Three-dimensional multimodal fractal dynamics integrating spatial metabolism, cytoarchitecture, and perfusion for preoperative prediction of microvascular invasion in colorectal liver metastases.

BACKGROUND: Accurately predicting microvascular invasion (MVI) preoperatively remains a core challenge for precision surgical decision-making in patients with colorectal liver metastases (CRLM). This study developed and validated a three-dimensional multimodal fractal dynamic model (FRAC-3D) that integrates spatial metabolism, cytoarchitecture, and blood perfusion, aiming to non-invasively identify MVI status preoperatively and evaluate patient prognosis. METHODS: Utilizing a retrospective nested cohort design, stabilized inverse probability of treatment weighting (sIPTW) was applied to the d

The Molecular and Functional Landscape of Resistance to FOLFIRI Chemotherapy in Metastatic Colorectal Cancer.

UNLABELLED: The combination of 5-fluorouracil and irinotecan (FOLFIRI) remains a standard-of-care treatment for metastatic colorectal cancer (mCRC) yet benefits only about half of patients. Using patient-derived xenografts, we investigated the biological underpinnings of this heterogeneous response. FOLFIRI-resistant models showed transcriptional upregulation of innate immunity and mitochondrial metabolism genes, together with reduced expression of the DNA polymerase POLD1. Sensitive counterparts exhibited a BRCAness-like phenotype with genomic scars of homologous recombination (HR) deficiency

Multikinase inhibitors in refractory metastatic colorectal cancer: an optimal sequence?

Despite major advances in management strategies, metastatic colorectal cancer remains an important clinical challenge because most patients experience progression after standard first- and secondline treatments. In the setting of refractory disease, defined as disease that progresses after 2 or more lines of treatment, the therapeutic landscape is growing. Options include regorafenib, trifluridine plus tipiracil (FTD/TPI) with or without bevacizumab, and fruquintinib, all of which received approval from the US Food and Drug Administration after showing modest survival benefits in phase 3 trial

Xiao-Yao-San suppresses psychological distress-related colorectal cancer metastasis and enhances antitumor immune responses through regulating gut microbiota-bile acid axis.

BACKGROUND: Psychiatric disorders are recognized as risk factors for tumor progression and metastasis in colorectal cancer (CRC). Xiao-Yao-San (XYS), a traditional Chinese herbal formula with antidepressant properties, has been reported to inhibit CRC growth and modulate gut microbiota. However, its efficacy and underlying mechanisms in preventing metastasis remain unclear. METHODS: The antimetastatic effect of XYS was assessed in a chronic restraint stress (CRS)-induced CRC model. XYS-associated gut microbiota was evaluated using 16S ribosomal RNA (rRNA) sequencing. Peripheral bile acid (BA)

Multimodal activity-affinity assay of ADAM-10 extracellular vesicles in untreated plasma reveals metastatic stage of colorectal cancer.

Metalloproteinases (MPs) such as a-disintegrin and metalloproteinase-10 (ADAM-10) are key drivers of extracellular matrix remodeling during tumor progression, yet MP-based liquid biopsy tests have not reached clinical utility. Here, we show that active ADAM-10 is selectively enriched on the surface of circulating extracellular vesicles (EVs) in the plasma of colorectal cancer patients. Our findings further suggest ADAM-10+ EVs are locally enriched in dense pre-metastatic tumor extracellular matrices and subsequently accumulate in blood post-metastasis. To capture these unique signatures of dis

A Plastic EMP1+ to LGR5+ Cell State Conversion as a Bypass to KRASG12D Pharmacologic Inhibition in Metastatic Colorectal Cancer.

UNLABELLED: Inhibitors of the oncogene KRAS hold promise for treating metastatic colorectal cancer (mCRC). In this study, we show that a selective, covalent small-molecule inhibitor of the active (ON) conformation of RAS-G12D, RMC-9945, exerts durable disease control in preclinical colorectal cancer models of early liver metastasis, but its therapeutic activity was diminished in the advanced metastatic disease. RMC-9945-treated metastases underwent a transition from a poor prognosis-associated Emp1+ transcriptional state to a WNT-driven Lgr5+ stem cell-like state that withstands the absence of

Outcomes of Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy (CRS/HIPEC) and Liver-Directed Therapy for Synchronous Peritoneal and Liver Metastatic Colorectal Cancer: A Systematic Review with Meta-analysis.

INTRODUCTION: Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) can improve survival in colorectal cancer (CRC) patients with peritoneal carcinomatosis (PC). The presence of synchronous liver metastases (LMs) often precludes patients from CRS/HIPEC; however, multiple studies suggest that CRS/HIPEC with liver-directed treatments may be beneficial. This systematic review examines outcomes and selection factors in CRC patients with PC and LM treated with CRS/HIPEC and liver-directed therapy. METHODS: A systematic review and meta-analysis was performed using the Pub

The impact of laparoscopic vs open primary colon resection on long-term outcomes after subsequent cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) for metachronous peritoneal metastasis.

BACKGROUND: Several factors have been known to correlate with the early and long-term outcomes of patients undergoing CRS/HIPEC. However, the relation between surgical approach (open vs laparoscopic colectomy) at the index colectomy and oncological outcomes following CRS/HIPEC have not been studied. METHODS: Patients who underwent CRS/HIPEC after previous colectomy for colorectal cancer with peritoneal metastases from 2015 to 2022 were identified from a prospectively maintained database of peritoneal surface malignancies and were divided into two groups according to the primary colectomy appr

How Frequently Is Colorectal Cancer Recurrence Detected in a Clinic?

BACKGROUND: Outpatient clinic follow-up after resection of colorectal cancer has been standard practice for decades; however, recent evidence regarding its efficacy in oncological detection is limited. With increasing health limitations and rising numbers of patients with colorectal cancer, the role of clinic follow-up may need reevaluation. OBJECTIVE: This study aims to determine the frequency of clinical examination compared to other modalities in the identification of colorectal cancer recurrence in Northland. DESIGN: Retrospective review of detection modalities for recurrence in colorect

A novel machine learning-based cancer-specific cardiovascular disease risk score among patients with breast, colorectal, or lung cancer.

BACKGROUND: Cancer patients have up to a 3-fold higher risk for cardiovascular disease (CVD) than the general population. Traditional CVD risk scores may be less accurate for them. We aimed to develop cancer-specific CVD risk scores and compare them with conventional scores in predicting 10-year CVD risk for patients with breast cancer (BC), colorectal cancer (CRC), or lung cancer (LC). METHODS: We analyzed adults diagnosed with BC, CRC, or LC between 2005 and 2012. An machine learning (ML) Extreme Gradient Boosting algorithm ranked 40-50 covariates for predicting CVD for each cancer type usi

The significance of exosomal non-coding RNAs (ncRNAs) in the metastasis of colorectal cancer and development of therapy resistance.

Colorectal cancer (CRC) represents a common type of carcinoma with significant mortality rates globally. A primary factor contributing to the unfavorable treatment outcomes and reduced survival rates in CRC patients is the occurrence of metastasis. Various intricate molecular mechanisms are implicated in the metastatic process, leading to mortality among individuals with CRC. In the realm of intercellular communication, exosomes, which are a form of extracellular vesicle (EV), play an essential role. These vesicles act as conduits for information exchange between cells and originate from multi

Surgical treatment of pancreatic metastases: More appropriate surgical methods based on a clinicopathologic study of 43 patients.

PURPOSE: There is no established surgical method for metastatic lesion to the pancreas. In the case of relatively small lesion, we often hesitate to select which surgical method, that is, wedge/partial resection or Whipple/distal pancreatectomy. Moreover, it is debatable whether lymph node dissection is necessary or not. We investigated clinicopathological characteristics in order to resolve the above problems. METHODS: Forty-three patients underwent pancreatic resection for metastatic tumors in Cancer Institute of the Japanese Foundation for Cancer Research, whose specimens were investigated
