Health-related quality of life in adult patients with esophageal atresia--a questionnaire study.

BACKGROUND/PURPOSE: Long-term sequelae caused by associated anomalies or respiratory and gastrointestinal disorders are common after the repair of esophageal atresia (EA). The aim of this study was to assess the effect of these sequelae on the health-related quality of life (HRQoL) of patients with EA. METHODS: A questionnaire including a 36-item Gastrointestinal Quality of Life Index, a 15-item Respiratory Symptoms-Related Quality of Life Index, and a 36-item psychosocial survey and a symptoms query was sent to 159 patients with EA with or without tracheoesophageal fistula (TEF), who were op

Development and Internal Validation of a Web-based Tool to Predict Sexual, Urinary, and Bowel Function Longitudinally After Radiation Therapy, Surgery, or Observation.

BACKGROUND: Shared decision making to guide treatment of localized prostate cancer requires delivery of the anticipated quality of life (QOL) outcomes of contemporary treatment options (including radical prostatectomy [RP], intensity-modulated radiation therapy [RT], and active surveillance [AS]). Predicting these QOL outcomes based on personalized features is necessary. OBJECTIVE: To create an easy-to-use tool to predict personalized sexual, urinary, bowel, and hormonal function outcomes after RP, RT, and AS. DESIGN, SETTING, AND PARTICIPANTS: A prospective, population-based cohort study wa

Survey of transfemoral amputee experience and priorities for the user-centered design of powered robotic transfemoral prostheses.

BACKGROUND: Transfemoral amputees experience a complex host of physical, psychological, and social challenges, compounded by the functional limitations of current transfemoral prostheses. However, the specific relationships between human factors and prosthesis design and performance characteristics have not yet been adequately investigated. The present study aims to address this knowledge gap. METHODS: A comprehensive single-cohort survey of 114 unilateral transfemoral amputees addressed a broad range of demographic and clinical characteristics, functional autonomy, satisfaction and attitudes

The Ready for Safe Cancer Treatment (RESET) tool: enhancing quality and optimizing timely and safe discharge with transitional care adjustments for Head and Neck Cancer patients.

<b>Introduction:</b> The Ready for Safe Cancer Treatment (RESET) tool is a structured, multimodal program designed to optimize hospitalization and enhance the quality of perioperative care for oncological patients. While RESET has been developed as a general framework for surgical oncology, its adaptation to Head and Neck Cancer (HNC) patients remains unexplored despite their unique physiological and psychological needs. HNC patients frequently face severe functional impairments affecting speech, swallowing, and respiration, as well as heightened nutritional deficiencies, sarcopeni

[Burden on the caregiver of people with disabilities secondary to neurological damage. A multicenter study in Argentina].

INTRODUCTION: Caregiver burden encompasses the physical, financial, and psychosocial impact of caring for a person, constituting a risk factor for the deterioration of the caregiver's health and quality of life. The primary objective was to quantify caregiver burden in primary caregivers of individuals with disabilities secondary to neurological damage in four healthcare centers in Argentina. The secondary objective was to explore its association with the duration of disability, level of functional independence, hours of care, and the caregiver's gender. MATERIALS AND METHODS: This was a mult

Psychosocial stress and autoimmune thyroid disease: a systematic review of clinical phenotypes and vulnerability factors.

Psychological stress has been proposed as a potential trigger or modulator of autoimmune thyroid diseases, but evidence remains fragmented. This systematic review examined the relationship between stress and thyroid autoimmunity, with particular attention to Hashimoto's thyroiditis and Graves' disease, highlighting the critical methodological distinction between stressor exposure (objective events) and the subjective stress response (psychological burden). PubMed, Embase, Scopus, and PsycINFO were searched for controlled human studies; 25 studies involving a total of N = 117,034 participants m

Cross-cultural Adaptation and Validation of the Spanish Version of the Generic Body-Focused Repetitive Behavior Scale-45 (GBS-45) for the Assessment of Rhinotillexomania.

INTRODUCTION: Body-focused repetitive behaviours (BFRBs) may lead to significant physical and psychosocial consequences. Rhinotillexomania is a common and underdiagnosed behaviour for which no validated Spanish-language instruments are currently available for specific assessment. The Generic Body-Focused Repetitive Behavior Scale-45 (GBS-45) is a transdiagnostic tool designed to assess the severity and functional impact of BFRBs. The aim of this study was to validate and cross-culturally adapt the rhinotillexomania-specific section of the GBS-45 for a Spanish-speaking population. MATERIALS AN

Dietary Supplementation with Omega-3 Fatty Acids Rescues Neurodevelopmental Deficits in a Combined Exposure Paradigm of Maternal Oxycodone Exposure and Chronic Stress.

In recent years, opioid abuse has become a major public health crisis affecting millions of individuals throughout the world. This widespread abuse of prescription opioids and dramatic increase in their availability have created the opioid epidemic. Further, oxycodone (oxy), which is prescribed for multiple types of pain, is gaining momentum as a pain reliever in pregnant women. Whilst successful in inducing analgesia, oxy freely passes through the placenta and can impact the overall development of the fetus in major ways. Currently, mechanisms that lead to impairment in the exposed offspring,

Effect of housing insecurity on dermatological conditions among PLHIV in Kenya.

Dermatological conditions are a leading cause of non-fatal disability globally and are highly prevalent among people living with HIV (PLHIV). Housing insecurity is widespread and disproportionately affects PLHIV, exposing them to environmental and psychosocial stressors that can trigger or exacerbate inflammatory and infectious skin conditions and undermine HIV care. However, evidence directly linking housing insecurity to dermatological outcomes among PLHIV remains scarce. Therefore, this study investigates the relationship between housing insecurity and skin conditions among PLHIV. Data were

Indian psychiatrist at the crossroads of transgender care: Gatekeeper, advocate, or redundant authority?

ICD-11 marks a conceptual shift by recognizing gender incongruence as not a mental disorder while retaining it within the classification under sexual health to facilitate healthcare access, coding, and insurance reimbursement. Thus, its continued inclusion is primarily an administrative necessity rather than a reflection of psychopathology. Although psychiatrists have increasingly viewed their role as facilitating care rather than policing identity, mandatory psychiatric documentation has often been experienced by transgender individuals as gatekeeping. The 2026 amendments to the Transgender P

A Trim21-GSDMD Axis Links Psychosocial Stress to IL-1β-Driven Neuroinflammation and Blood-Brain Barrier Dysfunction in a Rat Model of Repeated Social Defeat.

The molecular trigger that transduces psychosocial stress into the release of the key pro-inflammatory cytokine interleukin-1β (IL-1β) has been a missing link in stress-related neuropathology. We identify the E3 ubiquitin ligase tripartite motif-containing protein 21 (TRIM21) as this critical upstream regulator. In a rat model of repeated social defeat stress, TRIM21 was upregulated in the hippocampus, brain microvasculature, and peripheral blood mononuclear cells (PBMCs). This upregulation correlated with anxiety-like behavior and cognitive deficits and was more pronounced in females. Mechani

Emotional Factors in inclusive early childhood education: An integrated STEM+SEL programme in a preschool context.

Affective factors, operationalised in this study as SEL-aligned emotional factors, shape children's participation, social adjustment, and learning in inclusive early childhood classrooms, yet STEM education is still frequently enacted as a primarily cognitive endeavour with social-emotional learning (SEL) treated as peripheral. This mixed methods study examined an integrated STEM+SEL programme designed to support emotion regulation, empathy, perseverance/resilience, and belonging-oriented participation in a preschool context in Türkiye. Using a convergent mixed methods design, quantitative dat

The home within care: exploring home-like design in paediatric/adolescent palliative care environments.

This study explores the integration of home-like design elements in paediatric/adolescent palliative care inpatient units, drawing on perspectives from both medical and architectural professionals. Recognising the unique developmental and psychosocial needs of young patients, this research explores how the spatial and emotional qualities associated with 'home' are interpreted and operationalised in palliative care environments. A multimethod approach was employed, including a targeted literature review, analysis of seven international paediatric hospice design and semistructured interviews wit

Palliative care in fragility hip fracture: An empirical narrative review of an underrecognised need.

BACKGROUND: Fragility hip fractures are associated with high morbidity, mortality, and complex care needs that extend beyond acute management. Despite this, palliative care remains underutilised in this population. Consequently, palliative care needs in people with fragility hip fractures remain insufficiently recognised, assessed and integrated into care pathways. OBJECTIVE: To identify, synthesise, and analyse key concepts emerging from the literature on palliative care in fragility hip fractures, with a focus on their interrelationships and implications for care. METHODS: An empirical nar

Neuroendocrine and neuroinflammatory mechanisms in postpartum depression: From hormonal withdrawal to central immune dysregulation and emerging therapeutic targets.

Postpartum depression (PPD) affects approximately 10-15% of new mothers worldwide with profound consequences extending to offspring and partners. While the classical "hormonal withdrawal hypothesis" has long emphasized the abrupt decline in estradiol, progesterone, and cortisol following childbirth, emerging evidence demonstrates that these endocrine perturbations converge with dysregulated neuroimmune pathways as central drivers of PPD pathophysiology. This narrative review integrates mechanistic evidence linking reproductive hormone withdrawal, HPA axis dysregulation, neurotransmitter imbala

Palliative care in cardiovascular medicine.

Cardiovascular disease remains the leading global cause of morbidity and mortality. Although advances in prevention, diagnostics, and disease-modifying therapies have prolonged survival, many individuals now live longer with high symptom burden, functional decline, and complex decisional needs. Palliative care (PC) for adults with advanced cardiovascular disease can improve quality of life, support caregivers, and align treatments with patient values and goals. Core elements include symptom management, effective communication, shared decision-making, advance care planning, and integration of p
