Postmeal walking reduces nighttime central SBP and increases nocturnal dipping of SBP in healthy, physically inactive young women.

INTRODUCTION: Postmeal walking (PMW) blunts postprandial blood glucose, but the impact on postexercise hypotension and ambulatory blood pressure (BP) has not been examined in young women. Further, it remains unknown how PMW influences ambulatory central BP among young women. METHODS: Fourteen physically inactive, nonhypertensive women (20 ± 1 years; body fat: 28.1 ± 12%) completed the study during the early follicular or placebo phase of their contraceptive cycle. Participants completed a control day (CON; no exercise/excess physical activity) and PMW day (three bouts for 15 min of brisk walk

Relationship Between Jump-Landing Kinematics and Lower Extremity Overuse Injuries in Physically Active Populations: A Systematic Review and Meta-Analysis.

BACKGROUND: Lower extremity overuse injuries are common in athletes participating in sports with repeated bouts of landing manoeuvres. Biomechanical alterations during landing may be associated with these types of injuries. The objective of this systematic review with meta-analysis was to summarise and determine the relationship between kinematic alterations during a landing task and the development of lower extremity overuse injuries in physically active populations. METHODS: PubMed, Embase, Web of Science, CINAHL, and SPORTDiscus were consulted up to and including February 2020. Cohort, cro

Exertional Heat Stroke: Pathophysiology, Epidemiology, Diagnosis, Treatment, and Prevention.

Temperature increases due to climate changes and operations expected to be conducted in hot environments make heat-related injuries a major medical concern for the military. The most serious of heat-related injuries is exertional heat stroke (EHS). EHS generally occurs when health individual perform physical activity in hot environments and the balance between body heat production and heat dissipation is upset resulting in excessive body heat storage. Blood flow to the skin is increased to assist in dissipating heat while gut blood flow is considerably reduced, and this increases the permeabil

Beyond aerobic intensity: Comparing the effects of cognitively engaging gross-motor coordination training and aerobic exercise on working memory and selective attention in school-age children: A randomized controlled trial.

BACKGROUND: Working memory and selective attention develop rapidly during middle childhood and are sensitive to physical activity. Aerobic exercise is thought to act mainly through intensity-driven physiological pathways (cerebral perfusion, neurotrophic signalling and arousal), whereas gross-motor coordination training additionally imposes perceptual, decision-making, motor-planning and inhibitory demands that may provide stronger and more domain-specific cognitive stimulation. This study compared 14-week gross-motor coordination and aerobic exercise interventions, designed a priori to be mat

Distinct loading conditions reveal various patterns of right ventricular adaptation.

Right ventricular (RV) failure due to chronically abnormal loading is a main determinant of outcome in pulmonary hypertension (PH) and congenital heart disease. However, distinct types of RV loading have been associated with different outcomes. To determine whether the adaptive RV response depends on loading type, we compared hemodynamics, exercise, and hypertrophy in models of pressure overload due to pulmonary artery banding (PAB), pressure overload due to PH, combined pressure and volume overload, and isolated volume load. Ninety-four rats were subjected to either PAB, monocrotaline-induced

Detrimental effect of combined exercise training and eNOS overexpression on cardiac function after myocardial infarction.

It has been reported that exercise after myocardial infarction (MI) attenuates left ventricular (LV) pump dysfunction by normalization of myofilament function. This benefit could be due to an exercise-induced upregulation of endothelial nitric oxide synthase (eNOS) expression and activity. Consequently, we first tested the hypothesis that the effects of exercise after MI can be mimicked by elevated eNOS expression using transgenic mice with overexpression of human eNOS (eNOSTg). Both exercise and eNOSTg attenuated LV remodeling and dysfunction after MI in mice and improved cardiomyocyte maxima

Left ventricular remodeling with exercise in hypertension.

We investigated how exercise training superimposed on chronic hypertension impacted left ventricular remodeling. Cardiomyocyte hypertrophy, apoptosis, and proliferation in hearts from female spontaneously hypertensive rats (SHRs) were examined. Four-month-old SHR animals were placed into a sedentary group (SHR-SED; n = 18) or a treadmill running group (SHR-TRD, 20 m/min, 1 h/day, 5 days/wk, 12 wk; n = 18). Age-matched, sedentary Wistar Kyoto (WKY) rats were controls (n = 18). Heart weight was greater in SHR-TRD vs. both WKY (P < 0.01) and SHR-SED (P < 0.05). Morphometric-derived left ventricul

Role of proinflammatory cytokines and redox homeostasis in exercise-induced delayed progression of hypertension in spontaneously hypertensive rats.

Hypertension is a well-known risk factor for various cardiovascular diseases. Recently, exercise has been recommended as a part of lifestyle modification for all hypertensive patients. However, the precise mechanisms of exercise training (ExT)-induced effects on the development of hypertension are poorly understood. Therefore, we hypothesized that chronic ExT would delay the progression of hypertension in young spontaneously hypertensive rats (SHRs). In addition, we explored whether the beneficial effects of chronic ExT were mediated by reduced proinflammatory cytokines and improved redox stat

The value of DDD pacing in patients with an implantable cardioverter defibrillator.

Although the beneficial effects of DDD pacing are well known, currently available ICDs provide only fixed rate ventricular antibradycardia pacing. In a consecutive series of 139 patients with ICDs, we have analyzed the need for antibradycardia pacing and the indications for DDD pacing. We also report our initial experience with the Defender 9001 (ELA Medical, France) DDD-ICD. Out of 139 patients, 25 (18%) were in need of antibradycardia pacing. Ten patients already had a pacemaker at the time of ICD implantation and ten other patients had a conventional pacemaker indication at that time. Five

Wearable Sensors Detect Movement Differences in the Portable Warrior Test of Tactical Agility After mTBI in Service Members.

INTRODUCTION: Assessment of functional recovery of service members following a concussion is central to their return to duty. Practical military-relevant performance-based tests are needed for identifying those who might need specialized rehabilitation, for evaluating the progress of recovery, and for making return-to-duty determinations. One such recently developed test is the 'Portable Warrior Test of Tactical Agility' (POWAR-TOTAL) assessment designed for use following concussion in an active duty population. This agility task involves maneuvers used in military training, such as rapid stan

Should we cross the valve: the risk of retrograde catheterization of the left ventricle in patients with aortic stenosis.

CLINICAL SCENARIO: A 67-year-old man is referred to your cardiology clinic complaining of worsening angina and dyspnea on exertion. Physical examination reveals a harsh grade IV/VI late-peaking crescendo-decresendo systolic murmur, loudest at the upper sternal border. The aortic closure sound is diminished. Echocardiography demonstrates left ventricular hypertrophy, an ejection fraction of 50%, no evidence of mitral regurgitation, and severe aortic stenosis (AS) with a peak aortic gradient of 4.8 m/s (92 mm Hg) and a mean aortic gradient of 55 mm Hg. You schedule him for coronary angiography b
