Toll-like receptor 7 deficiency promotes survival and reduces adverse left ventricular remodelling after myocardial infarction.

AIMS: The Toll-like receptor 7 (TLR7) is an intracellular innate immune receptor activated by nucleic acids shed from dying cells leading to activation of the innate immune system. Since innate immune system activation is involved in the response to myocardial infarction (MI), this study aims to identify if TLR7 is involved in post-MI ischaemic injury and adverse remodelling after MI. METHODS AND RESULTS: TLR7 involvement in MI was investigated in human tissue from patients with ischaemic heart failure, as well as in a mouse model of permanent left anterior descending artery occlusion in C57B

Mechanism of harm from left bundle branch block.

The impact of left bundle branch block (LBBB) on cardiac mechanical function ranges from minimal effect in some patients to marked reduction in left ventricular (LV) systolic function in others. It appears that this variability in part reflects differences in anatomical location of the bundle block. In most patients with LBBB and congestive heart failure, however, there is associated cardiac disease such as cardiomyopathies or coronary artery disease which contributes to LV dysfunction. The mechanism of harmful effect of LBBB on cardiac function is in-coordinated ventricular contractions which

Cardiac Structural Remodeling, Longitudinal Systolic Strain, and Torsional Mechanics in Lean and Nonlean Dysglycemic Chinese Adults.

BACKGROUND: Diabetes mellitus and pre-diabetes mellitus are associated with lower body mass indices and increased risk of cardiovascular events (including heart failure) at lower glucose thresholds in Chinese compared with Western cohorts. However, the extent of cardiac remodeling and regulation on cardiac mechanics in lean and nonlean dysglycemic Chinese adults is understudied. METHODS AND RESULTS: We studied 3950 asymptomatic Chinese (aged 49.7±10.7 years; 65% male; body mass index: 24.3±3.5 kg/m2) with comprehensive echocardiography including speckle tracking for left ventricular global lo

Echocardiographic phase imaging to predict reverse remodeling after cardiac resynchronization therapy.

OBJECTIVES: The aim of our study was to investigate whether echocardiographic phase imaging (EPI) can predict response in patients who are considered for cardiac resynchronization therapy (CRT). BACKGROUND: CRT improves quality of life, exercise capacity, and outcome in patients with bundle-branch block and advanced heart failure. Previous studies used QRS duration to select patients for CRT; the accuracy of this parameter to predict functional recovery, however, is controversial. METHODS: We examined 42 patients with advanced heart failure (New York Heart Association [NYHA] functional class

Chemotherapy-Induced Right Ventricular Dysfunction.

The right ventricle has been considered by many clinicians to be the "forgotten chamber" of the heart. In this narrative review of the literature, we aim to examine chemotherapy-induced right ventricular failure. We will discuss the clinical significance of the subject, the effects of chemotherapy on the heart, and why it is crucial to take a closer look at the right ventricle. We will present key aspects of the anatomy and physiology of the right ventricle and how it differs from the left ventricle. Furthermore, this review explains the mechanisms behind chemotherapy-induced right ventricular

Right heart failure due to loss of right ventricular capture in a patient with atrioventricular junction ablation and biventricular pacing.

We describe the case of a patient with atrioventricular (AV) junction ablation and chronic biventricular pacing in which intermittent dysfunction of the right ventricular (RV) lead resulted in left ventricular (LV) stimulation alone and onset of severe right heart failure. Restoration of biventricular pacing by increasing device output and then performing lead revision resolved the issue. This case provides evidence that LV pacing alone in patients with AV junction ablation may lead to severe right heart failure, most likely as a result of iatrogenic mechanical dyssynchrony within the RV. Thus

Technical aspects of the combined arterial switch and senning operation for congenitally corrected transposition of the great arteries.

Congenitally corrected transposition of the great arteries (ccTGA) is a rare defect characterized by discordant atrioventricular and ventriculoarterial connections. Symptoms result from one or a combination of associated cardiovascular malformations, including ventricular septal defect, pulmonary stenosis or atresia, and tricuspid valve dysfunction. Survival of patients with ccTGA is often determined by the associated anomalies, but even in isolation ccTGA results in a diminished life expectancy. Regurgitation of the tricuspid valve and decreased morphologic right ventricular function tend to

Paradoxical embolisation of a catheter fragment to a coronary artery in an infant with congenital heart disease.

Six weeks after cardiac catheterisation via the right femoral vein an infant underwent an anatomical correction for transposition of the great vessels and closure of a large ventricular septal defect. The patient died intraoperatively as a result of severe left ventricular dysfunction. Necropsy showed an old myocardial infarction associated with thrombosis of the circumflex coronary artery. Electron probe x ray analysis showed that foreign refractile material within the thrombus contained bismuth, an element used in the construction of some cardiac catheters. This is believed to be the first r

Stress echocardiography for evaluation of hemodynamics: valvular heart disease, prosthetic valve function, and pulmonary hypertension.

A broad variety of diseases can affect dynamic cardiac function and contribute to exertional symptoms. Although hemodynamic data at rest may be sufficient to guide therapy in some patients with cardiovascular disease, medical and surgical decisions may be impacted by the cardiac response to stress and the ability to associate exertional symptoms with cardiac hemodynamic response. Stress echocardiography/ Doppler provides a unique noninvasive means to evaluate cardiovascular function and ehmodynamic responses during increased cardiac work. Specifically, stress echocardiographic/Doppler imaging

Impaired cardiovascular function in primary biliary cirrhosis.

Cardiovascular system dysregulation in the form of autonomic dysfunction is common at all stages of the disease process in the autoimmune liver disease primary biliary cirrhosis (PBC) and associates with the symptom of fatigue. The mechanisms underpinning autonomic dysfunction in PBC are, however, at present unclear. In this study we set out to explore, for the first time, cardiac structure and function in PBC using impedance cardiography (ICG) and magnetic resonance methodologies. ICG was assessed beat to beat in response to orthostasis (by head-up tilt) in age and sex case-matched high-fatig

Changes in overall ventricular myocardial architecture in the setting of a porcine animal model of right ventricular dilation.

BACKGROUND: Chronic pulmonary regurgitation often leads to myocardial dysfunction and heart failure. It is not fully known why secondary hypertrophy cannot fully protect against the increase in wall stress brought about by the increased end-diastolic volume in ventricular dilation. It has been assumed that mural architecture is not deranged in this situation, but we hypothesised that there might be a change in the pattern of orientation of the aggregations of cardiomyocytes, which would contribute to contractile impairment. METHODS: We created pulmonary valvular regurgitation by open chest, s

Arrhythmogenic right ventricular cardiomyopathy: current diagnostic and management strategies.

Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a heart muscle disease of unknown etiology characterized by the peculiar right ventricular (RV) involvement. Distinctive pathologic features are myocardial atrophy and fibro-fatty replacement of the RV free wall, and clinical presentation is usually related to ventricular tachycardias with a left bundle branch block pattern or ventricular fibrillation leading to cardiac arrest, mostly in young people and athletes. Later in the disease evolution, progression and extension of RV muscle disease and left ventricular involvement may result i

Dysrhythmias after the modified Fontan procedure.

The cardiac rhythm before and after the modified Fontan procedure was reviewed in 24 patients. Transient atrial dysrhythmias were common in the immediate postoperative period. Late postoperative premature atrial contractions were detected by ambulatory monitoring in 20 of 23 patients; eight (34.8%) had supraventricular tachycardia. Late ventricular dysrhythmia was detected in 18 of 23 patients: ten had low-grade ventricular dysrhythmias and eight (34.8%) had multiform premature ventricular contractions. Five of the latter had couplets and one of these five plus another had ventricular tachycar

[Takotsubo myocardiopathy with cardiac tamponade].

Takotsubo cardiomyopathy or stress cardiomyopathy is characterized by left ventricular dysfunction, usually hypokinesia of the apical segments and basal hypercontractility, with symptoms suggestive of acute myocardial infarction in the absence of coronary atherothrombotic disease. In most cases it has a benign course. We present the case of a patient with takotsubo cardiomyopathy with cardiac tamponade secondary to hemopericardium with subsequent cardiorespiratory arrest requiring emergency pericardiocentesis.

Coronary vasodilation without myocardial erection. Simultaneous haemodynamic, echocardiographic and arteriographic findings during adenosine and dipyridamole infusion.

AIM: The aim of this study was to evaluate simultaneously echocardiographic, haemodynamic and angiographic changes that occur during adenosine and dipyridamole infusion, in patients with one-vessel coronary artery stenosis. This would assess whether deterioration in left ventricular haemodynamics during vasodilator agent infusion is influenced by vasodilation per se, or the development of myocardial ischaemia. METHODS AND RESULTS: We performed adenosine (140 micrograms.kg-1.min-1 over 4 min) and dipyridamole (up to 0.84 mg.kg-1 over 10 min) stress echocardiography tests, together with angiogr

Comparative assessment of chordal preservation versus chordal resection in mitral valve replacement for mitral stenosis.

Mitral valve replacement with preserving all chordae tendineae in patients with mitral regurgitation has been proved to be beneficial for left ventricular performance in the postoperative period. To evaluate the effectiveness of this technique in patients with mitral stenosis a comparison of the hemodynamic and echocardiographic data between patients having operation with this technique (Group P, n = 15, mean age = 37.5 +/- 12 years), and those having operation with the conventional method of mitral valve replacement (Group C, n = 15, mean age = 39 +/- 10.4 years) was made. Association between time-domain heart rate variability, diastolic dysfunction and unplanned readmission to cardiovascular department in older type 2 diabetes mellitus patients.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is a risk factor for the development of left ventricular diastolic dysfunction (LVDD). Reduced heart rate variability (HRV) was linked to autonomic nervous system dysfunction and higher risk of cardiovascular mortality. HRV has been associated with systolic dysfunction, recent studies indicate its relevance to diastolic dysfunction. The objective of our study was to investigate the association between time-domain HRV, diastolic dysfunction and unplanned readmission (UR) in older patients with T2DM. METHODS: Older patients with T2DM were included in t

Assessing Cardiac Mechanical Dysfunction in Transfusion-Dependent β-Thalassemia With History of Atrial Fibrillation: The Role of Speckle Tracking Echocardiography.

PURPOSE: Atrial fibrillation (AF) is highly prevalent in patients with transfusion-dependent beta-thalassemia (TDT). Speckle tracking echocardiography (STE) provides detailed information about left ventricular (LV) and atrial function, however its role in TDT patients with AF has not been completely investigated. This study aimed to assess differences in cardiac mechanical parameters between thalassemia patients with and without history of AF. METHODS AND RESULTS: 223 TDT patients in sinus rhythm were enrolled and, among them, 26 (11%) had history of AF. A complete STE analysis with the evalu
