Clinical analysis of 66 cases with orbital apex syndrome: a case series.

BACKGROUND: Orbital apex syndrome (OAS) is a rare ocular complication characterized by vision loss and ophthalmoplegia from infection, inflammation, trauma, neoplasms, and vascular causes, due to the involvement of ocular motor nerves in the anatomical region of the orbital apex. The epidemiological features of OAS are limited. Therefore, this study aimed to present the clinical features and determine the causes to evaluate the visual prognosis of patients with OAS. METHODS: A retrospective analysis was conducted on 126 adult patients with clinically confirmed OAS by reviewing the electronic

Orbital Extension of Sinonasal Mucocele Presenting as Proptosis: A Report of Two Cases.

Sinonasal mucoceles are benign cystic lesions caused by obstruction of sinus drainage; however, they may progressively expand and erode surrounding bone, leading to orbital complications. We report two cases of sinonasal mucoceles with orbital extension presenting as proptosis. The first case involved a 34-year-old man presenting with acute proptosis, diplopia, and decreased visual acuity, with imaging revealing a frontoethmoidal lesion extending into the orbit. The second case involved a 43-year-old woman with progressive proptosis without visual impairment, also related to a frontoethmoidal

Unveiling the hidden link: diabetes mellitus as a catalyst for orbital apex syndrome.

BACKGROUND: Orbital Apex Syndrome (OAS) is a disease of multiple brain nerves at the orbital apex leading to vision loss and neurological impairments. Diabetes Mellitus (DM), a metabolic disorder with cardiovascular, immunological and neurological effects, is involved in OAS pathogenesis. However, the association between DM and OAS is not well studied. DM and OAS are poorly understood and may not be diagnosed correctly, especially when outbreaks such as COVID-19 are being investigated. METHODS: A systematic review of 33 studies published between 2000 and 2025 was conducted to analyze DM-relat

Etiopathogenesis, Classification, and Management of Eyelid, Lacrimal, and Orbital Fistulas: A Comprehensive Review.

Fistulas involving the eyelid, lacrimal system, and orbit represent a diverse group of pathologies characterized by abnormal epithelialized communications between 2 separate epithelial-lined tissue elements. They arise from a wide range of etiologies, including congenital defects, trauma, surgery, infection, inflammation, and neoplasia. Although individually rare, their cumulative clinical burden is significant due to diagnostic delays, recurrent infection, cosmetic deformity, and the potential for functional impairment. This review synthesizes data from the current literature to (1) provide a

Orbital pseudotumor and skull base infiltration as the sole manifestation of granulomatosis with polyangiitis.

Granulomatosis with polyangiitis (GPA) is a vasculitis affecting small and medium-sized blood vessels. It typically presents with upper respiratory tract, lung, and kidney involvement. However, the disease may involve multiple other organs, and not all cases present with these classic manifestations. Periocular lesions are described in the GPA and, especially in the case of isolated, single-site disease, require extensive differentiation from other diseases, such as IgG4-related disease or myofibroblastic tumor. However, periocular lesions and coexisting symptoms suggesting changes in the cent

Juvenile idiopathic arthritis presenting with orbital pseudotumor: A rare pediatric association.

Orbital inflammatory pseudotumor is a rare, idiopathic condition characterized by nonspecific inflammation of orbital tissues. Juvenile idiopathic arthritis (JIA), a chronic autoimmune disease, may present with both systemic and extra-articular manifestations, including ocular involvement. This case highlights the importance of a multidisciplinary approach in the diagnosis and management of rare complications of JIA and underscores the need for further investigation into the relationship between systemic autoimmune disorders and localized inflammatory processes.

PEEK patient-specific implants for orbital reconstruction: accuracy and clinical outcomes.

Orbital fractures represent a common and complex challenge in maxillofacial surgery, with functional and aesthetic sequelae. Advances in computer-aided design and manufacturing, virtual planning, and three-dimensional printing have enabledpatient-specific implants (PSIs). Polyetheretherketone (PEEK), a bioinert polymer with properties similar to cortical bone, has emerged as a suitable material for orbital reconstruction. Surgical navigation has also been advocated to enhance accuracy in challenging regions. This retrospective service evaluation analysed ten patients with unilateral orbital fr

High Embolic Risk with Orbital Atherectomy for Calcified Peripheral Artery Disease.

PURPOSE: Orbital atherectomy (OA) is used to treat heavily calcified peripheral arterial lesions, aiming to improve vessel compliance and reduce the need for high-pressure angioplasty and stenting. However, clinical data raise concerns regarding distal embolization, vessel injury, and uncertain long-term benefit. This study aimed to evaluate biomechanical and embolic effects of OA in calcified human femoropopliteal arteries (FPAs). METHODS: OA was studied in 10 human FPAs with advanced atherosclerosis and calcification. A pulsatile flow loop approximated FPA hemodynamics. Sequential OA and ba

Ophthalmological injuries in isolated orbital fractures: which patients need evaluation?

Orbital fractures are frequently associated with ophthalmological injuries, which range from minor irritation to vision-threatening trauma. This study was intended to identify predictive factors for ophthalmological injuries in patients with isolated orbital fractures, and to assess whether conventional assessment of facial trauma can reliably determine the need for specialist ophthalmological assessment. This prospective cohort study included adult patients with isolated orbital floor and/or medial wall fractures, who underwent standardised ophthalmological evaluation by an ophthalmologist. T

Organ-limited ANCA-associated orbital inflammatory disease: a diagnostic challenge.

Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a group of autoimmune disorders that cause inflammation and necrosis of small- to medium-sized blood vessels. It most commonly affects the kidneys and lungs, leading to complications such as rapidly progressive glomerulonephritis and pulmonary haemorrhage. Ocular involvement occurs in up to 50% of patients with AAV, most commonly presenting as scleritis, episcleritis, or orbital inflammatory disease (also known as orbital pseudotumor). Other ocular findings may include keratitis, uveitis, or retinal vasculitis. Eyelid ne

A Systematic Review Comparing Endoscopic and Open Approaches in Orbital Fracture Surgery.

Orbital floor fractures are a common form of facial bone injury and typically present with diplopia, infraorbital nerve paresthesia, and enophthalmos. Open surgical approaches offer wide exposure but are potentially associated with visible scarring and eyelid malposition. Despite growing use of minimally invasive methods, comparative data on functional outcomes remain limited. This systematic review evaluated postoperative results (enophthalmos and diplopia) of open versus endoscopic orbital floor fracture repair. In accordance with PRISMA guidelines, a comprehensive search of PubMed/MEDLINE,

Congenital encephalomeningocele presenting as a giant orbital mass in a neonate: a case report.

BACKGROUND: Congenital orbital masses encompass a broad spectrum of etiologies, among which encephalomeningocele is exceedingly rare and particularly diagnostically challenging when atypical. CASE PRESENTATION: We report a 2-hour-old female neonate who presented with a giant left orbital mass causing marked proptosis. Imaging revealed a mixed cystic-solid lesion with calcifications and septations, but no bony defect was identified preoperatively. A transcranial approach was performed, revealing a small orbital roof defect with herniation into the anterior cranial fossa. Histopathology confirm

[Etiological investigation of inflammatory orbital disease in patients seen in internal medicine].

Inflammatory orbital diseases are characterized by inflammation that may involve any structure of the orbit including lacrimal gland, and may represent the manifestation of numerous disorders. Thereby, they are grouped under the term orbital inflammatory syndrome (OIS). A distinction is made between primary OIS of purely inflammatory etiology, whether identified or not - the latter being considered idiopathic orbital inflammatory syndrome (IOIS) - and secondary OIS resulting from a local or systemic cause such as infection, neoplasia, or even iatrogenic conditions. In this review, we describe

Rare ophthalmic complication of tuberculosis meningitis: tuberculosis orbital apex syndrome.

Orbital apex syndrome (OAS) is characterised as dysfunction of the optic nerve, oculomotor nerve, trochlear nerve, abducens nerve and ophthalmic branch of the trigeminal nerve (V1) and has a diversity of aetiologies, including infectious, inflammatory and neoplastic causes. Tuberculosis OAS, a rare manifestation of extrapulmonary tuberculosis, presents a considerable diagnostic challenge because of both its rarity-particularly in non-tuberculosis-endemic countries-and its localisation to this anatomically complex site. In this case report, we present a woman in her mid-60s, living in the midwe

DEK::AFF2 Fusion-Associated Sinonasal Carcinoma With Intracranial and Orbital Involvement.

Sinonasal carcinomas encompass a molecularly heterogeneous group of malignancies. DEK::AFF2 fusion-associated carcinoma is a recently recognized subtype of nonkeratinizing squamous cell carcinoma characterized by deceptively bland morphology yet clinically aggressive behavior. We report the case of a 67-year-old woman who presented with a rapidly enlarging sinonasal mass with extensive local invasion involving both the intracranial compartment and orbit. Histologically, the tumor was composed of cytologically bland cells with monotonous nuclei and exhibited a combination of exophytic and endop

A very rare case: superior rectus intramuscular orbital adnexal lipoma.

Lipomas are common benign mesenchymal tumors, often found in the subcutaneous plane. Deeper lipomas situated in intramuscular or retroperitoneal locations are seen less frequently. Orbital lipomas are sporadic, with an incidence varying from 0% to 11%, whereas extraocular muscle lipoma is rare and accounts for only 0.6%. Despite their infiltrative nature and tendency to recur locally, intramuscular lipomas do not have malignant potential. A review of literature revealed only four cases of extraocular muscle lipomas worldwide. Through this paper, we report an indolent case of superior rectus li
