[Occupational allergic asthma in a laboratory animal keeper with COPD].

This is a case report of a 63-year-old woman, who had worked as a laboratory animal keeper for 23 years. She developed increasing symptoms of rhinitis, coughing and shortness of breath and was diagnosed and treated for COPD based on her smoking history and spirometry results. She went on sick leave due to dyspnoea, and after testing she was diagnosed with occupational allergic asthma from rodents. Her lung function was at this point significantly reduced. Occupational asthma is common, and occupational causes of respiratory symptoms should always be taken into consideration when assessing pati

Does impaired O2 delivery during exercise accentuate central and peripheral fatigue in patients with coexistent COPD-CHF?

Impairment in oxygen (O2) delivery to the central nervous system ("brain") and skeletal locomotor muscle during exercise has been associated … y function/interactions and blood flow redistribution to the overloaded respiratory muscles (i.e., ↑work of breathing) may underpin these abnormalities. Unfortunately, COPD and CHF coexist in almost a third of elderly patients making these mechanisms potentially more relevant to exercise intolerance. In this context, it remains unknown whether decreased O2 delivery …

3D Printed Conformal Strain and Humidity Sensors for Human Motion Prediction and Health Monitoring via Machine Learning.

Wearable sensors have garnered considerable attention due to their flexibility and lightweight characteristics in the realm of healthcare applications. However, developing robust wearable sensors with facile fabrication and good conformity remains a challenge. In this study, a conductive graphene nanoplate-carbon nanotube (GC) ink is synthesized for multi jet fusion (MJF) printing. The layer-by-layer fabrication process of MJF not only improves the mechanical and flame-retardant properties of the printed GC sensor but also bolsters its robustness and sensitivity. The direction of sensor bendin

Smartphone Screen Integrated Optical Breathalyzer.

One third of fatal car accidents and so many tragedies are due to alcohol abuse. These sad numbers could be mitigated if everyone had access to a breathalyzer anytime and anywhere. Having a breathalyzer built into a phone or wearable technology could be the way to get around reluctance to carry a separate device. With this goal, we propose an inexpensive breathalyzer that could be integrated in the screens of mobile devices. Our technology is based on the evaporation rate of the fog produced by the breath on the phone screen, which increases with increasing breath alcohol content. The device s

Reliability of autonomic activations as surrogates of cortical arousals in ventilated patients affected by amyotrophic lateral sclerosis.

PURPOSE: The study aims to evaluate the performance of autonomic activations as a tool to assess sleep fragmentation and to recognize hypopneas in patients with amyotrophic lateral sclerosis (ALS) under non-invasive mechanical ventilation and secondarily, to evaluate, in patients with the same disease, the relationship between disruption of autonomic nervous system (ANS) activity and the usefulness of the autonomic activations as surrogates of cortical arousals. METHODS: Sixteen ALS patients underwent simultaneous polysomnography and portable cardiorespiratory monitoring (PM). On the polysomn

A strategy for in-flight measurements of physiology of pilots of high-performance fighter aircraft.

Some pilots flying modern high-performance fighter aircraft develop "hypoxia-like" incidents characterized by short periods of confusion and cognitive impairment. The problem is serious and recently led to the grounding of a fleet of aircraft. Extensive discussions of the incidents have taken place but some people believe that there is inadequate data to determine the cause. There is a tremendous disconnect between what is known about the function of the aircraft and the function of the pilot. This paper describes a plan for measuring the inspired and expired Po2 and Pco2 in the pilot's mask,

Noninvasive ventilation through a helmet in postextubation hypoxemic patients: physiologic comparison between neurally adjusted ventilatory assist and pressure support ventilation.

PURPOSE: Neurally adjusted ventilatory assist (NAVA) has been shown to improve patient-ventilator interaction and reduce asynchronies in intubated patients, as opposed to pressure support ventilation (PSV). This is a short-term head-to-head physiologic comparison between PSV and NAVA in delivering noninvasive ventilation through a helmet (h-NIV), in patients with postextubation hypoxemic acute respiratory failure. METHODS: Ten patients underwent three 20-min trials of h-NIV in PSV, NAVA, and PSV again. Arterial blood gases (ABGs) were assessed at the end of each trial. Diaphragm electrical ac

Quasi-dynamic breathing model of the lung incorporating viscoelasticity of the lung tissue.

We advanced a novel model to calculate viscoelastic lung compliance and airflow resistance in presence of mucus, accounting for the quasi-linear viscoelastic stress-strain response of the parenchyma (alveoli) tissue. We adapted a continuum-based numerical modeling approach for the lung, integrating the fluid mechanics of the airflow within individual generations of the bronchi and alveoli. The model accounts for elasticity of the deformable bronchioles, resistance to airflow due to the presence of mucus within the bronchioles, and subsequent mucus flow. Simulated quasi-dynamic inhalation and e

Management of dyspnea in severe chronic obstructive pulmonary disease.

Progression of chronic obstructive pulmonary disease (COPD) is frequently associated with increasing dyspnea; indeed, patients with severe COPD constitute the largest group of patients with chronic respiratory insufficiency. The sensation of dyspnea in these patients is mostly related to increased work of breathing, a consequence of an increased resistive load, of hyperinflation, and of the deleterious effect of intrinsic positive end-expiratory pressure (PEEP(i)). Once optimal medical treatment has been provided, pharmacological treatments of dyspnea exist (beta2-agonists, methylxanthines, op…

Time-varying signal analysis to detect high-altitude periodic breathing in climbers ascending to extreme altitude.

This work investigates the performance of cardiorespiratory analysis detecting periodic breathing (PB) in chest wall recordings in mountaineers climbing to extreme altitude. The breathing patterns of 34 mountaineers were monitored unobtrusively by inductance plethysmography, ECG and pulse oximetry using a portable recorder during climbs at altitudes between 4497 and 7546 m on Mt. Muztagh Ata. The minute ventilation (VE) and heart rate (HR) signals were studied, to identify visually scored PB, applying time-varying spectral, coherence and entropy analysis. In 411 climbing periods, 30-120 min in

The effects of actors vocal exercises for relaxation on fluency: A preliminary study.

PURPOSE: To determine the efficacy of treatment based on Kristin Linklater's technique for vocal preparation for performance for use with people who stutter. METHOD: A protocol for a treatment for stuttering involving breathing exercises, relaxation techniques, and focus on awareness was designed by the first author from Linklater's published exercises in her book Freeing the Natural Voice (2006). Four adults who stutter participated in a 12-week, single-case reversal design study. Treatment efficacy was determined by baseline and post-treatment measures on the OASES, self-report naturalness,

Contribution of peripheral chemoreceptor drive in exercise hyperpnea in humans.

The peripheral chemoreceptors play a dominant role in the respiratory compensation of lactic acidosis during heavy exercise of humans. Our object was to determine the contribution of peripheral chemoreceptors to exercise hyperpnea during mild to moderate and heavy exercise above the anaerobic threshold. We used a hyperoxic suppression test in six normal male subjects. Inspired gas was abruptly changed without the subject's knowledge from air to pure oxygen for 5 to 6 breaths. The maximal ventilatory depression after O2 breathing was 5.5 +/- 1.7 L/min (BTPS) at mild exercise, and the depression

Motion- Based Respiratory Rate Estimation with Motion Artifact Removal Using Video of Face and Upper Body.

Respiratory rate (RR) is a significant indicator of health conditions. Remote contactless measurement of RR is gaining popularity with recent respiratory tract infection awareness. Among various methods of contactless RR measurement, a video of an individual can be used to obtain an instantaneous RR. In this paper, we introduce an RR estimation based on the subtle motion of the head or upper chest captured on an RGB camera. Motion-based respiratory monitoring allows us to acquire RR from individuals with partial face coverings, such as glasses or a face mask. However, motion-based RR estimatio

[Sleep apnea syndromes].

Sleep apnea syndromes have been identified only relatively recently. Their most frequent form is characterized by a sleep-related upper airway obstruction resulting in apneas which may repeat themselves up to several hundred times during a night's sleep. Their mean duration is about 30 to 40 seconds, but some apneas last over one minute. Breathing resumption requires an arousal, which may be clearly identified on the EEG but usually goes unnoticed by the patient. The most immediate consequence are hypoxemia and sleep fragmentation. There may be associated arrhythmias and hemodynamic changes, e

Expiratory glottic widening in asthmatic subjects during exercise-induced bronchoconstriction.

Glottic caliber is modulated in a way that optimizes the airway resistance and the work of breathing. The midexpiratory glottic narrowing that normally occurs during tidal breathing is enhanced during histamine- or methacholine-induced airway narrowing. Exercise is associated with increased ventilatory demand and midexpiratory glottic widening in healthy subjects. However, the relative contribution of glottic aperture to airflow limitation during exercise-induced asthma is unknown. Consequently, we used the acoustic reflection technique to test whether a decrease in midexpiratory glottic size …

Wavelet-enhanced TD-GC-MS analysis of molecular pattern alterations in gas samples induced by breath sampling devices.

This study presents a computational method to identify volatile organic compound (VOC) artifacts introduced by breath sampling hardware. To exclude endogenous biological variability, ambient air was collected using two sampling devices working in the same experimental conditions: the Mistral end-tidal breath sampler and the ACTI-VOC PLUS pump, a low-emission reference system. VOCs were pre-concentrated on sorbent-packed thermal desorption (TD) tubes and analyzed by TD-gas chromatography-mass spectrometry (TD-GC-MS). Differential chromatograms obtained by subtracting ACTI-VOC signals from Mistr

An Unusual Case of Severe Atelectasis: Mucus Impaction in a Young Obese Female.

Atelectasis is defined as the the loss of lung volume due to collapse of lung tissue and is usually associated with symptoms of respiratory distress, including increased work of breathing and increased oxygen requirements. It is common in hospitalized patients with limited mobility and in patients with underlying lung conditions. Treatment is largely supportive when no underlying condition is identified. It is rare to occur in otherwise healthy individuals. The patient in this case presented to the emergency department with complaints of progressive shortness of breath, productive cough, chest…
