Topical application of mitomycin C before versus after myringotomy: an experimental study.

The objective of this animal study was to compare topical mitomycin C (MMC) before and after nonlaser myringotomy. Bilateral myringotomies were performed in 12 rats. On the left side, MMC was applied to the intact tympanic membrane for 10 min before myringotomy (preapplication group). On the right side, MMC was applied to the tympanic membrane following myringotomy. Tympanic membranes were observed for a total of 8 weeks. By day 35, all myringotomies with application of MMC after perforation of the tympanic membrane were closed, whereas 42% of the myringotomies with application of MMC prior to

[Peculiarities of the treatment of the patients presenting with exudative otitis media].

The objective of the present study was to enhance the effectiveness of provision of medical assistance and to standardize the approaches to its realization for the patients presenting with exudative otitis media (EOM). A total of 67 children (103 cases) at the age varying from 11 months and 17 years were available for the examination after the surgical treatment. The secretory phase of exudative otitis media was diagnosed in 23.5% of the children, mucous phase in 68%, and fibrous phase in 8.5%. The results of the study indicate that diagnostics of EOM requires the application of such methods a

Resorption rate and biocompatibility characteristics of two polyester ventilation tubes in a guinea pig model.

OBJECTIVES: Determine the resorption rate and biocompatibility characteristics of 2 polyester ventilation tubes, and to determine whether soap and water exposure accelerates polyester tube degradation. STUDY DESIGN AND SETTING: 50/50 poly (D, L-lactide-co-glycolide; PLGA-50) and poly (L-lactide; PLA) polymers were placed into the tympanic membranes of Hartley pigmented guinea pigs. Integrity of the tubes was determined by weekly otoscopic examination. Biocompatibility was assessed by comparing auditory brainstem response (ABR) thresholds and by examining tympanic membrane changes following tu

A Case of Bilateral Congenital Aural Stenosis with Down Syndrome Underwent Canalplasty with Underwater Transcanal Endoscopic Ear Surgery.

Congenital aural stenosis (CAS) sometimes can be a risk for external auditory canal (EAC) cholesteatoma and may require surgical treatment. We report a child of Down syndrome with bilateral CAS who underwent bilateral canalplasty with underwater transcanal endoscopic ear surgery (TEES). Case Report: A 12-year-old boy with bilateral EAC stenosis, in whom tympanostomy tube insertion for secretory otitis media (SOM) had failed at 5 and 8 years of age, was referred to our department for surgical treatment. The bilateral EACs were so narrow and curved that the tym-panic membranes could not be entir

Creation and evaluation of 3D printed compostable myringotomy sets: A sustainable proof of concept.

BACKGROUND: Single-use disposable medical supplies contribute significantly to the healthcare sector's carbon footprint. While most myringotomy sets are reusable, there has been a trend towards single-use disposable kits. No published research investigates the creation of surgical supplies using compostable biopolymers, a possible mitigation strategy for single-use disposable waste. Polyhydroxyalkanoates (PHAs), biopolymers synthesized through bacterial fermentation, are compostable at a non-industrial level. This study presents a novel, fully compostable 3D printed myringotomy set as a proof-

Balloon Eustachian Tuboplasty Combined With Tympanocentesis Is not Superior to Balloon Eustachian Tuboplasty in Chronic Otitis Media With Effusion-A Randomized Clinical Trial.

OBJECTIVE: Balloon Eustachian tuboplasty (BET) appears to be a promising therapeutic option for Eustachian tube dysfunction. However, data are lacking on its effect in adults with chronic otitis media with effusion (COME) and whether it should be combined with tympanocentesis. The aim of our study was to determine if there is a beneficial effect of BET combined with tympanocentesis compared with BET only and provide data on the effect of BET in adults with COME. STUDY DESIGN: Randomized clinical trial. SETTING: Tertiary referral hospital. PARTICIPANTS: Adults with COME lasting more than 6 m

The History of Myringotomy and Grommets.

The first recorded myringotomy was in 1649. Astley Cooper presented 2 papers to the Royal Society in 1801, based on his observations that myringotomy could improve hearing. Widespread inappropriate use of the procedure followed, with no benefit to patients; this led to it falling from favor for many decades. Hermann Schwartze reintroduced myringotomy later in the 19th century. It had been realized earlier that the tympanic membrane heals spontaneously, and much experimentation took place in attempting to keep the perforation open. The first described grommet was made of gold foil. Other materi

WITHDRAWN: Grommets (ventilation tubes) for recurrent acute otitis media in children.

BACKGROUND: This is an update of a Cochrane review first published in The Cochrane Library in Issue 4, 2008 and previously updated in 2011.Acute suppurative otitis media is one of the most common infectious diseases in childhood. Recurrent acute otitis media is defined for the purposes of this review as either three or more acute infections of the middle ear cleft in a six-month period, or at least four episodes in a year. Strategies for managing the condition include the assessment and modification of risk factors where possible, repeated courses of antibiotics for each new infection, antibio

Tympanostomy Tube Placement in Children Using a Single-Pass Tool with Moderate Sedation.

Insertion of tympanostomy tubes is a common elective pediatric surgical procedure and is typically performed under general anesthesia. The potential to reduce general anesthetic requirements for young children has led to increased interest in alternatives for tympanostomy tube placement. A tympanostomy tube system, developed to enable tympanostomy tube placement in a single pass on conscious patients under moderate sedation, was evaluated. A prospective study on 128 children and 253 tympanostomy tube placements conducted at 4 centers in the United States demonstrated an 88.3% success rate in p

What does tympanostomy tube placement in children teach us about the association between atopic conditions and otitis media?

Otitis media is the most common infection second only to viral upper respiratory infection in the outpatient setting. Tympanostomy tube insertion (TTI) is the most common ambulatory surgical procedure in the USA. While many risk factors for otitis media have been identified, atopic conditions have been underrecognized as risk factors for recurrent and persistent otitis media. Given that asthma and other atopic conditions are the most common chronic conditions during childhood, it is worth examining the association between atopic conditions and risk of otitis media, which can provide insight in

Section two: otitis media.

With the widespread use of vaccinations against Streptococcus pneumoniae, the frequency of acute otitis media (OM) has decreased significantly in recent years. Current management varies considerably in different countries, but there is consensus that most mild cases in children older than 6 months can be treated with observation and analgesics. When antibiotic therapy is indicated for children younger than 6 months or older children with severe or unrelenting symptoms, initial management with amoxicillin or amoxicillin-clavulanate is recommended. For children with recurrent OM or persistent se
