Weight-loss surgery (WLS) patients present a relatively new population for the health care system. Lack of clinical information about the WLS patient and the postoperative WLS patient can negatively affect quality of care and put the patient at unnecessary risk. Postoperatively, the bariatric patient is often admitted with complex care management requirements that differ from those of other surgical patients. Acute and critical care nurses must have a thorough understanding of the health implications of obesity, be familiar with common WLS procedures, and remain vigilant regarding the potentia

Patients complaining of pain in tooth bearing regions of the oral cavity may be suffering from a disorder unrelated to pulpal pathology. Unnecessary dental procedures may complicate the diagnosis and aggravate the underlying disorder. Referral of pain to the oral cavity and teeth may easily be explained by the extensive neurological connections of the trigeminal brain stem complex with other cranial nerves and input from the upper segment of the cervical spine. Accurate diagnosis of painful disorders can be a complex and time consuming process. The primary responsibility of the dentist in this

Behçet's disease is a vasculitis of unknown origin that was traditionally defined by oral and genital ulcers and uveitis. We describe a case of a patient with a diagnosis of Behçet's syndrome who presented a palpable lesion in the right breast with inflammatory signs. X-ray findings posed a differential diagnosis between tumoral and inflammatory pathology. The pathological findings confirmed a small-vessel vasculitis. We found two reports of breast involvement by this disease in the literature. Our patient was studied by mammogram and sonogram which together with clinical history are important

Baker's cysts are rare cause of peripheral nerve entrapment and only a few cases of tibial nerve entrapment resulting from the popliteal cyst in the calf muscle have been reported in the literature. We present a case of rheumatoid arthritis complicated by a Baker's cyst with a tibial nerve entrapment. It is important to diagnose a Baker's cyst early and to differentiate it from thrombophlebitis, a popliteal aneurysm, tumor or muscle tear to effect optimal therapy and to obviate a potential neuropathy. Prompt recognition of these cases may save the patients unnecessary procedures and delay in t…

Three young male patients with chewing pads are reported. Their tic-like habit of chewing on the dorsal aspects over the middle joints of fingers I-IV caused a pad-like thickening of soft tissue. These chewing pads are a variant of "false" knuckle pads. The importance of a medical history and dermatological inspection is stressed to avoid unnecessary diagnostic procedures. This harmless augmentation of soft tissue is commonly confused with rheumatoid disease of the fingers, sarcoidosis or pachydermoperiostosis. Patients should be informed of the true nature of their pads, as discontinuation of

To the great disappointment of all concerned, too many operations performed for pulmonary carcinoma still prove to be exploratory thoracotomies or non-curative resections, in spite of careful preoperative investigations. A follow-up study of 168 patients operated upon for pulmonary carcinoma clearly shows that such non-curative procedures yield very disappointing results. To reduce this number of unnecessary and often harmful operations mediastinoscopy was introduced as an aid in detecting mediastinal spread of the tumour. It was thought that such knowledge would make preoperative assessment m

Male urethral catheterization is a common procedure rarely causing complications. Difficult urethral catheterization, however, can be problematic for both clinician and patient, leading to unexpected complications. This article will present two unreported cases of urethral trauma during difficult catheterization that resulted in the development of a hard, tumor-like mass in the corpus spongiosum. This is a rare complication that if not recognized might lead to unnecessary invasive interventions for the patient.

Sphenochoanal polyp is a rare form of choanal polyp. If unrecognised, they can be mistaken for an antrochoanal polyp. This will result in unnecessary exploration of the maxillary sinus, and a failure to remove the sphenoidal component of the sphenochoanal polyp. Adequate preoperative evaluation with computed tomography or magnetic resonance is mandatory to ascertain the correct diagnosis, and to facilitate the planning of the appropriate surgical procedure. We present two patients with sphenochoanal polyp and a review of the literature.

We report three cases of ipsilateral spinal accessory nerve palsy complicating carotid endarterectomy. Awareness of this cranial nerve injury, as a complication of this common surgical procedure, can lead to early diagnosis and avoids unnecessary investigations. This should be considered whenever such patients complain postoperatively of ipsilateral shoulder pain/weakness or "aching" about the ear, even if some time has elapsed since surgery.

In the context of healthcare reform, Surgery stands at a critical juncture. Attempting to rein in healthcare spending, legislators and payers can be expected to closely examine the legitimacy and necessity of a variety of medical treatments, including surgical procedures. Among these procedures, the most at risk for dismissal based on perceived ineffectiveness or lack of need may be those performed near the end of life, when the potential benefit of surgical intervention may seem negligible. While procedures may be performed for a variety of reasons toward the end of life--some indeed being in

PURPOSE: Combat-related peripheral nerve injuries (CRPNIs) are frequently associated with significant long-term disability. While conventional practice often favors delayed exploration to avoid unnecessary interventions, emerging evidence supports early intervention.
METHODS: We retrospectively reviewed 184 patients (265 CRPNIs) treated during the first ten months of the 2023-2024 Israel-Hamas war. Collected data included demographics, injury details, surgical timing, intraoperative findings, procedures performed, and postoperative complications. Surgical Explorations were considered positive

Invasive arterial line insertion is a common procedure in the intensive care unit ICU; however, it can cause unnecessary blood loss while procuring blood for laboratory tests. To reduce blood loss resulting from flushing out the arterial line dead space, we developed a new blood-preserving arterial line system: Hematic Auto-Management & Extraction for arterial Line (HAMEL, MUNE Corp.). Five male three-way crossbred pigs were used to evaluate the necessary amount of blood to be withdrawn before sampling to produce accurate results. We then tested whether the traditional sampling method and the

Cataract surgeries are among the most common outpatient procedures in the United States, typically accomplished with topical anesthesia and light sedation. Evidence demonstrates no benefit for comprehensive preoperative medical evaluation before low-risk procedures. In 2019, the Centers for Medicare & Medicaid Services (Baltimore, Maryland) revised regulations for hospitals and ambulatory surgery centers, eliminating the previous requirement for a history and physical examination within 30 days of ambulatory surgeries, regardless of risk. Professional guidance from the Society for Ambulatory A

OBJECTIVE: Trigeminal neuralgia (TN) and orofacial pain (OFP) patients frequently refer to dentists. It is often confused with odontogenic pain and dental procedures are performed. In this study, the authors aimed to reveal the knowledge and experience of dentists about TN.
METHODS: This is a cross-sectional study that includes dentists who participate in volunteering via an online questionnaire. The questionnaire form contains demographic data, TN treatment, and diagnosis consisting of 18 questions.
RESULTS: The data of 229 dentists were examined. Almost 82% of the participants reportedly k

BACKGROUND: Tapia syndrome is a rare complication of surgical positioning with resulting unilateral cranial nerve X and XII deficits that may provide diagnostic challenges in the perioperative period. Timely diagnosis will facilitate obtaining the necessary supportive care while preventing unnecessary workup and procedures.
CASE DESCRIPTION: The following case report illustrates a patient that developed Tapia syndrome immediately after a posterior cervical laminoplasty with eventual resolution of symptoms. A review of the literature was also undertaken for comparison.
CONCLUSIONS: Tapia synd

Background. Pelvic inflammatory disease (PID) rarely results in diffuse ascites. Severe adhesive disease secondary to PID may lead to the formation of inclusion cysts and even pelvic peritoneal nodularity due to postinflammatory scarring and cause an elevation of serum CA-125 levels. The constellation of these findings may mimic an ovarian neoplasm. Case. We report a case of a 22-year-old female who presented with multiple pelvic cysts and diffuse ascites due to Chlamydia trachomatis infection. The initial gynecologic exam did not reveal obvious evidence of PID; however, a positive Chlamydia t

Lower gastrointestinal (GI) hemorrhage is a frequently encountered and challenging clinical problem. GI bleeding scans are extremely useful for localizing the source of GI bleeding before interventional radiology procedures. It is essential that physicians understand the numerous possible pitfalls when interpreting these scans. Understanding the potential causes of false-positive scan interpretation eliminates unnecessary procedures for the patient and minimizes costs. We report a rare case of an 8-year-old boy who presented with GI bleeding. Upper and lower GI endoscopy did not reveal a sourc…

The prognostic value and cost effectiveness of generally recommended ancillary tests and staging procedures in 115 patients with clinical Stage IB carcinoma of the cervix were retrospectively reviewed. All 112 intravenous pyelograms, 108 barium enemas, 102 cystoscopies and 98 sigmoidoscopies were normal. No malignant cells were found in pelvic washings. Of 111 patients who had paraaortic lymph node biopsies, only one had a positive node. This node was grossly enlarged and clinically suspicious. Paraaortic lymph node status was not influenced by tumor grade, prior conization, lymphovascular spa

Slit ventricle syndrome is characterized by chronic or recurring headaches associated with subnormal ventricular volume in patients who have undergone shunt treatment for hydrocephalus. There appear to be at least three pathophysiological mechanisms that cause this syndrome: 1) intermittent shunt malfunction; 2) intracranial hypotension; and 3) paroxysms of increased intracranial pressure in the presence of normal shunt function. To treat seven patients with slit ventricle syndrome caused by paroxysms of elevated intracranial pressure, we successfully used antimigraine therapy rather than stan

Urethral disease in women and girls often is overlooked. As the urine may seem to be normal as determined by repeated urinalysis, the symptoms-urinary frequency and burning-may be attributed entirely to other pelvic disease or to functional disorder. Since erroneous diagnosis may lead to unnecessary procedures or to neglect of treatment with consequent development of severe disease in the kidneys or ureters, it is important to consider urethral lesions as a possible cause in any case of abdominal discomfort in women. The most common lesions of the urethra in women are urethritis, stricture, ca…
