Rhino-orbital-cerebral mucormycosis.

This review focuses on sinus, sino-orbital, and rhinocerebral infection caused by the Mucorales. As the traditional term of "rhinocerebral" mucormycosis omits the critical involvement of the eye, the more comprehensive term as rhino-orbital-cerebral mucormycosis (ROCM) is used. The most common underlying illnesses of ROCM are diabetes mellitus, hematological malignancies, hematopoietic stem cell transplantation, and solid organ transplantation. Sporangiospores are deposited in the nasal turbinates and paranasal sinuses in immunocompromised patients. Qualitative and quantitative abnormalities o

Trigeminal herpes zoster: early recognition and treatment are crucial.

Reactivation of varicella zoster virus (VZV) is not uncommon in older patients, particularly in cases of chronic autoimmune disorders and in patients taking immunosuppressant drugs. We present a case of a 57-year-old woman presenting with severe herpes zoster infection, involving the maxillary and ophthalmic branches of the trigeminal nerve. Despite an initial delay in instigating crucial antiviral treatment, the patient achieved an excellent recovery, with only some mild scarring at 2 months postinfection. Trigeminal herpes zoster is a potentially devastating clinical occurrence, and is assoc

Magnetic resonance evidence of perineural metastasis.

Contiguous spread along perineural and endoneural spaces, that is, perineural tumor extension, in cutaneous squamous cell carcinoma is fairly common. Infrequently, these tumors spread and involve intracranial structures. One consequence of this complication is meningeal carcinomatosis which is underrecognized. Herein described is a patient with recurrent cutaneous squamous cell carcinoma with perineural invasion along the maxillary nerve that was subsequently shown by magnetic resonance imaging to the trigeminal root. The patient initially presented with a cavernous sinus syndrome but despite …

Severe Craniofacial Morbidity from a Bean Bag Projectile.

Bean bag munitions, less-lethal weapons primarily used by law enforcement, can cause severe morbidity. Although bean bag munitions are less likely to cause severe injury when compared with regular gunshot rounds, it is crucial to understand that bean bag munitions may be life-threatening. In this case, we describe our experience with a patient who suffered a zygomaticomaxillary complex fracture, facial nerve injury, and retained munition round in his maxillary sinus from a bean bag projectile shot during an altercation with the police. The patient was urgently transferred to the operating room

Intraoperative complications during oral implantology.

Dental implant placement is a controlled, programmed surgical procedure, not without its complications. The aim of the present paper is to study the intraoperative complications in implant surgery, carrying out a review of articles appearing in Medline over the last 10 years. Among the intra operative complications related with surgery, hemorrhagic accidents occur most frequently in the interforaminal region, since the majority of the vascular branches enter the mandibular bone in this region. Nerve damage can arise as a consequence of inferior alveolar nerve transposition or lateralization; o

Unilateral anomalous left common carotid artery; a case report.

An anomaly of the left common carotid artery was observed in a Japanese male cadaver during an anatomy class at the Saitama Medical School in 1995. The superior thyroid, lingual and facial arteries arose from the common carotid artery, and the posterior auricular, maxillary and superficial temporal arteries arose from the common carotid artery by a common trunk. The occipital and ascending pharyngeal arteries arose from the internal carotid artery. The left carotid body (glomus caroticum) was observed to be slightly below the lingual artery, behind the common carotid artery, and it was located

Herpes zoster in HIV infection with osteonecrosis of the jaw and tooth exfoliation.

BACKGROUND: Herpes zoster (HZ) infection of the trigeminal nerve is associated with complications such as postherpetic neuralgia, facial scarring, loss of hearing ability and conjunctivitis. Until 2005, postherpetic alveolar necrosis and spontaneous tooth exfoliation have been described in 20 cases unrelated to HIV infection. OBJECTIVE: The aim of this study was to describe HIV infection in patients (two women, two men, average age 30 years) who suffered from HZ attacks to their trigeminal nerves. MAIN OUTCOME MEASURES: None of the patients had received antiherpetic medications or antiretrov

Three-Dimensional Visualization of Developing Neurovascular Architecture in the Craniofacial Region of Embryonic Mice.

Recent studies have highlighted the mechanism of vascular and axonal guidance to ensure proper morphogenesis and organogenesis. We aimed to perform global mapping of developing neurovascular networks during craniofacial development of embryonic mice. To this end, we developed histology-based three-dimensional (3D) reconstructions using paraffin-embedded serial sections obtained from mouse embryos. All serial sections were dual-immunolabeled with Pecam1 and Pgp9.5/Gap43 cocktail antibodies. All immunolabeled serial sections were digitized with virtual microscopy to acquire high spatial resoluti

Differential Changes in TRPV1 expression after trigeminal sensory nerve injury.

UNLABELLED: We have recently demonstrated that inferior alveolar nerve and mental nerve (branches of the mandibular nerve) injury from rats serves as a valid trigeminal neuropathic pain model. In these animals, we found that neuronal loss of trigeminal ganglion (TG) was not correlated with pain hypersensitivity. In this study, we examined changes of transient receptor potential vanilloid 1 (TRPV1) expression in the injured and uninjured TG neurons using immunohistochemical analysis at 3 days after surgery, the time point where we observed significant pain hypersensitivity. Injured neurons were

Expression of Galpha proteins in the developing, denervated, or injured rat molar tooth.

The purpose of this study was to map the distribution of alpha-subunits of G-proteins--Galpha(olf s), Galpha(olf), Galpha(s), Galpha(i), Galpha(o), Galpha(z) and Galpha(q11)--in developing, denervated or injured rat molar teeth, using fluorescence microscopic immunohistochemistry coupled with immunogold electron microscopic immunocytochemistry. In rat fetuses (E17-E21), a widespread expression of Galpha(q11) was seen in maxillary/mandibular mesenchyme as well as in developing teeth. In addition, intensely Galpha(o)-positive nerve fibers were associated with the dental epithelium and the dental

Cone-Beam CT Morphometry of the Foramen Rotundum and Adjacent Skull-Base Landmarks: Associations With Sphenoid Sinus Pneumatization.

BACKGROUND: The foramen rotundum (FR) is a key landmark in endoscopic skull-base surgery, transmitting the maxillary nerve (V2) from the middle cranial fossa to the pterygopalatine fossa. Its anatomical relationships may be influenced by sphenoid sinus pneumatization, affecting surgical corridors. METHODS: This retrospective CBCT study analyzed 50 adult patients (100 sides). Bilateral morphometric measurements of the FR and its relationships with adjacent landmarks, including the vidian canal (VC), foramen ovale, optic nerve, anterior clinoid process, and skull-base midline, were obtained. FR…

Inadvertent injection of formalin mistaken for local anesthetic agent: report of a case.

Chemical facial cellulitis, while commonly seen in domestic accidents or attempted suicide, is uncommon in the dental office and hence rarely addressed in the dental literature. We present an unusual case of chemical facial cellulitis caused by inadvertent injection of formalin into the soft tissues of the oral cavity, which was mistaken for local anesthesia solution. This report comprises the immediate symptoms, possible root cause, and management of the difficult situation. We also provide some guidelines to avoid such unfortunate events.
