A survey of serious infections due to Haemophilus influenzae, in the years 1973-1984, was made at the Birmingham Children's Hospital. Eight-three cases were recorded, comprising 42 of meningitis, 18 of septicaemia, 16 of epiglottitis and seven of septic arthritis or osteomyelitis. The mortality rate for meningitis and septicaemia was 5%, but no child with epiglottitis or septic arthritis died. The data are compared with those of American studies. Between Jan 1st 1978 and June 30th 1981 we treated 98 patients with multiple fractures of the lower extremities. Of a total of 260 fractures 86,2% were operated following the rule of primary definitive treatment. The postraumatic osteomyelitis rated 8,2%; the lethality was 16,3% due to other concomitant injuries of polytraumatized patients. The different patterns of fractures required individual operative-planning. Since in patients with multiple fractures of the lower limbs mobilisation is difficult, early operation to achieve training-stability proved to be valuable.

CRP levels have been found to be helpful in differential diagnosis and in following the clinical course and response to treatment of various disorders. Postoperative surgical site infection is one of the serious postoperative complications of spine surgery, especially fungal infections. Late-stage surgical site fungal infections often lack typical clinical symptoms and have a variable clinical presentation. Primary carcinoma of the middle ear is extremely rare. A 41-year-old woman with a history of skull base osteomyelitis and chronic suppurative right otitis media presented with 1 month of right-sided facial droop, tearing, and headaches. Initial head CT revealed bony destruction and soft tissue opacification involving the right middle ear. Surgical biopsy yielded poorly differentiated carcinoma. Subsequent F-FDG PET/CT and MRI scans demonstrated FDG-avid soft tissue in the right middle ear with associated heterogeneously enhancing soft tissue on MRI.

Staphylococcus lugdunensis is a virulent coagulase-negative staphylococcus (CoNS) that behaves like Staphylococcus aureus. Toxic shock syndrome, osteomyelitis, septic arthritis and postoperative endopthalmitis have been observed. Restoration of digital extension after chronic extensor loss has not been detailed extensively in the literature. The present report details an unusual case of composite tissue loss from the dorsum of the hand after a chronic burn wound. After debridement for chronic carpal osteomyelitis and free-tissue transfer were performed, staged wrist fusion and two-stage extensor tendon reconstruction resulted in a stable, pain-free wrist and functional digital extension. The present case illustrates that two-stage extensor tendon reconstruction, when necessary, is indeed feasible.

Infectious sinusitis may on occasion be associated with meningitis, subdural empyema, epidural empyema, brain abscess, or osteomyelitis. We report a 29-year-old male patient with frontal sinusitis who developed all of these intracranial complications due to two previously unreported causative organisms, Neisseria sicca and Eubacterium lentum. Chronic rhinocerebral mucormycosis, as distinct from the acute fulminating form, has been previously unreported in the otolaryngology literature. We have observed two cases of chronic rhinocerebral mucormycosis with osteomyelitis and bony sequestration at 8 months and 2 years after apparent control of initial infection. A white girl with sickle O-Arab disease (Hb S/O-Arab) had three separate episodes of acute splenic sequestration at 8, 12, and 15 months of age with hemoglobin levels falling to 3.9, 2.8, and 4.2 g/dl, respectively. Functional hyposplenism was suggested by the radionucleotide spleen scan. Following splenectomy her hemoglobin stabilized in the range of 0.7--8.5 g/dl. Life-threatening episodes of splenic sequestration, pneumococcal infections and osteomyelitis may occur in both Hb S/O-Arab and homozygous sickle cell diseases.

Cryptococcosis is a disseminated infection of man and animals that involves many organs, especially the central nervous system. Isolated bone involvement may cause lytic bone lesions. Diagnosis may be made by pathologic sections and culture of specimens obtained through surgical biopsy.
