INJURY TO THE RESPIRATORY TREE. ROENTGENOLOGIC FINDINGS AFTER SEVERE BLUNT TRAUMA.

The roentgenologic signs of free air in the subcutaneous, mediastinal pleural spaces may be indicative of fractures of the trachea and bronchi and injuries to the larynx. While these findings are also present in otherwise uncomplicated rib fracture and following tracheostomy, care must be taken to make sure that the cause of these abnormalities is not an unrecognized bronchial or tracheal rupture.

Protective use of N-methyl-D-aspartate receptor antagonists as a spinoplegia against excitatory amino acid neurotoxicity.

OBJECTIVE: Paraplegia remains a serious complication of thoracic and thoracoabdominal aortic operations. To avoid this dreadful complication, N-methyl-D-aspartate (NMDA) receptor antagonists have been examined in the ischemic or excitotoxic neuronal injury model. In the present study, we evaluated the protective efficacy of NMDA receptor antagonists that were infused segmentally after aortic clamping, as a spinoplegia, to reduce aspartate neurotoxicity in the spinal cord. METHODS: Infrarenal aortic isolation was performed in New Zealand white rabbits. Group A animals (n = 7) were pretreated w

Surgical Stabilization of Rib Fractures: Indications, Techniques, and Pitfalls.

UNLABELLED: Rib fractures are a common thoracic injury that is encountered in 20% to 39% of patients with blunt chest trauma and is associated with substantial morbidity and mortality1,2. Traditionally, the majority of patient with rib fractures have been managed nonoperatively. Recently, the utilization of surgical stabilization of rib fractures has increased considerably because the procedure has shown improved outcomes3-5. DESCRIPTION: Surgical stabilization should be considered in cases of multiple bicortically displaced rib fractures, especially in those with a flail chest and/or a conco

Complete transection of the left main bronchus caused by blunt thoracic trauma in a child treated by bronchoplasty and lung parenchyma preservation.

Blunt bronchial injuries can be difficult to diagnose and can present months or years after an initial incident. The present case report describes complete transection of a major airway after a potentially fatal delayed diagnosis with a successful outcome after bronchoplasty without the removal of lung parenchyma.

[Early rethoracotomies for diseases and chest traumas].

Rethoracotomies were performed on 34 patients operated for diseases and traumas of the chest, diseases of the esophagus which made up 1.3%. Emergency and reoperations were performed on 14 (46%) patients for ongoing intrapleural bleeding, clotted hemothorax and pulmonary bleeding. Emergency and delayed rethoracotomies were performed on 7 patients for non-hermeticity of the lung and incompetence of the bronchus suture, on 5 patients for incompetence of esophagogastroanastomosis, necrosis of the transplant and gastric wall, 3 patients for chylothorax not-arrested conservatively, 2 patients for ga

Acute traumatic aortic rupture: early stent-graft repair.

OBJECTIVE: Prospective evaluation of early stent-graft repair of acute traumatic aortic rupture. METHODS: Twelve patients with acute traumatic aortic rupture of the descending aorta, out of a series of 337 endovascular aortic procedures, were treated by implantation of self-expanding stent-grafts. The procedures were performed within a mean post-injury time-period of 5+/-7 days (median: 1 day). The feasibility of stent-grafting was assessed by CT scanning and echography. Implantation was performed under local (n=6), or general anesthesia (n=6) if patients were already intubated (n=5) or requi

Long-term disability associated with flail chest injury.

Twenty-two trauma victims who had sustained flail chest as their only significant injury were evaluated to determine the final outcome. Fourteen patients (63.9 percent) were found to have long-term sequelae. The most common long-term problems after flail chest injury were persistent chest wall pain, chest wall deformity, and dyspnea on exertion. Five patients (22 percent) remained disabled in varying degrees.

[Polytrauma in cyclists. Incidence, etiology, and injury patterns].

BACKGROUND: In our region we have noticed an increasing number of cyclists and consequently a rise in bicycle-related accidents in recent years. A large number of our polytraumatized patients are victims of bicycle-related accidents. MATERIAL AND METHODS: Retrospectively we analyzed the data of our polytraumatized patients recorded between May 2003 and June 2004 for bicycle-related injuries. Of 153 polytraumatized patients treated in our emergency room 32 were cyclists (21%). The average age of our polytraumatized cyclist was 42 years, and the male-to-female ratio was 1:1. RESULTS: The media
