New molecular targets in mantle cell lymphoma.

Mantle cell lymphoma (MCL) is a malignancy of mature B cells characterized by aberrant expression of cyclin D1 due to the translocation t(11;14). Epigenomic and genomic lesions in pathways regulating B-cell activation, cell cycle progression, protein homeostasis, DNA damage response, cell proliferation and apoptosis contribute to its pathogenesis. While patients typically respond to first-line chemotherapy, relapse is the rule resulting in a median survival of 5-7 years. The PI3K/AKT/mTOR appears as a key pathway in the pathogenesis and can be targeted with small molecules. Most experience is

Preventing asthma exacerbations: what are the targets?

Exacerbations of asthma are the main cause of asthma morbidity. They induce acute respiratory failure, and sometimes death. Two immunological signals acting in synergy are necessary for inducing asthma exacerbations. The first, triggered by allergens and/or unknown agents leads to the chronic Th2 inflammation characteristic of asthma. The second, caused by either viral infection, allergens, pollutants or a combination of these, results in an acute Th1 and Th2 inflammation precipitating symptoms. In both, innate and adaptive immunities are involved, providing a series of potential targets for t

Research Progress on Targeted Therapies for Pemphigus Vulgaris and Bullous Pemphigoid: Mechanisms, Opportunities and Challenges.

Autoimmune bullous diseases (AIBDs) are a group of severe disorders characterized by autoantibodies that attack structural proteins within the skin/mucosa, leading to blister and bullae formation. Pemphigus vulgaris (PV) and bullous pemphigoid (BP) are among the most prevalent representative AIBDs. Traditional treatments involving corticosteroids and immunosuppressive agents are often associated with substantial adverse effects and a high rate of relapse. In recent years, the therapeutic landscape has evolved toward precision targeted therapies, encompassing biological agents, small-molecule d

Thymic epithelial tumors: what's next?

Thymic epithelial tumors (TETs) are rare malignancies arising in an organ central to T cell development and immune tolerance, creating a distinctive interface between tumor biology and immunotherapy. Complete surgical resection remains the mainstay for resectable disease, whereas systemic treatment options for unresectable, recurrent, and metastatic TETs remain limited. The unique immunobiology of thymoma, particularly impaired central immune tolerance, may account for the unusual balance between antitumor activity and severe immune-related adverse events observed with immune checkpoint blocka

ADCs for colorectal carcinoma: decoding clinical evidence for molecular design innovation.

Antibody-drug conjugate (ADC) is a novel type of targeted systemic therapy that is changing the current landscape of tumor treatment. By integrating the specificity of antibodies with the cytotoxic effects of their payloads, ADCs facilitate precise tumor killing. In solid tumors such as breast cancer, non-small cell lung cancer (NSCLC), and urothelial carcinoma, ADCs have demonstrated significant efficacy with acceptable toxicity profiles. Colorectal cancer (CRC) ranks as the third most prevalent tumor globally, and there remains an unmet clinical need for targeted treatment options. The appro

Targeted therapeutic strategies in rare subtypes of pancreatic cancer: Histology, molecular profiles, and emerging opportunities.

Pancreatic cancer remains one of the deadliest malignancies, with a 5-year survival below 13% and limited response to standard therapies. Conventional pancreatic ductal adenocarcinoma (PDAC), which accounts for over 90% of cases, dominates the landscape. Yet rare subsets of histological and molecular variants - such as medullary, colloid, intraductal papillary mucinous neoplasm-associated, and acinar cell carcinomas - have distinct biology and actionable alterations. These tumors are more frequently KRAS wild-type and are enriched for homologous recombination deficiency, DNA mismatch repair de

Unraveling giant cell arteritis: From immunopathology to emerging targeted therapies (2026 update).

Giant cell arteritis (GCA) is the most common systemic vasculitis in the elderly mainly because of cellular senescence and "inflammaging" It results from granulomatous inflammation primarily affecting the aorta and its major branches. Recent advances highlight vascular dendritic cell (DC) activation via TLRs as the initiating event, leading to chemokine-mediated recruitment and polarization of CD4+ T cells mainly into Th1/Th17 subsets. These effector T cells drive monocyte differentiation into pro-inflammatory macrophages and giant cell formation, with subsequent matrix metalloproteinase produ

Targeting Integrins in Immunity: Precision Matters for Therapeutic Interventions.

Integrins are transmembrane receptors that mediate interactions between the extracellular matrix (ECM) and cells. Integrins are expressed as heterodimers composed of an α-subunit and a β-subunit. Based on several α-subunits and β-subunits present in mammals, each integrin receptor is functionally distinct with specific ligand-binding and signalling features. Upon activation, integrins regulate diverse cellular functions via the formation of multi-protein assemblies with intracellular signalling proteins at the cell surface. However, integrin dysfunction or aberrant activation is also associate

An Update Comprehensive Review on Epigenetic Modifications and Therapies in Autoimmune Diseases.

Autoimmune diseases are chronic disorders characterized by dysregulated immune responses directed against self-tissues. However, current therapies are limited by suboptimal efficacy, substantial adverse effects, or an inability to halt or reverse disease progression. Emerging evidence indicates that epigenetic regulation—including DNA methylation, histone modification and non-coding RNA—has a central role in the pathogenesis of autoimmune diseases, creating transformative opportunities for the development of targeted therapies. This review provides an up-to-date synthesis of advances in epigen

The immune landscape of systemic sclerosis: from pathogenic mechanisms to precision therapeutic breakthroughs.

Systemic sclerosis (SSc) is a chronic autoimmune disease characterized by immune dysregulation, microvascular damage, and multi-organ fibrosis. Recent breakthroughs in single-cell and spatial multi-omics technologies have profoundly revealed the high heterogeneity of the SSc immune microenvironment, including extensive aberrant activation of innate immunity (e.g., dendritic cells, macrophages, neutrophils) and adaptive immunity (T cells, B cells), and their interaction with fibroblasts and endothelial cells through an "immune-stromal-vascular" network that collectively drives the fibrotic proc

From Barrier Disruption and Immune Dysregulation to Targeted Therapies in Atopic Dermatitis.

Atopic dermatitis (AD) is a heterogeneous inflammatory skin disorder characterized by recurrent eczematous lesions and persistent pruritus. Its pathogenesis involves epidermal barrier dysfunction, immune dysregulation, microbial dysbiosis, and altered neuroimmune signaling. Genetic defects in structural proteins, particularly filaggrin, and altered epidermal lipids increase allergen and microbial penetration, perpetuating itch, scratching, tissue injury, and inflammation. Molecular evidence further reveals AD endotypes beyond conventional clinical phenotypes, with heterogeneity in immune polar

Systemic therapy of biliary tract cancer: The road to personalised strategies.

Biliary tract cancers (BTC), including cholangiocarcinoma, gallbladder cancer, and ampullary cancer, remain poor-prognosis tumours. Chemo-immunotherapy is now established as the first-line standard of care. In parallel, precision medicine is now well established in the form of targeted therapies. These have transformed outcomes for defined subgroups, particularly intrahepatic cholangiocarcinoma with FGFR2 fusions/rearrangements, where pemigatinib and futibatinib provide meaningful response rates and durable disease control. On top of this, next-generation FGFR inhibitors aim to address seconda
