The Impact of a Preoperative Cognitive Behavioural Therapy (CBT) on Dysfunctional Eating Behaviours, Affective Symptoms and Body Weight 1 Year after Bariatric Surgery: A Randomised Controlled Trial.

BACKGROUND: To examine whether a preoperative cognitive behavioural therapy (CBT) intervention exceeds usual care in the improvements of dysfunctional eating behaviours, mood, affective symptoms and body weight 1 year after bariatric surgery. METHODS: This is a 1-year follow-up of a single centre parallel-group randomised controlled trial ( http://clinicaltrials.gov/ct2/show/NCT01403558). A total of 80 (55 females) patients mean (SD) age 44 (10) years were included. The intervention group received 10 weeks of CBT prior to bariatric surgery, and the control group received nutritional support a

The relation between insomnia symptoms, mood, and rumination about insomnia symptoms.

STUDY OBJECTIVES: Research suggests that rumination may play an important role in insomnia. Whereas some have suggested that rumination mainly relates to depression, the evidence suggests that there may be insomnia-specific rumination. This paper explores insomnia symptom rumination across two distinct samples of varying levels of depressed mood and insomnia symptom severity. METHODS: The first sample consisted of nonclinical participants (N = 327) with a range of insomnia and depressed mood symptoms, and the second sample consisted of those who met both Major Depressive Disorder and Insomnia

[Disorders of psychoneurohumoral interrelations in patients suffering from arterial hypertension].

To elucidate disorders of psychoneurohumoral relationships in patients with grade I-II arterial hypertension at low, moderate and high overall cardiovascular risk, we measured body mass index, activity of sympathetic nervous system, serum leptin level and performed psychological tests aimed to elucidate situational and personal anxiety, aggressiveness and hostility. The data obtained allow a patient to be referred to one of the three possible types of disturbed psychoneurohumoral interrelations, viz. anxiety-metabolic, anxiety-adrenergic, and aggressive-metabolic, each having its own specific

Symptoms at presentation in children with sleep-related disorders.

OBJECTIVE: To assess the link between sleep-disordered breathing and cognitive function in children. To identify correlations among polysomn … f intelligence (WPPSI-R), a neuropsychological test battery (NEPSY) and a parental questionnaire on behavioral symptoms (CBCL) were administered. RESULTS: Frequently snoring children scored lower in Language functions (Comprehension of Instructions, P=0.01; Speeded naming, P=0.007) and had more internalizing problems, P=0.04 than did the non-snoring group. However, …

Does executive impairment define a frontal variant of Alzheimer's disease?

BACKGROUND: People with Alzheimer's disease (AD) who present with prominent frontal features such as a dysexecutive syndrome may be difficul … osed that subjects with dementia presenting with greater executive impairment but without prominent behavioral symptoms are likely to have AD rather than FTLD, especially if they are quite functionally impaired. With time FTLD subjects develop increasing executive dysfunction and increasingly resemble the more severely affected AD subjects.

Factors that predict infant disorganization in mothers classified as U in pregnancy.

The unresolved (U) state of mind in parents has been validated by its association with infant attachment disorganization (D), yet all studies show a transmission gap, and a proportion of individuals classified as U have infants who are not D. This paper reports on 31 mothers who showed the characteristic lapses in thinking and reasoning of the unresolved/disorganized state of mind in relation to stillbirth (U(sb)), when assessed with the Adult Attachment Interview (AAI) in the pregnancy after stillbirth. Seventeen (55%) of their infants were D at 1 year old. We evaluate social, attachment, and

Alexithymia and health behaviors in healthy male volunteers.

The association between alexithymia and maladaptive health behaviors was evaluated in 118 young, healthy men, aged 18-45 years. Subjects completed the Toronto Alexithymia Scale (TAS-26), and a health behaviors questionnaire, measuring alcohol and drug use, sedentary lifestyle, poor nutritional consumption, and risky sexual practices. In forced hierarchical regression analyses, the association between alexithymia and health behaviors was evaluated after adjusting for age, body mass index, social support, ambivalence over expression of emotion, and the expression of emotion. Results indicated th

X linked severe mental retardation, craniofacial dysmorphology, epilepsy, ophthalmoplegia, and cerebellar atrophy in a large South African kindred is localised to Xq24-q27.

To date over 150 X linked mental retardation (XLMR) conditions have been documented. We describe a five generation South African family with XLMR, comprising 16 affected males and 10 carrier females. The clinical features common to the 16 males included profound mental retardation (100%), mutism despite apparently normal hearing (100%), grand mal epilepsy (87.5%), and limited life expectancy (68.8%). Of the four affected males examined, all had mild craniofacial dysmorphology and three were noted to have bilateral ophthalmoplegia and truncal ataxia. Three of 10 obligate female carriers had mil

Family stress, family functioning and emotional/behavioural problems following child psychiatric treatment.

This study assesses family stress, family functioning and emotional/behavioural problems in a sample of 80 formerly daytreated or residentially treated boys after a mean discharge period of four years. Family stress was assessed with a Questionnaire of Life Events (QLE) and family functioning with the Family Adaptability and Cohesion Evaluation Scales (FACES). Emotional/behavioural problems were measured with the Child Behavior Checklist (CBCL). The results showed that there were significant relationships between the QLE, FACES and CBCL scores. In particular, experiencing less negative life ev

Emergence of social behavior deficit, blunted corticolimbic activity and adult depression-like behavior in a rodent model of maternal maltreatment.

Disrupted social behavior is a core symptom of multiple psychiatric and neurodevelopmental disorders. Many of these disorders are exacerbated by adverse infant experiences, including maltreatment and abuse, which negatively affect amygdala development. Although a link between impaired social behavior, abnormal amygdala function and depressive-like behavior following early adversity has been demonstrated in humans and animal models, the developmental emergence of maltreatment-related social deficits and associated amygdala neural activity are unknown. We used a naturalistic rodent model of mate

John Hunter: early association of Type A behavior with cardiac mortality.

Free-floating (easily aroused) hostility is a cardinal symptom of the type A behavior pattern, the first psychosocial factor scientifically linked to coronary artery disease (CAD). Anger and hostility are associated with the onset and outcome of CAD, triggering of myocardial infarction, and lowering the threshold to ventricular arrhythmia, all increasing the risk of sudden cardiac death. The life of the legendary eighteenth century English surgeon John Hunter is illustrative of the type A behavior pattern. His demise and that of a contemporary surgeon provide important lessons for the manageme

Urinary frequency: going beyond the tract.

Obsessive-compulsive disorder (OCD) is a common anxiety disorder which can be chronic and sustained. An OCD sufferer experiences intrusive and repetitive thoughts, impulses, and behaviors, which ultimately cause extreme discomfort. We report a case of a patient that primarily presented with lower urinary tract symptoms who was subsequently treated with antibiotics. Nonetheless, the symptoms persisted. In subsequent consultations, the patient clarified the compulsive nature of his symptoms and was treated as a case of OCD. Therefore, it is crucial for physicians to correctly identify the nature

Psychiatric Manifestation in Patients with Parkinson's Disease.

Parkinson's disease (PD) is the second most common neurodegenerative disorder. Although its major manifestation is motor symptoms, resulting from the loss of dopaminergic neurons in the substantia nigra, psychiatric symptoms, such as depression, anxiety, hallucination, delusion, apathy and anhedonia, impulsive and compulsive behaviors, and cognitive dysfunction, may also manifest in most patients with PD. Given that the quality of life - and the need for institutionalization - is so highly dependent on the psychiatric well-being of patients with PD, psychiatric symptoms are of high clinical si

Brief report: Lack of correlation between age of symptom onset and contemporaneous presentation.

The parents/guardians of 50 individuals were surveyed using a semistructured interview to determine the feasibility of this method and to establish ages of symptom onset. Thirty-eight informants were able to recall sufficient detail to allow categorization of the age of symptom onset. Chi-square analysis confirmed a significant association between investigators' categorization and informants' categorization. Contemporaneous presentation was indexed using Childhood Autism Rating Scale, the Autism Behavior Checklist, the Conners Hyperactivity Index, and the Ritvo-Freeman Real Life Rating Scale f

Characterizing changes in obsessive-compulsive symptoms over the course of treatment for adolescent bulimia nervosa.

OBJECTIVE: Data suggest that obsessive-compulsive (OC) symptoms are commonly observed in adolescents with eating disorders and predict poorer treatment response. Further, emerging data among adults suggest that changes in OC symptoms relate to changes in eating disorder symptoms across treatment. Given evidence that early invention decreases risk for protracted illness, evaluating processes that may relate to treatment response will be useful in increasing the effectiveness of existing interventions. Therefore, the current investigation explored changes in general and eating disorder-specific

Natural History of Psychological Symptoms in Individuals With Rome IV Irritable Bowel Syndrome and Association With Gastrointestinal Symptom Severity.

BACKGROUND AND AIMS: Psychological symptoms are common in irritable bowel syndrome (IBS) and are associated with more severe gastrointestinal symptoms. However, data describing the relationship between psychological symptoms and IBS symptom severity, and how this changes over time, are limited. We aimed to characterize the longitudinal course of psychological symptoms in individuals with Rome IV IBS and examine their association with gastrointestinal symptom severity over the same time period. METHODS: We recruited individuals with self-reported IBS from three UK community-based organizations

Radiofrequency as the New Opportunity in Treating Overactive Bladder and Urge Urinary Incontinence-A Single-Arm Pilot Study.

Background and Objectives: Until now, overactive bladder (OAB) with or without urge urinary incontinence (UUI) has been treated mainly in two ways: with behavioral methods and patient education, or using antimuscarinic drugs and/or beta-3 adrenergic receptor agonists. Unfortunately, these drugs may cause side effects in some women or are insufficiently effective, so patients abandon them. Therefore, in this pilot study, radiofrequency was evaluated as a new option in the treatment of OAB and UUI. Materials and Methods: Nineteen patients were enrolled in this pilot study using radiofrequency (R

Headache and psychological variables as predictors of disability in individuals with primary headache disorders.

OBJECTIVE: To examine the relative contribution of headache symptoms and psychological factors to headache-related disability. BACKGROUND: Both headache symptoms and comorbid psychological factors (psychiatric symptoms and transdiagnostic constructs) negatively impact functioning among individuals with migraine and tension-type headache, but few studies have explored their relative contribution to headache-related disability. We hypothesized that psychiatric symptoms and transdiagnostic variables would afford incremental contribution to disability beyond headache symptoms, and we investigated
