INTRODUCTION: Non-union of long bone fractures after intramedullary nailing (IMN) remains a demanding clinical challenge. Augmentation plating - applying a supplementary plate over the retained nail - offers superior rotational control without the morbidity of nail extraction. This prospective study evaluated clinical, radiological, and functional outcomes of this technique for aseptic femoral and tibial non-unions.
MATERIALS AND METHODS: Thirty patients with aseptic diaphyseal non-union of the femur or tibia underwent augmentation plating with selective autologous bone grafting (April 2024-S

With the exception of revision osteosynthesis, conversion total hip arthroplasty (CTHA) following sliding hip screw (SHS) and cephalomedullary nail (CMN) failure in intertrochanteric fractures (ITF) is the most commonly used treatment option. This review determined the relative risk of medical and orthopedic complications, including periprosthetic femoral fractures (PFF), following CTHA in failed SHS and CMN fixation of ITF, as well as the Harris hip score (HHS). Major electronic databases were searched for studies and reports on CTHA after SHS and CMN fixation failures in ITF. To assess the r

PURPOSE: Several studies have compared various methods of treatment for displaced femoral neck fractures (FNF), including multiple cannulated screw fixation, in young and active patients. However, there have been few studies on the cephalomedullary nail with additional cannulated screw fixation in displaced FNF. Therefore, the aim of this study is to evaluate the outcomes of the cephalomedullary nail technique with either a cannulated screw or multiple cannulated screw fixation in displaced FNF.
MATERIALS AND METHODS: Among 69 consecutive patients who underwent internal fixation for displaced

BACKGROUND: To explore the effect of intramedullary nailing versus external ring fixation for patients with tibial shaft fractures on 12-month maximal isometric muscle strength for knee extension and knee flexion. We hypothesise that patients treated with external ring fixation will show significantly better maximal isometric muscle strength for knee extension and knee flexion 12 months after surgery compared to treatment with intramedullary nailing. Moreover, we aim to explore the association between muscle strength and the Knee Injury and Osteoarthritis Outcome Score (KOOS) at 12 months foll

Introduction Distal femur fractures are common but challenging as they are usually displaced and comminuted. They are prone to functional impairment of the knee joint because of injury to the quadriceps system. Such fracture often occurs in elderly patients with osteoporosis. Various treatment methods, such as closed traction, application of a cast brace following preliminary traction, and open reduction with internal fixation using various implants, are suggested. We evaluated the surgical and functional results of distal femur fractures treated with anatomical distal femur locking compressio

The global rise in total knee arthroplasty (TKA), driven by an aging population, has led to an increased incidence of periprosthetic fractures (PPFs). Dual implants for distal femur periprosthetic fractures (PDFFs) are a growing area of interest for these challenging fractures with dual plating (DP) and plate-retrograde femoral intramedullary nail (PN) emerging as viable constructs for these injuries. However, dual implants have inherent limitations. Herein we focus on describing a modified PN fixation-retrograde tibial intramedullary nail (RTN) combined with a less invasive stabilization syst

BACKGROUND: A combination retrograde femoral intramedullary nail (IMN) and locking plate for periprosthetic distal femoral fractures (PDFFs) may offer improved construct strength and early weight-bearing versus locked plating constructs or IMN fixation. This study compared clinical and radiographic outcomes in PDFF treatment with nail-plate combination (NPC), locked plating, or IMN.
METHODS: Patients aged ≥ 18 years undergoing PDFF surgery between January 1, 2015, and April 1, 2024, at three institutions were retrospectively identified. Patients were included with NPC, locked plate only (i.e.

AIMS: Musculoskeletal injuries are more common in sub-Saharan Africa than anywhere else in the world, yet there is limited evidence to guide the management of these injuries in low- and middle-income countries. We aimed to evaluate the outcomes of lower limb intramedullary nailing (IMN) for long bone fractures in South Africa and identify predictors of these outcomes.
METHODS: Adults who sustained tibia and femur shaft fractures, and were treated with IMN at two tertiary hospitals in South Africa from September 2017 to December 2018, were followed for at least 12 months. We compared fracture

INTRODUCTION: This study reports the successful application of distal tibial nail (DTN) for managing complex nonunions in open distal tibial fractures in patients with diabetes and a history of pin-site infection.
PRESENTATION OF CASE: A 55-year-old diabetic female presented with an open left tibia-fibula fracture (Gustilo-Anderson Type IIIA) sustained in a car accident. She initially underwent debridement, suturing, and external fixation at a referring hospital. Postoperatively, the external fixation pin sites showed erythema, swelling, and discharge. Radiographs and CT scans revealed a tibi
