Survival of infants born with hypoplastic left heart syndrome (HLHS) has steadily improved since Norwood and colleagues first reported a multistage reconstructive approach in 1983. Although several experienced centers have achieved operative survival for the Norwood procedure between 63-94%, this procedure still remains a challenging step with high mortality for many institutions. Despite successful reconstructive surgery, most deaths occur in the first 24 to 48 hours after surgery due to hemodynamic instability secondary to unpredictable rapid fall in pulmonary resistance. Therefore, many eff…

UNLABELLED: The authors present the incidence of Kallmann's syndrome in two families in four persons. In one family the syndrome was present in a boy and the brother of his mother. In the second family in siblings -- a boy and a girl. The two boys at preschool age underwent a surgery because of bilateral cryptorchism. In the first patient and his uncle additional hearing dysfunction and agenesis of the left kidney were diagnosed. In all the patients anosmia was diagnosed. All the patients receive a pharmacological treatment which improved significantly their clinical state, caused the developm…

Vascular Ehlers-Danlos syndrome, also known as Ehlers-Danlos syndrome type IV, is a life-threatening inherited disorder of connective tissue, resulting from mutations in the COL3A1 gene coding for type III procollagen. Vascular EDS causes severe fragility of connective tissues with arterial and gastrointestinal rupture, and complications of surgical and radiological interventions. As for many rare orphan diseases, delay in diagnosis is common, even when the clinical features are typical, leading to inadequate or inappropriate treatment and management. In childhood many individuals with vascula…

The thoracic outlet region contains three important structures: the brachial plexus, the subclavian artery, and the subclavian vein. As they travel from the upper mediastinum to the upper extremity, these structures run through three important spaces: the interscalene triangle, the costoclavicular space, and the subpectoral space. Compression can occur in any of these three spaces because of structural anomalies or trauma.

Polycystic ovary syndrome (PCOS) is a common condition characterised by menstrual abnormalities and clinical or biochemical features of hyperandrogenism. Features of PCOS may manifest at any age, ranging from childhood (premature puberty), teenage years (hirsutism, menstrual abnormalities), early adulthood and middle life (infertility, glucose intolerance) to later life (diabetes mellitus and cardiovascular disease). While pelvic ultrasound examination is useful, many women without PCOS have polycystic ovaries; ultrasound evidence is not necessary for the diagnosis. Testing for glucose intoler…

The history and clinical findings of 30-year man with Alport's syndrome are presented. The main features of Alport's syndrome are hereditary chronic nephritis leading to renal failure resulting in death and sensory-neural deafness. Ocular features include anterior lenticonus, macular and peripheral flecks resembling fundus albipunctatus. The etiology of this syndrome is unknown. It has been suggested that there may be a metabolic defect in the biosynthesis of collagen with changes in the glomerular basement membrane, cochlea and capsule of the lens. The retinal flecks may also be related to th…

This article describes a case of a 44-year-old male patient, with history, of tuberculosis who was admitted to the hospital with high temperature, coughing, haemoptysis, arterial hypertension and end-stage renal failure. In the differential diagnosis of haemoptysis we took into account pneumonia, exacerbation of tuberculosis, pulmonary neoplasm and Goodpasture syndrome. Presence of antibodies against the glomerular basement membrane (GBM) and histopathologic examination of biopsy specimen confirmed the diagnosis of Goodpasture syndrome. Due to advanced renal failure haemodialysis therapy was i…

Carpenter syndrome (Acrocephalopolysyndactyly type II), first described in 1901, consists of acrocephaly, syndactyly, polydactyly, congenital heart disease, mental retardation, hypogenitalism, cryptorchidism, obesity, umbilical hernia and bony abnormalities. We report a 6 years old boy presenting as a union of these malformations and also having bilateral sensorineural hearing loss. Auditory disturbances are not common among Carpenter syndrome patients. According to our knowledge, this is the first Carpenter syndrome case whose hearing loss is demonstrated by auditory brainstem response (ABR) …

INTRODUCTION: Short rib-polydactyly syndrome (SRPD) is an autosomal recessive, lethal skeletal dysplasia. It is characterized by short limb dwarfism, short ribs with thoracic hypoplasia, polydactyly, and multiple anomalies of major organs.
CASE REPORT: We report a case of SRPD subtype II (Majewski) that was detected in the 36th week of gestation, showing hydropic change, narrow thorax, shortened limbs, protuberant abdomen, micromelia, polydactyly and extremely low set ears, depressed nasal bridge, and cleft palate. The family was informed of the fatal outcome of the condition. Delivery was in…

Scimitar syndrome is a rare congenital anomaly. This syndrome is characterized by partial or complete anomalous pulmonary venous drainage of the right lung to the inferior vena cava. There is a characteristic abnormal radiographic shadow which descends along the right cardiac border (scimitar sign). We examined 71-year old woman with severe pulmonary hypertension due to a large shunt between pulmonary veins and right atrium. Other cause of pulmonary hypertension is atrial septum defect. Our patient required permanent pacemaker implantation for tachy-brady syndrome.

The main manifestation of Korsakoff's syndrome is a disproportionate impairment in memory in comparison to other cognitive functions. Most prominent is anterograde amnesia for both verbal and nonverbal information. The marked sensitivity to interference is the main characteristic of memory impairment of these patients. Retrograde amnesia is also a typical feature of this disorder. Memory for autobiographical information as well as knowledge of public events and facts are affected and they have a temporal gradient. Confabulations may also occur but they are not specific to Korsakoff's syndrome.…

The history of nephrotic syndrome can go back to beginning of the era. In old Alexandria, urine was conceived to be produced in kidneys. In 1827, Bright classified diseases with proteinuria into three categories, and lipoid nephrosis seemed to be included in group 1. Mechanisms of urinary protein leakage have been in great controvesies, and nowadays defects of slit-membrane protein are supposed to play important roles. In the middle of the 20th centuries, electron microscopy and renal biopsy technique were adopted in practical use. Since then, the treatment and research of nephrotic syndrome h…

In minimal change nephrotic syndrome (MCNS), selective proteinuria is associated with structural alterations of the glomerular epithelial cells, such as effacement of the foot process. Although the pathogenesis of MCNS has not been completely clarified, clinical and experimental observations suggest that it results from T cell dysfunction in the pathogenesis. Recently, it has been proposed that the occurrence of MCNS has been associated with type 2 Th (Th2) lymphocyte-dependent conditions and some vascular permeability factors, which are induced by T cell disorder. In general, MCNS has the goo…

Post-acute myocardial infarction (AMI) syndrome was first described by Dressler in 1956. Its incidence has decreased in the reperfusion era, most likely because of the extensive use of thrombolysis and coronary balloon angioplasty, therapies that dramatically decreased the size of myocardial necrosis. The authors suggest that drugs that have been prescribed in previous decades as the post-AMI "standard-of-care," such as angiotensin-converting enzyme inhibitors, beta-blockers, and statins, may also play an important role in the disappearance of Dressler syndrome due to their immunomodulatory ef…

Adrenalectomy is a radical therapeutic approach to control hypercortisolism in some patients with Cushing's disease. However it may be complicated by the Nelson's syndrome, defined by the association of a pituitary macroadenoma and high ACTH secretion after adrenalectomy. This definition has not changed since the end of the fifties. Today the Nelson's syndrome must be revisited with new to criteria using more sensitive diagnostic tools, especially the pituitary magnetic resonance imaging. In this paper we will review the pathophysiological aspects of corticotroph tumor growth, with reference t…
