Closed Reconstruction of Depressed Frontal Sinus Fractures: An Innovative Technique.

This article describes a novel technique for reconstructing a frontal sinus fracture that minimizes the risks associated with conventional approaches. In this study, we report a patient presented with a depressed frontal sinus fracture following a car accident. Through a small (5 mm) sub-brow incision, a freer elevator was inserted beneath the fractured fragments to elevate them. A 16-gauge venous catheter was then introduced horizontally from the medial aspect of the bone, exiting at the lateral side. Following this, a 24-gauge wire was threaded through the catheter, which was subsequently re

Clinical Outcomes According to Surgical Strategies in Proximal Early Gastric Cancer: A Systematic Review and Network Meta-Analysis.

PURPOSE: Many surgical methods have been developed for the optimal treatment of proximal early gastric cancer (GC); however, these approaches are still controversial. Proximal gastrectomy with direct esophagogastrostomy (EG), double-tract reconstruction (DTR), and double flap-technique (DFT) are considered function-preserving surgeries for proximal early GC rather than total gastrectomy (TG). We conducted a systematic review and network meta-analysis comparing EG, DTR, DFT, and TG to determine the surgical strategy for proximal GC. MATERIALS AND METHODS: We systematically searched PubMed, Emb

Bilateral Pedicled Superior Gluteal Artery Perforator (SGAP) Flap for Double-Layer Closure and Reconstruction of Sacrococcygeal Defect After Chordoma Resection: A Case Report.

Chordomas are rare, slow-growing, locally aggressive malignant tumors arising from notochord remnants that commonly affect the sacrococcygeal area. Surgical resection with negative margins is crucial but often results in extended defects with exposed critical structures, necessitating complex reconstructions. The reconstruction techniques commonly employed and described in the literature often utilize musculocutaneous flaps, which are associated with higher donor site morbidity. Thus, the challenge remains to minimize donor site morbidity while ensuring effective reconstruction and long-term s

Novel Vascularized Pedicle Flaps for Dural Reconstruction via Endoscopic Transorbital Approach: Temporalis Muscle and Pericranial Flaps.

BACKGROUND: The endoscopic transorbital approach (ETOA) has emerged as a valuable minimally invasive technique in skull base surgery, providing direct access to the anterior and middle cranial fossae. However, effective reconstruction of dural defects remains a significant technical challenge. This study evaluates the feasibility and anatomical characteristics of temporalis muscle flaps (TFs) and pericranial flaps (PFs) for dural reconstruction following ETOA, using cadaveric models. METHODS: Four fresh cadaveric heads were dissected. The TFs were harvested using an inferior-based pedicle, wh

Flap reconstruction of post-burn neck contractures: A systematic review.

BACKGROUND AND OBJECTIVES: Post-burn neck contractures present a complex reconstructive challenge. Flap-based reconstruction remains the standard in surgical management, yet no consensus exists regarding optimal flap selection or outcome evaluation. This systematic review aims to evaluate the effectiveness of flap reconstruction for post-burn neck contractures and to identify gaps in the literature. METHODS: A systematic search of MEDLINE, Embase, and Web of Science was conducted following PRISMA guidelines. Studies reporting flap-based reconstruction for post-burn neck contractures published

The zygomatic implant perforated flap in the reconstruction and early rehabilitation of low-level maxillectomies: A technical note.

The "Zygomatic Implant Perforated Flap" (ZIP flap) technique, first described by Butterworth in 2017, combines the closure of bucco-sinusal or bucco-nasal communications using a fasciocutaneous free flap with early implant-based prosthetic rehabilitation via zygomatic implants that perforate the flap. This approach enables rapid dental rehabilitation within weeks of tumor resection, promoting early recovery of oral function. We present the case of a 49-year-old male who underwent delayed ZIP flap reconstruction one year after maxillectomy and radiotherapy. Four zygomatic implants were placed u

Delayed Onset Free Jejunal Flap Necrosis due to Thrombosis: A Case Report.

Reports of jejunal flap necrosis caused by vascular anastomosis thrombosis as a late complication are rare. We report the case of a 68-year-old man who presented with free jejunal necrosis more than 2 years after reconstruction surgery and achieved a successful outcome due to relatively early necrosis detection, enabling salvage surgery before severe infection developed. It highlights the clinical importance of regular monitoring and documentation of the internal appearance of the free jejunal flap's lumen and the need to consider the possibility of delayed necrosis.

A Systematic Review and Meta-Analysis of Treatment Outcomes Following Tongue Reconstruction With Neurotized Free Flaps.

BACKGROUND AND OBJECTIVES: The tongue is the most common site of oral malignancy, and surgical treatment may result in impaired feeding, swallowing, and speech. Free tissue transfer is the preferred method for reconstructing complex defects. Evidence remains unclear on whether sensory reinnervation improves function. The objective of this review is to evaluate the outcome of patients undergoing innervated versus noninnervated free flap reconstruction following tongue cancer surgery. METHODS: A systematic review was performed to determine the role of sensory neurotization in lingual reconstruc

Reconstruction of a Sternal Cleft and Ventral Hernia in an Adolescent with Pentalogy of Cantrell Utilizing Titanium Plates.

Pentalogy of Cantrell (PC) is a rare congenital disorder that presents with several midline defects including the abdominal wall, sternum, diaphragm, pericardium, and heart. Surgical intervention for omphaloceles and cardiac defects typically occur in infancy, but the optimal surgical repair for thoracoabdominal wall defects varies by patient. We present a 15-year-old female with PC, who underwent previous omphalocele, diaphragm, and cardiac repair as an infant, with recurrent ventral and diaphragmatic defects as well as a large sternal cleft with ectopia cordis. This case demonstrates the fea

Surgical Strategies for Functional Upper Extremity Reconstruction After Spinal Cord Injury.

Spinal cord injuries (SCI) can substantially affect independence and quality of life, particularly by limiting upper extremity function. Surgical reconstruction offers the potential to restore motion in the hand, wrist, and elbow for those with deficits following cervical spinal cord injury. Techniques such as tendon transfer, tenodesis, and arthrodesis-often used in combination-are well-established strategies for enhancing upper extremity function. Nerve transfers have more recently been employed and differ from other procedures in that they are often time sensitive and should be performed be
