Diabetic ketoacidosis complicated by emphysematous pyelonephritis: a case report and literature review.

BACKGROUND: The management of emphysematous pyelonephritis (EPN) includes conservative medical treatment, percutaneous drainage, and surgical resection of the involved kidney. EPN with diabetic ketoacidosis(DKA) is very rare, in which the clinical management of refusing surgical drainage is inexperienced. CASE PRESENTATION: A 34-year-old woman presented with abdominal pain, chills, fever, nausea, vomiting, chest tightness, and shortness of breath. Blood test results were consistent with diabetic ketoacidosis. Urinary computed tomography scan showed multiple stones in the right kidney and lowe

Comparison of Prone and Supine Positions for Suction Mini Percutaneous Nephrolithotomy (PCNL) for Kidney Stone Disease. Results from a Prospective Multicenter Series from the Endourology Section of the European Association of Urology and the Suction Mini-PCNL Collaborative Study Group.

BACKGROUND AND OBJECTIVE: The optimal patient position for percutaneous nephrolithotomy (PCNL) remains a matter of debate. Our aim was to evaluate the association between prone versus supine positioning and perioperative and postoperative outcomes of suction mini-PCNL. METHODS: In this prospective multicenter observational study, we analyzed data for 1534 patients treated in 30 centers between March and November 2024. Outcomes included the stone-free rate (SFR) assessed via 30-d computed tomography, and complication rates. Multivariable analysis was used to assess the effect of prone position

Frequency and indications for double-J stenting in endourology: a review by the EAU section of endourology.

BACKGROUND: Double J stents (DJS) are vital tools in endourological surgery and have a variety of indications. Their main function is to facilitate urinary drainage. Despite their many benefits, they do have drawbacks, chiefly their effect on quality of life (QoL). With the ongoing development of DJS, as well as ever-changing guidance regarding their placement, decision making regarding DJS can be difficult for clinicians to navigate. We therefore give an up-to-date summary of DJS indications and summarise the data concerning their frequency of insertion. METHODS: A narrative review of the lit

Laparoscopic simple nephrectomy after previous ipsilateral open versus percutaneous renal surgery.

BACKGROUND AND OBJECTIVES: Fibrosis after previous open or percutaneous renal surgery may interfere with ipsilateral laparoscopic nephrectomy. We prospectively compared the outcome of laparoscopic nephrectomy in patients with previous open renal surgery or percutaneous nephrolithotomy. PATIENTS AND METHODS: During the study period, 38 patients with previous ipsilateral open renal surgery (n 22) or percutaneous nephrolithotomy (n 16) who underwent transperitoneal laparoscopic nephrectomy were evaluated. All patients had symptomatic nonfunctioning small or hydronephrotic kidneys. RESULTS: Mean

Endourological Options for Small (< 2 cm) Lower Pole Stones - Does the Lower Pole Angle Matter?

PURPOSE OF REVIEW: Small renal stones in the lower pole are often difficult to treat. The angle of the lower pole to the renal pelvis (lower pole angle) is a limiting factor to rendering the patient stone free. This review explores the definitions of the lower pole angle, the various treatment options available, and how outcomes are influenced by the angle. RECENT FINDINGS: It is clear the lower pole angle definition varies widely depending on described technique and imaging modality. However, it is clear that outcomes are worse with a steeper angle, especially for shock wave lithotripsy and

Single session endoscopic management of intrinsic ureteropelvic junction obstruction and concomitant renal stone disease in a child: a case report.

BACKGROUND: Percutaneous nephrolithotomy is a well known therapeutic modality for stone diseases of childhood. Antegrade and retrograde endopyelotomies are also well defined options of treatment for secondary ureteropelvic junction obstruction. Yet there are few reports regarding endoscopic therapy of intrinsic ureteropelvic junction obstruction. To our knowledge, there exist only a few reports of endosurgical treatment of children with stone disease and with concomitant intrinsic ureteropelvic junction obstruction, in the literature. CASE PRESENTATION: We present the endoscopic management of

Endoscopic Stone Removal.

Endoscopic stone management consists of ureteroscopy and percutaneous nephrolithotomy, 2 robustly developed techniques in which the surgeon directly visualizes kidney stones to facilitate removal. Ureteroscopy is performed in retrograde fashion and can be used to clear smaller stones from the ureter and renal collecting system. Percutaneous nephrolithotomy involves percutaneous renal access, guided by either ultrasound or fluoroscopy, to identify, fragment, and remove large renal stones. A key aspect to performance of these 2 procedures is careful preoperative work-up, including urine culture,

Vacuum-assisted versus conventional percutaneous sheaths in pediatric supine mini-PCNL: a comparative analysis.

Vacuum-assisted access sheaths have recently gained interest in percutaneous stone surgery; however, data regarding their use in pediatric patients remain limited. This study compared the perioperative and postoperative outcomes of vacuum-assisted and conventional percutaneous access sheaths in pediatric patients undergoing supine mini-percutaneous nephrolithotomy. Medical records of children younger than 18 years who underwent supine mini-percutaneous nephrolithotomy between January 2022 and January 2026 were retrospectively reviewed. Patients with anatomical anomalies, bilateral procedures,

Small caliceal stones: what is the best method of treatment?

In a retrospective analysis classical radial nephrolithotomy, percutaneous nephrolithotomy and extracorporeal shock wave lithotripsy for symptomatic small nonobstructive caliceal stones were compared to evaluate morbidity. Size (less than 1 cm.) and location of the stone (caliceal and nonobstructive) did not explain the severity of the symptoms nor would they have been an indication for an operation. In 8 patients treated by an open operation, 16 treated percutaneously and 6 who underwent extracorporeal shock wave lithotripsy the procedure time, success rate, complication rate and length of ho…

Open stone surgery in the treatment of bilateral complex renal stones with left infected hydronephrosis: A case report.

INTRODUCTION AND IMPORTANCE: With the strong development of percutaneous nephrolithotomy as well as other less invasive procedures, the indication of open surgery for the treatment of nephrolithiasis has been significantly reduced and is only applied in selective cases. CASE PRESENTATION: A 55-year-old male was admitted to the Department of Urology due to fever and left flank pain. Clinical examination and imaged studies reveal staghorn calculi in the right kidney and infected hydronephrosis with multiple stones on the left side. The stone removal surgery was performed in 2 sessions, left fir

[Stones in the lower pole and their treatment].

The most frequent initial clinical symptom is flank pain, which occurs in approximately half of affected patients with stones of the upper urinary tract. Of these patients approximately half require treatment and 50% will again have new stones in the future. Only 8% of urinary stones are clinically inapparent with 25-50% of the stones located in the lower calyx group and are incidentally discovered during the diagnostics of symptomatic stones. Stones in the lower calyx are more difficult to treat than other kidney stones due to the localization and the resulting access possibilities. The treat

A Major Complication in Micropercutaneous Nephrolithotomy: Upper Calyceal Perforation with Extrarenal Migration of Stone Fragments due to Increased Intrarenal Pelvic Pressure.

Micropercutaneous nephrolithotomy is a safe and efficient technique for appropriate sized stones. It is performed through a 4.85 Fr all-seeing needle and stones are fragmented into dust, without the need for tract dilatation, unlike other percutaneous nephrolithotomy types. Even though micropercutaneous nephrolithotomy has many advantages, increase in intrapelvic pressure during surgery may cause rare but serious complications. Herein we report a case of micropercutaneous nephrolithotomy in a 20-year-old woman with a 20 mm right renal pelvis stone and present an undesired outcome of this compl

Comparison of renal parenchymal trauma after standard, mini and ultra-mini percutaneous tract dilation in porcine models.

OBJECTIVE: To evaluate whether reducing tract dilation diameter in PCNL (percutaneous nephrolithotomy) procedures results in minimizing of renal trauma of the percutaneous tract. METHODS: A percutaneous renal access tract was established bilaterally to 11 pigs. Two pigs were euthanized immediately after the experiment, while nine pigs were sacrificed 1 month later. The percutaneous accesses were dilated up to 30Fr, 22Fr or 12Fr. The animals underwent a contrast-enhanced computer tomography immediately after the procedure and 30 days later. DMSA-scintigraphy with SPECT-CT was also performed. T

Comparative Analysis of A Novel Air-Bubble Guided Percutaneous Access Technique Against Traditional Fluoroscopic Techniques.

INTRODUCTION: Percutaneous nephrolithotomy (PCNL) requires precise renal access, a feat that is often challenging and time-consuming. In this light, we evaluated the bubble needle, a standard nephrostomy needle with an integrated two-dimensional bubble level designed to maintain anterior-posterior alignment after a single fluoroscopic image. We hypothesized that the bubble needle would streamline access while providing accuracy comparable to conventional fluoroscopic techniques and substantially reduced radiation exposure. METHODS: Twenty-one participants (students = 9, residents = 5, attendi

Nephrostomy tubes placed emergently prior to percutaneous renal stone surgery are practical for obtaining access.

OBJECTIVES: Renal access in percutaneous nephrolithotomy (PCNL) may be obtained via a pre-existing nephrostomy tube (NT) tract; however, emergent NTs are not always ideal for subsequent surgery. We sought to determine the rate of NT tract usability and assess factors related to the usability of emergently placed NTs. METHODS: A retrospective review was performed of UC San Diego subjects undergoing percutaneous renal surgery between January 2016 and October 2018. Demographics and peri-operative variables were collected. The primary outcome was the usability of NT tract for dilation and instrum
