Roles of circulating proteins in osteoporosis pathogenesis: Insights from Mendelian randomization.

BACKGROUND: Osteoporosis (OP) poses a significant health burden. Circulating proteins represent promising drug targets. However, previous Mendelian randomization (MR) studies have limited evidence for their causal role due to single-source protein quantitative trait loci (pQTL) data and lack of systematic, multi-layered validation. OBJECTIVE: To identify and prioritize circulating proteins causally implicated in OP using a proteome-wide MR framework integrating five pQTL datasets, Bayesian colocalization, transcriptome-wide analyses, and single-cell expression mapping. METHODS: A two-stage (

Incidence and risk factors of adverse drug reactions in multidrug-resistant/rifampicin-resistant tuberculosis (MDR/RR-TB) regimens containing new drugs: a retrospective study of two national multicenter cohorts.

BACKGROUND: Treatment of multidrug-resistant/rifampicin-resistant tuberculosis (MDR/RR-TB) faces severe challenges, including prolonged courses, marked drug toxicity, and poor patient compliance. While regimens containing new drugs (bedaquiline, delamanid) have substantially improved MDR/RR-TB outcomes, systematic research on the epidemiological characteristics and risk control of adverse drug reactions (ADRs) remains insufficient. This study, based on a national multi-center clinical cohort, retrospectively analyzed data from 2,151 patients, aiming to explore the characteristics and risk fact

Perspectives and Real-World Clinical Practice of Portuguese Dermatologists on Biologic Dose Spacing in Psoriasis: Results from a Nationwide Survey.

INTRODUCTION: Biologic therapies have transformed the management of moderate-to-severe psoriasis but are associated with long-term treatment burden and substantial healthcare costs. Dose spacing, defined as extending dosing intervals in patients with controlled disease, has emerged as a potential optimization strategy. However, data on real-world implementation and clinician perspectives remain limited. METHODS: We conducted a national, cross-sectional survey among Portuguese dermatologists experienced in prescribing biologic therapies for psoriasis. An anonymous, web-based questionnaire asse

Pharmacological and non-pharmacological therapies for chronic pancreatitis pain: a narrative review.

IMPORTANCE: Chronic pancreatitis (CP) pain is the major driver of morbidity, reduced quality of life, healthcare use, and opioid exposure in affected patients. Pain in CP is heterogeneous and often correlates poorly with structural pancreatic abnormalities alone. It may arise from overlapping obstructive, neuropathic, nociplastic, and psychosocial mechanisms, making simple stepwise analgesic escalation insufficient for many patients. OBSERVATIONS: The emerging evidence supports a multidimensional strategy for treating CP pain. In some patients, pain is driven mainly by ductal obstruction, sto

Investigating rituximab-induced infusion-related reactions in patients with non-Hodgkin lymphoma, with a focus on follicular lymphoma: a retrospective observational study.

INTRODUCTION: The incidence of infusion-related reactions (IRRs) caused by rituximab is very high, and many studies have explored factors related to the onset of IRR; however, most analyses have been conducted on a mixed range of disease indications. This study aimed to investigate rituximab-induced IRR in patients with non-Hodgkin lymphoma (NHL), particularly follicular lymphoma (FL), and examine the factors related to its occurrence. METHODS: Data on sociodemographic and blood test findings were collected on the closest possible date prior to rituximab administration. Cytokine/chemokine leve

Whole-blood coagulation and platelet dynamics identify differential prothrombotic states in patients with rheumatoid arthritis treated with JAK or TNF-ɑ inhibitors.

BACKGROUND: Rheumatoid arthritis (RA) is associated with inflammation-driven hypercoagulability and increased venous thromboembolic risk. Post hoc analyses of safety trials have raised concerns regarding a potential differential thrombotic risk with Janus kinase inhibitors (JAKi) compared with tumour necrosis factor inhibitors (TNFi). OBJECTIVES: To compare viscoelastic coagulation profiles and platelet reactivity in patients with RA treated with JAKi or TNFi, and in healthy controls (HC). METHODS: In this single-centre observational study, patients with RA with stable JAKi or TNFi therapy (

Hemodynamic resilience and enhanced early recovery with fospropofol versus propofol in high-BMI patients undergoing surgery: a randomised controlled trial.

BACKGROUND: Patients with elevated body mass index (BMI) are particularly susceptible to propofol-induced hypotension during anesthesia induction. Fospropofol, a water-soluble prodrug of propofol, may confer hemodynamic advantages through gradual release kinetics, yet its profile in high-BMI patients remains incompletely characterized. METHODS: This pre-specified secondary analysis of a randomised controlled trial (ChiCTR2500101725) enrolled 120 patients with BMI ≥28 kg/m² scheduled for elective surgery, randomised to fospropofol (FOS, n = 60) or propofol (PRO, n = 60). Impact of housing instability on hospital outcomes in alcohol-associated liver disease: A single-center retrospective cohort study.

Among unhoused individuals, alcohol use significantly contributes to the burden of liver disease. This study examines the effects of being unhoused on hospital outcomes and associated psychiatric conditions with alcohol-associated liver disease (ALD). We conducted a retrospective cohort study of adults hospitalized with ALD from 2020 to 2024 at a tertiary care center. Patients were stratified into housed (reference) and unhoused (exposure) cohorts. Multivariable regression analyses were performed to evaluate the association between housing status and clinical outcomes, using logistic regressio

Pacemaker therapy through a sex-specific lens: a narrative review of clinical and procedural disparities.

Pacemaker therapy is a cornerstone in the management of bradyarrhythmias, yet accumulating data highlight important sex-based disparities across indications, procedural care and long-term outcomes. Women are consistently under-represented among pacemaker recipients, tend to be older at implantation, and are more frequently admitted via emergency pathways despite substantial symptom burden from sinus node dysfunction. Men more often receive pacemakers for atrioventricular conduction disease, at younger ages and with greater cumulative exposure to long-term pacing and device-related procedures.

High-altitude exposure and ischemic stroke: pathophysiological mechanisms and current perspectives.

Stroke remains the second leading cause of death worldwide and the third leading cause of disability-adjusted life year lost. In recent years, environmental and geographic determinants have been increasingly recognized as key contributors to stroke risk. High-altitude environments-characterized by chronic hypoxia, hypobaria, and elevated ultraviolet radiation-exert profound effects on cardiovascular and cerebrovascular physiology and substantially elevate stroke burden. A comprehensive understanding of how high altitude modulates the pathophysiology of ischemic stroke is therefore critical to

Genomic Determinants and Clinical Outcomes of Second Primary Hematological Malignancies in Patients with B-cell Cancers.

Patients with lymphoid malignancies are at increased risk for second primary hematological malignancies (SPHM) due to reduced immune surveillance, genetic predispositions, and previous cancer therapies, including chemotherapy. SPHMs can substantially increase morbidity and mortality. In this study, we aim to characterize the clinical and genomic features, risk factors, and outcomes of patients who developed SPHMs following therapy for hematological malignancies, including multiple myeloma (MM), chronic lymphocytic leukemia (CLL), and lymphoma. We conducted a retrospective analysis of the incid
