Microbubbles Mimicking Prosthetic Mitral Valve Thrombosis.

A 36-year-old woman with a history of redo mitral valve replacement (bileaflet mechanical mitral valve prosthesis) in 2002 for prosthetic mitral valve endocarditis presented to the emergency department with a history of recurrent palpitations. A transthoracic echocardiogram was performed and falsely indicated a diagnosis of prosthetic mitral valve thrombosis. However, review of the echocardiogram by an experienced echocardiologist revised the diagnosis to microbubbles formation phenomenon with mechanical mitral prosthesis. Frame-by-frame analysis or reducing the frame rate of the recorded echo

Transcatheter Versus Surgical Pulmonary Valve Replacement: A Systemic Review and Meta-Analysis.

BACKGROUND: Transcatheter pulmonary valve replacement (TPVR) has emerged as an alternative to surgery in patients with pulmonary valve dysfunction. METHODS: We searched the Medline and Cochrane databases since their inception to January 2019 as well as references from article, for all publications comparing TPVR with surgical PVR (SPVR). Studies were considered for inclusion if they reported comparative data regarding any of the study endpoints. The primary endpoint was early mortality after PVR. Secondary endpoints included procedure-related complications, length of hospital stay, mortality

[Aortic Valve Translocation for Severe Prosthetic Valve Endocarditis with Aortic Annular Abscess].

We report a case of successful aortic valve translocation in a 71-year-old man with severe prosthetic valve endocarditis and an aortic annular abscess. Six years earlier, the patient had undergone aortic valve replacement for aortic regurgitation and coronary artery bypass grafting to the left anterior descending artery with a saphenous vein. Moreover, 4 years earlier, he had undergone total arch replacement for chronic aortic dissection. He was admitted to our hospital with suspected urinary tract infection. Despite antibiotic therapy, the patient developed a high fever. Transthoracic echocar

Successful mitral valve repair for isolated mitral regurgitation in a child with Williams syndrome.

Williams syndrome is a genetic disorder associated with various cardiovascular abnormalities, most commonly supravalvar aortic stenosis and peripheral pulmonary stenosis. However, isolated severe mitral regurgitation necessitating surgical intervention is extremely rare. Here, we present the case of a 14-year-old child with Williams syndrome and isolated severe mitral regurgitation who underwent successful mitral valve repair.

Traumatic leaflet injury: comparison of porcine leaflet self-expandable and bovine leaflet balloon-expandable prostheses.

OBJECTIVES: The main goal of this study was to compare the occurrence of post-deployment leaflet injury between prostheses made of porcine and bovine pericardium. METHODS: Two types of prostheses, self-expandable prostheses with porcine pericardial leaflet on one side and balloon-expandable prostheses with bovine pericardial leaflet on the other side, were used. In each group, crimped prostheses were compared with control non-crimped prostheses. Following a 15-min period of crimping, prostheses were deployed, and their leaflets were removed and subjected to analyses. The analyses included det

Mitral valve repair: an echocardiographic review: Part 2.

Echocardiographic imaging of the mitral valve before and immediately after repair is crucial to the immediate and long-term outcome. Prior to mitral valve repair, echocardiographic imaging helps determine the feasibility and method of repair. After the repair, echocardiographic imaging displays the new baseline anatomy, assesses function, and determines whether or not further management is necessary. Three-dimensional imaging has improved the assessment of the mitral valve and facilitates communication with the surgeon by providing the surgeon with an image that he/she might see upon opening u

Functional tricuspid regurgitation: an underestimated issue.

This review article focuses on functional tricuspid regurgitation (FTR) that has long been a neglected and underestimated entity. FTR is defined as leakage of the tricuspid valve during systole in the presence of structurally normal leaflets and chordae. FTR may be secondary to several heart diseases, more commonly mitral valve disease, pulmonary hypertension, atrial fibrillation, cardiomyopathies, right ventricular dysplasia, and idiopathic annular dilatation. The reported prevalence of moderate or greater FTR is roughly 16%, but it rises up to 89% when considering FTR of any grade. According

Myocardial hypertrophy after pulmonary regurgitation and valve implantation in pigs.

BACKGROUND: Patients may suffer from right ventricular (RV) failure and malignant cardiac arrhythmias after late pulmonary valve replacement correcting pulmonary regurgitation (PR). But the underlying mechanisms of the refractory arrhythmias are not well understood. METHODS: The aim of present study was to characterize the RV myocardium after percutaneous pulmonary valve implantation (PPVI) in a porcine model after severe PR for 3months. RV histology was evaluated with morphometric methods and RV function was assessed with electrophysiology, echocardiography, and biochemical measures: The res

Characteristics of compressive strains in porcine aortic valves cusps.

BACKGROUND AND AIM OF STUDY: Previous studies in our laboratory have revealed the presence of compressive strains at the base of the non-coronary (NC) cusp of porcine aortic valves. Since these strains have been shown to damage bioprosthetic valve tissue, the mechanism by which they are induced in valve tissues should be investigated. METHODS: Whole porcine aortic valves were pressurized and the induced strain patterns measured on aortic valve cusps and roots. Measurements were made before and after trimming of excess ventricular muscle and mitral valve tissue. RESULTS: Negative (compressive
