performed by many su

Computer-assisted local resection for exostosis osteochondroma of the mandibular condyle.

The purpose of this study was to evaluate the guiding effect of computer-assisted design technique in local resection of exostosis osteochondroma (EOC) from the mandibular condyle. Eight patients diagnosed with EOC through computed tomographic scan were selected from January 2011 to March 2012. SurgiCase CMF 5.0.0.32 software was used for preoperative design. The osteotomy angle, depth, and tumor shape were measured to guide the surgery. The tumors were resected from the stalk 2 mm within the normal tissue to restore the unaffected condyle as much as possible. Postoperative computed tomographi

[The role of scintigraphy in the diagnosis of mandibular bone changes].

Investigating the scintigraphic images of jaws may have a diagnostic value of bone alterations of dental origin. Anterior view of whole body bone scintigraphy revealed hot spot on jaws of 61% (279) of patients. Twenty-six patients (mean age 58.3 year) from all of those who had increased tracer uptake (ITU) in the maxillo-mandibular region were called back for dental examination. 279 out of 459 (61%) patients had ITU in the maxillo-mandibular region. Dental examination revealed the dental origin of ITU in all cases. In case of periapical pathosis tracer uptake showed 28.79% increase compared to

Use of conservative condylectomy for treatment of osteochondroma of the mandibular condyle.

PURPOSE: The aim of the present study was to present a conservative condylectomy technique (condylectomy performed below the condylar head but high in the condylar neck) and articular disc repositioning as the surgical treatment approach for management of osteochondroma of the head of the mandibular condyle. PATIENTS AND METHODS: Six patients (4 females and 2 males) with an average age of 22.3 years (range, 13 to 32 years) and with an osteochondroma of the mandibular condyle were treated with conservative condylectomy. The remaining condylar neck stump was recontoured, and the articular disc

The serratus anterior/rib composite flap in mandibular reconstruction.

A flap comprising the serratus anterior muscle and the underlying rib, based on the serratus branches of the thoracodorsal artery has been developed. In addition to the rib the potential exists to include a cartilaginous growth centre, a functional muscle and/or overlying skin. A latissimus dorsi musculocutaneous unit can be included on a common pedicle. This paper describes its use as both a free and a pedicled composite unit in mandibular reconstruction. The surgical anatomy of the flap is reviewed and cases are shown to illustrate its use in the treatment of congenital, malignant and post-t

Use of extended vertical lower trapezius island myocutaneous flaps to cover exposed reconstructive plates.

PURPOSE: Plate exposure is the most common complication after reconstruction of oncologic resection using a titanium plate. The outcomes of covering exposed reconstructive plates with extended vertical lower trapezius island myocutaneous flaps (TIMFs) were evaluated. PATIENTS AND METHODS: Twelve instances of exposure of reconstructive plates occurred in patients after segmental mandibulectomy to treat cancer of the oral cavity and oropharynx. The plates were covered with extended vertical lower TIMFs. The site of the primary tumor was the gingiva or mandible in 5 cases, the buccal mucosa in 3

[Malignant melanoma metastasis in the oral cavity].

Malignant melanoma occurs only rarely as a primary tumour in the oral cavity, with an incidence of 1-2 percent. Oral melanomas are predominantly to be found in the hard and soft palate, and less often in the gingiva and mandible. Mucosal malignant melanomas are much more aggressive than those situated in the skin. In two-thirds of the cases the route of formation of the metastases is lymphogenic, and haematogenic in the remainder. The typical sites of the distant metastases are the skin, the lungs, the brain, the liver, and the bones. Metastatic malignant melanoma in the oral cavity, a rarity

Microvascular free tissue transfer for reconstruction of head and neck cancer defects.

Over the last 10 years, microvascular free tissue transfer techniques have broadened the range of the head and neck surgeon, allowing for successful reconstruction of extensive curative extirpations in one operation with minimal morbidity. Success rates of more than 90% are now being achieved consistently. This article provides an overview of these revolutionary techniques for oral cavity, mandibular, and hypopharyngeal reconstructions. Patients are selected for microvascular free tissue transfer because of the large size of their defect, its location (eg, anterior mandibular loss), or complex

Malignant fibrous histiocytoma of the gingiva. A clinicopathological study.

A case is reported of malignant fibrous histiocytoma (MFH) of the gingiva. The 65-year-old female presented with a fast-growing mass originating from the left lower gingiva. The tumor measured 15 cm in diameter, occupying the entire oral cavity, protruding outside, and causing a mechanical airway obstruction. It appeared to respond to a 1000 rad single dose of preoperative radiation but recurred soon after composite resection with mandibulectomy, even though surgical margins were reportedly free from microscopic invasion. The patient expired from massive lung metastasis in spite of appreciable

[Mesenchymal chondrosarcoma--report of 4 cases].

This paper reports 4 cases of mesenchymal chondrosarcoma, 1 case in the retroperitoneum, 1 the left mandible, 1 right thoracic wall and 1 right thigh. 2 males and 2 females. The age ranged from 18 to 42 with an average of 29 years. In this series, 3 patients are still alive after operation and one died one year postoperatively. Microscopic examination: the tumor was made up not only of undifferentiated mesenchymal cells, but also of well-differentiated cartilaginous islands and hemangiopericytomalike lesions. Transitions between the undifferentiated small cells and neighbouring chondrocytes or

[A new case of basal cell naevomatosis (author's transl)].

The authors, in the light of a new case of naevomatosis, review the description of the disease, emphasising its symptoms and signs, and inheritance, drawing attention to the need for surveillance of such patients in view of the risk of malignant change in the naevi. The other signs of the disease must be sought following the discovery of multiple epidermal cysts of the maxilla and mandible.

Biology of metastasizing ameloblastoma.

The present report of a malignant metastasizing ameloblastoma and a critical review of literature was undertaken in an attempt to better understand the biological potential and behavior of this rare tumor and thus to facilitate its clinical management. Most of the 26 patients with a proven malignant ameloblastoma including the present case had developed multiple recurrences. The lung was the most frequent metastatic site (88%) followed by regional lymph nodes (27%). Furthermore metastases were observed in some cases in the bone, brain, kidney, small intestine and liver. The interval between di

Intraosseous pleomorphic adenoma of the mandible: A case report and literature review.

RATIONALE: Salivary gland tumors account for 3% to 6% of all head and neck tumors, with pleomorphic adenoma (PA) being the most common benign type. However, primary intraosseous PAs arising from ectopic salivary tissue within the jawbones are exceptionally rare, with only a handful of cases reported in the literature. Clinically and radiographically, these lesions often mimic odontogenic pathologies, posing significant diagnostic challenges. This rarity and diagnostic ambiguity underscore the need to document and analyze such cases to improve clinical recognition and differential diagnosis. P

A rare case of secondary ameloblastic carcinoma in a young Man.

Ameloblastic carcinoma is a rare and aggressive malignant odontogenic tumour that can arise de novo or from a preexisting benign lesion. It most frequently involves the mandible, and its clinical course is aggressive with extensive local destruction. Although rare, these lesions have been known to metastasize, mostly to regional lymph nodes or lungs. Surgical therapy, eventually followed by radiotherapy, is the treatment modality most frequently used, while the role of chemotherapy remains unclear. Here we present a case of secondary ameloblastic carcinoma of the mandible in a 33-year-old male
