Heterotopic pregnancy is defined as a simultaneously intra- and extrauterine pregnancy. We present a case of a 29-year-old woman, with a history of ectopic pregnancy, which required left salpingectomy, and with a tortuous and impermeable right fallopian tube, compatible with hydrosalpinx. As hydrosalpinx itself can compromise a future pregnancy, treatment with Essure was proposed before passing to medically assisted procreation techniques. Five months after placement of Essure in the right fallopian tube, an in vitro fertilisation cycle was successfully completed and the woman had a singleton pregnancy and vaginal delivery without intercurrences.

We report a mature solid teratoma of the fallopian tube in an elderly woman. This mass was noted on CT scan and considered metastatic in nature since following a bioptical diagnosis of endometrial adenocarcinoma. Hysterectomy and bilateral salpingectomy and ovariectomy were performed and a second minor mature solid teratoma was discovered inside the right ovary. Neoplasms of the fallopian tube are very uncommon and this is the first Italian case to be added to about other 60 of the literature. Moreover it joins minimum teratomatous diameter with maximum recorded age.

We report 2 cases of filariasis, one in the ovary and the other in the mesosalpinx. In the first case, the patient underwent panhystrectomy and in the second case, right ovarian cystectomy with right salpingectomy were performed under general anaesthesia. A case of simultaneous bilateral tubal pregnancy resulting from in-vitro fertilization and embryo transfer is presented. Repeated transvaginal ultrasound examinations confirmed an intrauterine sac but no fetus. A diagnosis of early missed abortion was incorrectly made and a curettage was performed. The pathological examination showed the presence of decidua and Arias-Stella phenomenon but no chorionic villi. Diagnostic laparoscopy and laparotomy performed 40 days after embryo transfer (eighth week of gestation), revealed bilateral tubal pregnancy. Bilateral salpingectomy was performed.

A second tubal pregnancy occurred in the same tube as the first one which had been treated with methotrexate injection. The hysterosalpingogram performed 3 months after the methotrexate injection was normal. The recurrent tubal pregnancy was treated by laparoscopic salpingectomy. The pathology report confirmed non-ruptured tubal pregnancy. No damage to the tube could be imputed to the methotrexate injection. This observation confirms that methotrexate has no deleterious effect on tubal epithelium but also emphasizes that recurrent ectopic pregnancy can occur despite a patent tube.

The authors evaluated 3 cases of acute abdomen during pregnancy: Spontaneous rupture of splenic artery at 30th gestational week (gw). Torsion of left normal Fallopian tube at 33rd gw. Left adnexal torsion at 36th gw. The symptomatology, the etio-pathology and the anatomo-pathological characteristics of every case wore discussed. In women that are usually young, maternal and fetal exitus in the first case and salpingectomy or adnexectomy in the second and third case can be avoided only thanks to timely surgical intervention.

A case is reported of a gestational choriocarcinoma arising in a tubal ectopic pregnancy of 7 weeks' gestation. Unilateral salpingectomy and single-agent actinomycin-D chemotherapy achieved a cure. A follow-up 4 years later revealed no evidence of recurrence, with two normal births within that period. The current trend to treat ectopic pregnancy by conservative surgery necessitates monitoring of beta-HCG titres in order to avoid missing a choriocarcinoma developing in an ectopic gestation, even though this is a very rare condition.

BACKGROUND: The effect of perforated appendicitis on the adnexa is an issue of concern and controversy. Long-term fertility studies have been conflicting.
CASE: We present the case of a patient with chronic pelvic infections, salpingitis, and hydrosalpinx after perforated appendicitis.
SUMMARY AND CONCLUSION: Magnetic resonance imaging was helpful in diagnosing a chronically obstructed fallopian tube, likely secondary to the dense adhesions from her previously treated perforated appendicitis. Salpingectomy relieved her symptoms of chronic pain and recurrent infections.
