Previous tuberculosis, hepatitis C virus and lichen planus. A report of 10 cases, a causal or casual link?

We report 10 cases of lichen planus (LP) and chronic liver disease linked to HCV. The mean age was 63.4 +/- 5.1 years (range 51-73), five were female; six patients had an established cirrhosis of the liver, as shown by either a liver biopsy or the ultrasonographic and biohumoral evidence. The remaining four patients had chronic hepatitis. Histological examination confirmed the presence of LP: the localization of the dermatosis was restricted to the skin in four patients, to the mucous membranes in five (4 atrophic erosive and one erosive) while the remaining had mucous-cutaneous localization.

[Chinese guideline for the chemotherapy of multidrug-resistant/rifampicin-resistant tuberculosis (2026 edition)].

Multidrug-resistant tuberculosis (MDR-TB) and rifampicin-resistant tuberculosis (RR-TB) pose significant challenges to global tuberculosis prevention and treatment efforts. At present, the chemotherapy of MDR/RR-TB is still hampered by prolonged treatment duration and suboptimal success rates. In 2019, the Chinese Medical Association Tuberculosis Branch issued the Chinese Expert Consensus on the Treatment of MDR-TB and RR-TB, which has provided essential guidance for standardizing the diagnosis, treatment, and clinical management of MDR/RR-TB in China. In response to recent advances in global

Patch Test-Positive Drug Reaction with Eosinophilia and Systemic Symptoms Syndrome Due to Cycloserine in Multidrug-Resistant Tuberculosis: A Case Report with a Review of the Literature.

Antitubercular treatment (ATT) is associated with multiple cutaneous adverse drug reactions. Second-line ATT is also associated with numerous adverse reactions; however, cutaneous reactions are under-reported. Oral drug provocation (ODP) in multidrug-resistant tuberculosis is challenging because of the paucity of time and the risk of developing secondary drug resistance in the case of interrupted medication. In this paper, we report a case of drug reaction with eosinophilia and systemic symptoms caused by cycloserine in an 18-year-old girl, which was confirmed with ODP and a patch test.

Tuberculous osteomyelitis of the mandible with diffuse swelling of the floor of the mouth: a case report.

Primary orofacial tuberculosis (TB) is uncommon, especially with regard to the jaw. We report an unusual case for which the final diagnosis was tuberculous osteomyelitis of the mandible with cervical tuberculous lymphadenitis. The follow-up examinations for our patient showed complete regression of the swelling and healing of the mandibular lesion after 4 months of TB antibiotic therapy. The purpose of the present study was to alert clinicians to our findings and encourage them to consider oral TB in the differential diagnosis for jaw lesions with multiple enlarged cervical lymph nodes.

Calcifications simulating sialolithiasis of the major salivary glands.

Calcifications found in routine and normal radiographs of the oral and facial region may turn out to be an interesting diagnosis. Although most of the radiopacities may be sialoliths, other differential diagnoses must not be exempted, such as calcified lymph nodes, phleboliths and other vascular calcifications, tuberculosis of lymph nodes or of the salivary gland itself. Several cases are presented which simulated pathological calcifications of the orofacial region and were diagnosed primarily as sialoliths, but further examinations revealed other pathology. These cases together with several s

Severe neuro-Behcet's disease treated with a combination of immunosuppressives and a TNF-inhibitor.

Abstract/ Resumo Behcet's disease (BD) is a multisystem inflammatory disorder characterized by recurrent oral and genital ulcers, skin lesions and uveitis. The nervous system involvement of BD, neuro-Behcet's disease (NBD), is one of the important causes of mortality of the disease. Herein, we present a 29-year-old male with parenchymal NBD who has progressed rapidly and was managed with an uncommon aggressive immunosuppresive combination therapy. The patient first presented six years ago with vertigo and difficulty in talking and walking. On examination, he had oral ulcers, acneiform lesions

Chronic cavitary pulmonary aspergillosis as a sequela of pulmonary tuberculosis: A case report from Pakistan.

Chronic pulmonary aspergillosis is a lung disorder characterized by the presence of single or multiple cavities with or without an aspergilloma or nodules on chest imaging, with mycological evidence of and/or demonstration of immunological response to Aspergillus spp. The affected patient should manifest relevant symptoms for at least 3 months. Chronic cavitary pulmonary aspergillosis is the most common subset of chronic pulmonary aspergillosis, which is often reported in patients previously treated for pulmonary tuberculosis, having residual cavities in their lungs. We present a case of a 55-

Recent advancements in drug development for pulmonary tuberculosis.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), is a global health threat. Despite advances in diagnostics and treatment regimens, it infects millions annually. It remains a significant global health challenge, particularly due to the appearance of resistance towards anti-tuberculosis drugs (ATD), mainly multidrug-resistant (MDR) and extensively drug-resistant (XDR) strains. Mtb poses resistance towards the currently used first- and second-line regimen. Recent advancements in drug development have introduced therapeutic options focussed on improving treatment outcomes for both d

HIV-associated drug-resistant TB: expanded treatment options and emerging threats.

PURPOSE OF REVIEW: To summarize recent advances in drug-resistant tuberculosis (DR-TB) treatment for people with HIV (PWH), including drug-drug interactions, investigational medications and host-directed therapy, as well as emerging evidence on novel treatment regimens, post-TB complications, and DR-TB medication resistance among PWH. RECENT FINDINGS: Treatment for DR-TB has evolved to shorter, all-oral regimens with reduced drug-drug interactions. However, emerging dolutegravir resistance may necessitate protease inhibitor-based ART regimens resulting in interactions that complicate DR-TB ma

Diagnostic challenges of an atypical presentation of ear and mastoiditis tuberculosis in a-5-year-old child in a developing country: A case report and review of literature.

We report a case of a 5-year-old girl who came to Otolaryngology Outpatient Clinic of our hospital with the chief complaint of recurrent ear discharge which had started to appear since 3 years ago and had recurred every month despite repeated antibiotics and antifungals ear drops treatments, given by general practitioners and otolaryngologists. Gram staining, acid-fast staining and Mycobacterium tuberculosis culture examination from the discharge had never been conducted. There was a history of inadequate weight increment since she was 2 years old. Her uncle who often met her died about 6 mont

Filling the pipeline - new drugs for an old disease.

Tuberculosis is a major global health problem. In the middle of the last century several laboratories identified, developed and synthesized several substances which were active against Mycobacterium tuberculosis, the causative agent of the disease. In the 1980s the standard oral treatment regimen was introduced with isoniazid, rifampicin, pyrazinamide, and ethambutol. In combination with the DOTS strategy it was possible treat TB within 6-8 months. But with the emergence of drug resistant strains, the formerly successful regiment became ineffective for MDR and XDR TB patients. Even more alarmi

Value of antibiotic therapy in tuberculin-positive children with parenchymal lung lesions.

BACKGROUND: A tuberculin-positive child with radiological evidence of a parenchymal lung lesion is likely to be treated for tuberculosis by a physician. However, non-tuberculous microbial infections may also cause parenchymal lung lesions. We tried to distinguish tuberculous from non-tuberculous lung lesions by administering a course of antibiotics. METHODS: Three hundred and five tuberculin-positive children with parenchymal lung lesions due to pneumonia, bronchiectasis (cylindrical and reversible) and minor fissure opacification were studied at the Tuberculosis Clinic, Institute of Child He

Prevalence of and risk factors for methicillin-resistant Staphylococcus aureus colonization among human immunodeficient virus-infected outpatients in Taiwan: oral Candida colonization as a comparator.

Human immuodeficency virus (HIV)-infected patients receiving highly active antiretroviral therapy (HAART) and community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) have increased in recent years in Taiwan. This study was undertaken to determine the prevalence of and risk factors for nasal and oral S. aureus and MRSA colonization among contemporary HIV-infected populations. Clinical variables for S. aureus and MRSA colonization among HIV-infected outpatients from three hospitals were analyzed and compared with those for oral Candida colonization. Genetic characteristics of

Remote Body Temperature Monitoring in Infectious Diseases: Pilot Studies in Different Artiodactyla Models.

Thermal microchips are increasingly used in animals as an alternative to rectal temperature measurements because handling is not required. These pilot studies aimed to determine the usefulness of orally administered thermal microchips in infectious disease studies of artiodactyla species when a temperature increase was expected. Thermal microchips were administered subcutaneously to two groups of seven alpacas infected with influenza C and D virus respectively and three alpacas serving as non-challenged controls and orally to one alpaca in each group. Three sheep exposed to border disease viru

Unilateral gallium-67 uptake in primary tuberculosis of the major salivary glands.

Unilateral radionuclide accumulation in salivary glands is an uncommon finding on gallium scintigraphy. The differential diagnosis includes malignant tumors and inflammatory processes of these organs. Two cases of unilateral gallium uptake of the parotid and submandibular gland respectively, verified as solitary tuberculosis, are presented, together with the correlative findings of Tc-99m pertechnetate scintigraphy, sialography, and sonography. None of these imaging modalities is specific enough to provide pathognomonic signs for tuberculosis. When assessing unilateral gallium uptake of the sa

Isoniazid-induced Pure Red Cell Aplasia in a Patient with Sarcoidosis: A Patient Summary and Review of the Literature.

A 41-year-old woman treated with isoniazid (INH) for latent tuberculosis infection and an oral corticosteroid for sarcoidosis developed severe anemia two months after initiating INH. A bone marrow examination showed erythroblastopenia, and a diagnosis of INH-induced pure red cell aplasia (PRCA) was made. Her reticulocyte count and hemoglobin levels improved two weeks after discontinuation of INH. A literature review of INH-induced PRCA shows that it occurs very rarely in the context of autoimmune disorders. This report describes a case of INH-induced PRCA occurring in a patient with sarcoidosi
