Characterizing the Mutational Landscape of In-Transit Melanoma Metastases.

In-transit melanoma (ITM) is a unique presentation of metastatic cutaneous melanoma associated with therapeutic challenges. Despite its clinical importance, the molecular drivers of ITM remain poorly defined. We aimed to characterize the mutational landscape of ITM and identify genetic alterations distinguishing it from other melanoma metastases. Tumor samples from 528 patients in the MSK-IMPACT dataset, comprised of over 300 cancer-related genes, were analyzed. Samples were classified as primary, in-transit, regional lymph node, or distant metastases. Driver mutation frequencies were compared

Clinical indications and implications of metastasis-directed therapy and primary-site treatment in metastatic urothelial carcinoma.

Metastatic urothelial carcinoma (mUC) is traditionally managed with systemic therapy; however, long-term disease control remains uncommon despite advances such as immune checkpoint inhibitors and antibody-drug conjugates. Interest has grown in integrating metastasis-directed therapy (MDT), including metastasectomy and stereotactic body radiotherapy (SBRT), and primary-site treatment (PST) in selected patients, aiming to reduce tumor burden, eliminate resistant clones, and enhance systemic therapy outcomes. Metastasectomy is not standard but may be considered for oligometastatic patients with g

Patient-derived models of prostate cancer: Capturing tumour complexity from initiation to metastasis.

Prostate cancer is a growing global health challenge. To identify new ways to improve patient care, researchers need a variety of preclinical models that faithfully recapitulate human tumours across the disease continuum, from initiation to metastasis. These complementary models include primary cultures of prostate epithelial cells (PrECs), co-cultures, patient-derived explants (PDEs), patient-derived organoids (PDOs) and patient-derived xenografts (PDXs). Collectively, these models enable researchers to study tumour biology and therapeutic responses in clinically relevant contexts. Yet, there

Beyond the barrier: Engineering the tumor-immune-soil nexus-a mechanistic blueprint for integrating Traditional Chinese Medicine with immunotherapy in metastatic colorectal cancer.

Metastatic colorectal cancer (mCRC) presents a stark immuno-oncologic dichotomy: while microsatellite instability-high (MSI-H) tumors respond to immune checkpoint inhibitors (ICIs), the vast majority of microsatellite-stable (MSS) cases remain refractory, underscoring a critical need for novel strategies to convert "cold" tumors into "hot" ones. This review advances the hypothesis that Traditional Chinese Medicine (TCM) can serve as a formula-defined ecological conditioner within the tumor-immune-soil nexus. We propose a mechanistic blueprint where TCM formulas, rather than acting as nonspecif

Methodology of murine lung cancer mimics clinical lung adenocarcinoma progression and metastasis.

Lung cancer is the leading cause of cancer-related deaths, of which adenocarcinoma is the most common subtype. Despite this, lung adenocarcinoma and its metastasis are poorly understood, due to difficulties in feasibly recapitulating disease progression and predicting clinical benefits of therapy. We outline a methodology to develop immunogenic orthotopic lung adenocarcinoma mouse models, by injecting cell-specific cre viruses into the lung of a genetically engineered mouse, which mirrors cancer progression defined by the International Association for the Study of Lung Cancer. Evaluation of di

Unlocking the Role of Metabolic Pathways in Brain Metastatic Disease.

The dissemination of malignant cells to the brain is a late-stage complication of cancer, leading to significant morbidity and mortality. Brain metastases (BM) affect 20-30% of cancer patients, primarily originating from lung cancer, breast cancer, and melanoma. Despite advances in molecular-targeted therapies, brain metastatic disease remains incurable, with a poor median survival of ≤12 months if left untreated. The lack of therapeutic efficacy is mainly attributed to the presence of the blood-brain barrier (BBB) and genetic differences between BM and their primary tumors. Previously publish

Functional assessment of migration and adhesion to quantify cancer cell aggression.

During epithelial-to-mesenchymal transition (EMT), cancer cells lose their cell-cell adhesion junctions as they become more metastatic, altering cell motility and focal adhesion disassembly associated with increased detachment from the primary tumor and a migratory response into nearby tissue and vasculature. Current in vitro strategies characterizing a cell's metastatic potential heavily favor quantifying the presence of cell adhesion biomarkers through biochemical analysis; however, mechanical cues such as adhesion and motility directly relate to cell metastatic potential without needing to

Homing Effect Directed Cuprous Delivery Activates Cuproptosis Against Nasopharyngeal Carcinoma and Metastasis.

Cuproptosis, a newly discovered cell death mechanism driven by cuprous ions (Cu(I)), faces challenges in achieving efficient pathway activation due to limitations in tumor cell targeting and intracellular copper accumulation. To address these, a NIR-triggered nanoplatform (Cu(I)-TA@HG) is developed for precise Cu(I) delivery to metastatic nasopharyngeal carcinoma (NPC) cells, thereby inducing potent cuproptosis. Drawing inspiration from the critical role of membrane proteins in cancer metastasis, tumor-homing nanovesicles derived from GAS6-overexpressing cell membranes (HG-CMs) are engineered.

Current treatment options for locally advanced and metastatic basal cell carcinoma. A narrative review.

INTRODUCTION: Although basal cell carcinomas (BCC) are the most common skin cancer and usually considered as 'easy-to-treat,' locally advanced BCC (laBCC) and metastatic BCC (mBCC) are rather exceptional and often more 'difficult-to-treat.' They load a high burden on the quality of life (QoL) of the patients, often elderly and frail individuals. Several management options are possible, varying from supportive therapy without any therapeutic intervention until anti-programmed cell death protein-1 (PD-1) immunotherapy, such as cemiplimab, either administered intravenously or intralesional. LDH-driven lactic acidosis in hypoxic solid tumors: Mechanisms of metastatic transformation and therapeutic opportunities.

Lactic acidosis is a characteristic feature of solid hypoxic cancerous tumors, those that develop in the breast, colon, and prostate tissue. Even though extreme lactic acidosis is damaging to healthy cells, malignant tumors actually benefit from it in several different ways. Lactic acidosis in TME (TME) imparts resistance to chemotherapy and helps them from immune invasion. Lactic acidosis further benefits the tumor cells by inducing the formation of new blood vessels. Acidic tumour microenvironment (TME) provides very favourable pH conditions for activation of proteolytic enzymes like Matrix

CTC cluster in breast cancer: synergetic metastasis promotion mechanism and transformation path from platelet cloaking to leukocyte escort.

Circulating tumor cell (CTC) clusters are potent drivers of metastasis in breast cancer, and their survival and dissemination within the circulatory system are critically governed by dynamic interactions with the host microenvironment. Two key mechanisms-"platelet cloaking" and "leukocyte escort"-have emerged as central axes that amplify the metastatic potential of these clusters. This review synthesizes these parallel yet intersecting pathways, elucidates the biological basis for their synergistic interplay, and proposes a translational roadmap from mechanistic insight to clinical application

NN-PCP: Screening phenotype-related core pathways to construct a prostate cancer metastasis prediction model based on multiple types of mutation data.

BACKGROUND AND OBJECTIVE: Prostate cancer causes >400,000 deaths annually worldwide, being the fifth leading cause of cancer-related death in men. However, accurate prediction of prostate cancer metastasis based on mutations remains a challenge. METHODS: We proposed a novel neural network constructed with phenotype-related core pathways, called NN-PCP, to accurately predict prostate cancer metastasis. NN-PCP comprises three main modules: IORA-driven modules, IGSEA-driven modules, and a hierarchy with two differential layers. IORA-driven modules are constructed using highly variable genes and

Impact of SPOP Mutations on Clinical Outcomes in Metastatic Prostate Cancer.

PURPOSE: Previous studies suggest that SPOP mutations result in increased sensitivity to androgen receptor pathway inhibitors (ARPIs) but are limited by lack of granular data. We hypothesize that SPOP mutations are associated with improved outcomes across the spectrum of advanced prostate cancer (PC). METHODS: Using the real-world clinicogenomic Prostate Cancer Precision Medicine Multi-Institutional Collaborative Effort database, we analyzed outcomes based on SPOP mutation status. The primary end point was overall survival (OS) from the diagnosis of metastatic disease. Secondary end points in

[Metastatic Squamous Cell Carcinoma with Unknown Primary Origin in the Mediastinal Lymph Node].

We report a case of a patient with squamous cell carcinoma metastasis confined to a mediastinal lymph node, with no identifiable primary tumor. The patient remains cancer-free 2 years after diagnosis. Although various theories exist regarding the etiology of lymph node carcinoma of unknown primary origin, advances in elucidating its pathogenesis and integrating treatment strategies based on genetic profiling are expected to form the foundation of future clinical practice.

Capture of circulating metastatic cancer cell clusters from lung cancer patients can reveal unique genomic profiles and potential anti-metastatic molecular targets: A proof-of-concept study.

Metastasis remains the leading cause of cancer deaths worldwide and lung cancer, known for its highly metastatic progression, remains among the most lethal of malignancies. Lung cancer metastasis can selectively spread to multiple different organs, however the genetic and molecular drivers for this process are still poorly understood. Understanding the heterogeneous genomic profile of lung cancer metastases is considered key in identifying therapeutic targets that prevent its spread. Research has identified the key source for metastasis being clusters of cells rather than individual cancer cel

[Advances of Fundamental Research on Traditional Chinese Medicine in Regulation of Tumor-associated Macrophages for the Prevention and Treatment of
Lung Cancer Metastasis].

Lung cancer is the leading cause of cancer-related deaths worldwide, with metastasis being the primary cause of mortality in lung cancer patients, and its prevention and control efficacy remain limited. In recent years, immunotherapy has emerged as a promising direction for overcoming the bottleneck of metastasis. Macrophages, as essential components of innate immunity, participate in the entire process of tumor initiation and progression. Tumor-associated macrophages (TAMs) represent the most abundant immune population in the tumor microenvironment (TME), displaying both anti-tumor M1-like an
