Incontinentia pigmenti (IP) is a rare genetic condition which is X-linked and predominantly affects the structures arising from neuroectodermal origin such as the skin, teeth, eyes, hair, nails and central nervous system. The dermatological manifestations are seen as the first developing signs. The dental manifestations are also commonly seen. Here we discuss a case of a girl in her early adolescence who was diagnosed with a case of IP. The dental features present in this case were typical, including missing permanent teeth, retained deciduous teeth, microdontia and delayed eruption.

Incontinentia pigmenti (IP) is a rare X-linked dominant neurocutaneous ectodermal disorder. It is caused by IKBKG gene mutations, leading to nuclear factor-kappa B (NF-kB) signaling pathway dysfunction. Skin involvement occurs in almost all patients. IP may also involve teeth, eyes, hair, central nervous system, bone structures, skeletal musculature and immune system. However, endocrine involvement, and specifically the coexistence of IP with complete pituitary aplasia, has not been previously documented in a patient with genetically confirmed IP. We describe a four-year-old female with geneti…

A female infant presented with asymptomatic hyperpigmented and hypopigmented verrucous papules and plaques along Blaschko's lines on the right side of her body, with an absence of blistering, flexural accentuation or family history. A systemic examination was normal. Differentials considered were epidermal nevus, incontinentia pigmenti stage 2, lichen striatus, inflammatory linear verrucous epidermal nevus and linear epidermolytic ichthyosis (EI). A skin biopsy revealed epidermolytic hyperkeratosis (EHK). Mutation analysis could not be done as the parents were unwilling. EHK is classically see…

Incontinentia pigmenti (IP) is a rare, X-linked dominant genodermatosis caused by mutations in the IKBKG gene and characterized by sequential cutaneous stages following the lines of Blaschko. The highly inflammatory initial vesiculobullous stage frequently mimics severe neonatal infections, such as herpes simplex or bullous impetigo, leading to potentially dangerous diagnostic delays and unnecessary antimicrobial therapies. The objective of this report is to highlight the utility of multimodal medical imaging and clinico-pathologic correlation in the rapid diagnosis of early-stage IP. We prese…

A term female neonate presented with vesicular skin lesions along Blaschko's lines and refractory seizures in the first week of life. Brain MRI revealed multifocal T2/fluid-attenuated inversion recovery hyperintensities and restricted diffusion in the periventricular and subcortical white matter, basal ganglia, thalami, corpus callosum and cerebellum, with microhaemorrhages on susceptibility-weighted imaging. These imaging findings, along with clinical history, suggested incontinentia pigmenti (IP), prompting genetic testing, which confirmed a heterozygous IKBKG exon 4-10 deletion. Seizures we…

INTRODUCTION: This article reports a detailed case of a patient with incontinentia pigmenti who exhibited epileptic status and dermatologic symptoms.
CASE PRESENTATION: A 5-month-old female patient was brought to our hospital due to status epilepticus, with erythematous vesicular skin lesions on her trunk and extremities. Routine magnetic resonance imaging revealed infarction, ischemia, and encephalomalacia. Skin biopsy pathology indicated pigmentation disorder. Molecular genetic testing was conducted to identify IKBKG mutations, and the case was finally diagnosed with incontinentia pigmenti …

Incontinentia pigmenti (IP) is caused by loss-of-function variants in IKBKG, with molecular genetic diagnosis complicated by a pseudogene. We describe seven individuals from three families with IP but negative clinical testing in whom long-read sequencing identified causal variants. Concurrent methylation analysis explained disease severity in one individual who died from neurologic complications, identified a mosaic variant in an individual with an atypical presentation, and confirmed skewed X-chromosome inactivation in an XXY individual.

BACKGROUND AND OBJECTIVES: DNA variations in the NF-kappa-B essential modulator (NEMO) gene are linked to incontinentia pigmenti (IP) and also immunodeficiency and autoinflammatory conditions. Some patients with IP present with neonatal vasculitis-like brain changes, although pathogenesis is unclear. We investigated cell-specific gene expression in a neonate with IP, who had encephalopathy, seizures, and vasculitis-like brain changes, and responded to steroid treatment.
METHODS: Single-cell RNA (ribonucleic acid) sequencing (scRNAseq), using the HIVE single-cell system, was performed on a neo…

This case describes a seven-month-old male infant with incontinentia pigmenti (IP), a rare X-linked dominant disorder that is typically lethal in male fetuses. The infant presented with blaschkoid hyperpigmentation but showed no neurologic, dental, or ocular abnormalities. Genetic testing revealed a normal karyotype without IKBKG (inhibitor of nuclear factor kappa B kinase regulatory subunit gamma) mutations, suggesting possible somatic mosaicism, a rare survival mechanism in male patients with IP. This case underscores the rarity of IP in male infants and highlights the need for multidiscipli…

Rare pediatric retinal disorders present significant challenges in diagnosis and management due to their limited prevalence and diverse clinical manifestations. This paper provides a comprehensive review of select rare retinal disorders affecting the pediatric population, focussing a brief on their epidemiology, clinical characteristics, diagnostic modalities, and therapeutic interventions. Through a systematic examination of current literature and clinical case studies, this review aims to elucidate the distinct features and challenges associated with each disorder. Despite the rarity of thes…

OBJECTIVE: To provide preimplantation genetic testing (PGT) for a patient with Incontinentia pigmenti (IP) due to IKBKG gene variant but without family samples through construction of single nucleotide polymorphism (SNP)-based haplotype by Long-read sequencing (LRS) technology.
METHODS: A female IP patient with a heterozygous IKBKG c.1167dup variant but without family genetic data who sought genetic counseling at Women and Children' Hospital of Ningbo University in November 2021 was selected as the study subject. The IKBKG gene has a highly homologous pseudogene IKBKGP1. Genomic DNA was extra…

Incontinentia pigmenti (IP) is a rare hereditary disorder affecting 1.2 in 100,000 live births, predominantly females. Genetic analysis of IP is complicated by a homologous pseudogene, making conventional short-read sequencing challenging. While long-range PCR is typically used to overcome this, skewed X-inactivation detection can also aid in assigning variants to IKBKG. We employed a comprehensive approach, incorporating whole-exome sequencing (WES), long-range PCR, RT-PCR, X-inactivation analysis, and nanopore sequencing, to identify and accurately phase a small heterozygous deletion, NM_001…

Congenital and neonatal skin disorders encompass a broad spectrum of conditions ranging from transient benign rashes to severe genodermatose … dermolysis bullosa, epidermolytic hyperkeratosis in ichthyoses, and eosinophilic spongiosis in incontinentia pigmenti). Many disorders form part of broader syndromes with distinctive cutaneous and systemic features. Recent advances in immunofluorescence mapping and genetic testing have improved diagnostic precision, while emerging therapies (e.g., gen …
