Since 1985 to 1990, 153 isolates of non typhoidic Salmonella were recovered from pediatric unit of the Charles Nicolle Hospital in Tunis. The epidemiological profile of these isolates was established according to the biochemical and serological characterizations, the antibiotic susceptibility patterns (disk diffusion method) to betalactams and aminoglycosides. 127 isolates of Salmonella spp were from stools and 22 from blood cultures. The main serotypes were S. ser. Wien (108 isolates) and S. ser. Typhimurium (21). A sample of 50 isolates was selected among the different resistance phenotypes

This study investigated the genetic diversity and occurrence of blaNDM variants associated with carbapenem resistance in Enterobacterales obtained from colonization (rectal swabs) and infection sites in patients hospitalized in a public hospital in northeastern Brazil. A total of 58 multidrug-resistant clinical isolates were analyzed using ERIC-PCR to assess clonal relatedness, and PCR followed by amplicon sequencing to detect resistance genes (blaNDM, blaKPC, blaGES, blaVIM, and blaIMP). Klebsiella pneumoniae was the most frequently identified species (n = 28, 48.27%), followed by Serratia ma

Escherichia coli is a ubiquitous bacterium that acts both as a commensal and a pathogen, serving as a major reservoir of antimicrobial resistance genes (ARGs). The global spread of ARGs, particularly those conferring resistance to last-resort antibiotics such as carbapenems and colistin, poses a serious threat to human and animal health. In this study, we analyzed 9,696 publicly available E. coli genomes from human, animal, and unknown-origin metadata, identifying 42,813 acquired ARG occurrences across ten antibiotic classes. Multidrug resistance (ARGs from ≥ 3 classes) was observed in 44.7% o

Targeted-Antibacterial-Plasmids (TAPs) offer a precise approach to combat multidrug-resistant bacteria by selectively removing resistant strains while preserving commensals. Here, we assess TAPs that deliver CRISPR/Cas systems via conjugation to kill or resensitize extended-spectrum β-lactamase (ESBL)-producing Escherichia coli carrying the blaCTX-M-15 gene. We systematically tested multiple variables in the TAP approach, including two commensal E. coli donors, three distinct helper plasmids encoding the transfer machineries, and six recipient strains harbouring the resistance gene either chro

INTRODUCTION: The incidence of carbapenemase-producing (CP) Escherichia coli has increased worldwide, with the first reports in Europe dating back to the early 2000s. As part of the European Genomic Reference Laboratory Capacity Building (EURGen-RefLabCap) project, Slovenia conducted its first national genomic surveillance study of CP-E. coli. The aim was to characterize CP-E. coli isolates recovered from Slovenian patients between 1 January and 31 December 2023 using whole-genome sequencing (WGS), providing a national snapshot of the population structure and circulating strains.
METHODS: WGS

Wastewater antimicrobial resistance (AMR) surveillance is limited in many low- and middle-income countries where wastewater infrastructure is absent or inadequate. Surface water may provide an alternative for AMR surveillance in such settings. We investigated the resistome, virulome, and phylogenetic relationships of ESBL-producing Escherichia coli (ESBL-Ec) from surface water (SW), healthy residents (HR), and a clinical facility (CL) to determine whether SW isolates reflect human-associated AMR. During an eight-month longitudinal study in South Africa, 95 ESBL-Ec isolates (SW = 32, HR = 32, C

Nacubactam is a novel bicyclic non-β-lactam β-lactamase inhibitor that acts as a penicillin-binding protein (PBP-2) active antibacterial, almost exclusively cleared by renal excretion. This study assessed the effect of renal function and hemodialysis on nacubactam pharmacokinetics following a single intravenous (IV) 1 g infusion. Part 1 included three subjects with normal renal function, and six each with mild, moderate, or severe renal impairment; Part 2 included eight subjects with end-stage renal disease undergoing hemodialysis. We observed a strong linear relationship between renal functio

This study was conducted to investigate the effects of heavy metals on the resistance levels exhibited by extended-spectrum β-lactamase (ESBL)-producing and non-ESBL-producing Escherichia coli isolated from cattle farms and raw milk. The findings indicated that ESBL-producing E. coli strains exhibited significantly higher resistance to Cu, Zn, and Cd than non-ESBL-producing strains. Furthermore, ESBL-producing strains harbored various plasmid-mediated heavy metal resistance genes. Whole-genome sequencing (WGS) results showed that ESBL-producing strains carried plasmids encoding 148 heavy metal

In India, most carbapenem-resistant Pseudomonas aeruginosa strains produce metallo-β lactamases, and cefiderocol, the sole reliable treatment option, is currently not available. This study evaluated aztreonam/avibactam with fosfomycin as an alternative in 85 MBL-producing isolates. Aztreonam/avibactam and fosfomycin inhibited 7.1% and 27.1% of isolates at 8 and 32 mg/L, respectively, while cefiderocol at 4 mg/L inhibited 78.8%. Time-kill assays on 10 isolates showed synergy in 7/10 and enhanced activity in 6/10 with the combination versus monotherapy. These findings warrant further in vitro an

This study systematically elucidates how ST88-KL186 Klebsiella aerogenes evolved carbapenem resistance through a dual synergistic mechanism involving porin deficiency and inducible AmpC β-lactamase, providing a mechanistic explanation for carbapenem resistance independent of carbapenemase production. From seven homologous strains (single-nucleotide polymorphism [SNP] difference 3-30) consecutively isolated from a patient with post-pancreaticoduodenectomy infection, three subsequent ascites isolates (R5-R7) developed carbapenem resistance after imipenem treatment. All strains carried the OmpK36

We analyzed antimicrobial resistance markers in 189 Neisseria gonorrhoeae patient encounters from 2 US military medical centers. Co-occurring resistance-associated markers were common; we detected plasmid-mediated β-lactamase in extragenital sites. Our findings highlight the importance of anatomic site-specific surveillance and, potentially, molecular antimicrobial resistance detection to guide screening and treatment strategies.

Immunocompromised patients, especially hematopoietic stem cell transplants (HSCT) / bone marrow transplant recipients, are at risk for colonization and infection with carbapenem-resistant Klebsiella pneumoniae (CR-KP) which is associated with increased bloodstream infections and mortality in this population. In this susceptible population, we investigated the molecular epidemiology of CR-KP. The strains isolated over a five-year period (2017-2021) were then characterized by PCR and sequencing targeting carbapenemase genes. ERIC-PCR was utilized to examine clonal relations, while multilocus seq

BACKGROUND: Carbapenem-resistant Pseudomonas aeruginosa (CRPA) represents a critical threat to clinical therapy, yet the genetic basis underlying extremely high-level carbapenem resistance remains poorly understood. An emerging CRPA clone, ST1976, has recently been identified in China with unusually high meropenem MICs.
METHODS: ST1976 CRPA isolates collected between 2018 and 2022 were subjected to antimicrobial susceptibility testing, whole-genome and Nanopore sequencing, and bioinformatic analyses. Cloning assays and sodium mercaptoacetic acid inhibition tests were performed to investigate

Salmonella serovar Stanley ST29 is an emerging foodborne pathogen with community spread potential, while its carbapenem resistance remains uncommon. In a longitudinal pediatric case with diarrhea, two clonal ST29 isolates sampled 2 months apart revealed within-host evolutionary dynamics: Tn7051-mediated excision of blaNDM-5 from an IncHI2/ST3 plasmid. Moreover, phylogenomics connected the case isolate to strains from a healthy carrier and from food-chain-associated sources in the same region (17-18 SNPs), underscoring community dissemination and the need for One-Health surveillance.

Carbapenems are a class of β-lactam antibacterial drugs with a broad antibacterial spectrum and strong activity, commonly used to treat serious bacterial infections. However, improper or excessive use of carbapenems can lead to increased bacterial resistance, which is a significant concern as they are often used as last resort for treating multidrug-resistant (MDR) gram-negative bacteria. Confronted with this challenge, it is crucial to comprehensively understand the mechanism of carbapenem resistance to develop effective therapeutic strategies and innovative drugs. In recent years, emerging r
