Late blink reflex changes in lesions of thalamus and internal capsule.

A 60-year-old man suddenly suffered from left hemiplegia with sensory loss on the left side of his face, trunk, and extremities. Brain CT disclosed hemorrhage in the right thalamus and internal capsule. Blink reflex with electrical stimulation on the left supraorbital nerve showed normal early response (R1) on the left side but absent late response (R2) on both sides, while stimulation of the right supraorbital nerve showed normal R1 on the right side and normal R2 on both sides. This observation suggests that the impulses of R2 from stimulation of the supraorbital nerve on the affected side m

Cervical spinal cord stimulation for treatment of upper limb paralysis: a narrative review.

Recent advances in cervical spinal cord stimulation (SCS) have demonstrated improved efficacy as a therapeutic intervention for restoring hand functions in individuals with spinal cord injuries or stroke. Accumulating evidence consistently shows that cervical SCS yields significant improvements in grip force, proximal arm strength and muscle activation, with both immediate and sustained effects. This review synthesizes the evidence that electrical stimulations modulate the spinal and supraspinal organization of uninjured descending motor tracts, primarily the residual corticospinal tract, reti

Neuromodulation techniques in traumatic brain injury: a narrative review of the current state.

Traumatic brain injury (TBI) is a leading cause of death and disability throughout the world. Despite significant advances in medical care, many TBI survivors continue to have cognitive, physical, and psychological deficits that have a significant impact on their quality of life. Neuromodulation techniques, which use electrical or magnetic stimulation to modulate brain activity, have shown promise in the treatment of TBI symptoms. The purpose of this narrative review is to provide an overview of the current state of neuromodulation techniques for TBI, such as transcranial magnetic stimulation,

A multiple modulation synthesis method with high spatial resolution for noninvasive neurostimulation.

Noninvasive neurostimulation plays a pivotal role in the direct control of neural circuits and the modulation of neuronal function. However, it is difficult to balance both spatial resolution and penetration depth when stimulating deep neurons. Here, we designed a multiple (time-division, frequency and polarity) modulation synthesis (MMS) method for noninvasively stimulating deep neurons with low-frequency envelopes. Compared to conventional transcranial electrical stimulation, we demonstrated that it can stimulate deep neurons at the desired firing rate (beat frequency) with higher spatial re

Transcranial Magnetic and Direct Current Stimulation in the Treatment of Depression: Basic Mechanisms and Challenges of Two Commonly Used Brain Stimulation Methods in Interventional Psychiatry.

Noninvasive neuromodulation, including repetitive trans-cranial magnetic stimulation (rTMS) and direct current stimulation (tDCS), provides researchers and health care professionals with the ability to gain unique insights into brain functions and treat several neurological and psychiatric conditions. Undeniably, the number of published research and clinical papers on this topic is increasing exponentially. In parallel, several methodological and scientific caveats have emerged in the transcranial stimulation field; these include less robust and reliable effects as well as contradictory clinic

Laryngeal pacing as a treatment for vocal fold paralysis.

We summarize etiologies of vocal fold paralysis and current treatments. The recent literature involving electrical stimulation of the larynx is reviewed. Four canines were involved in a study to test a new laryngeal pacemaker system. This system was used to stimulate both the lateral cricoarytenoid and thyroarytenoid muscles. The data are taken from two of these canines. One of the goals was to stimulate the paralyzed side of the larynx based on the activity of the normal (nonparalyzed) side of the larynx. The best stimulation parameters for full addition of the paralyzed vocal cord were 3-7 V

Chronic cerebellar stimulation in the monkey. Preliminary observations.

In a single monkey, the surface of the cerebellum was stimulated electrically for 205 hours with electrodes and parameter values similar to those currently used in humans for treatment of epilepsy. Im pedance of stimulating and nonstimulating control electrodes remained unchanged throughout an observation period of six months. Potentials evoked by cerebellar stimulation could be recorded from the cranium, providing a noninvasive technique of determining the level of current delivered to cerebellum. Examination of the implantation site showed marked meningeal thickening surrounding the stimulat

Role of intradental A- and C-type nerve fibres in dental pain mechanisms.

The arousal of the two components of pain (the first rapid or sharp pain and the second dull pain) are considered to be related to activation of A delta- and C-type nociceptive primary afferents, respectively. The same dichotomy of pain sensations may also exist in teeth, although due to the short distance between the site of stimulation and the brain the two sensations might not be as clearly separated as in stimulation of, for example, the extremities. The sensations evoked by stimulation of human teeth vary according to the type of the stimuli applied. Low-intensity electrical stimulation i

tDCS induced GABA change is associated with the simulated electric field in M1, an effect mediated by grey matter volume in the MRS voxel.

BACKGROUND AND OBJECTIVE: Transcranial direct current stimulation (tDCS) has wide ranging applications in neuro-behavioural and physiological research, and in neurological rehabilitation. However, it is currently limited by substantial inter-subject variability in responses, which may be explained, at least in part, by anatomical differences that lead to variability in the electric field (E-field) induced in the cortex. Here, we tested whether the variability in the E-field in the stimulated cortex during anodal tDCS, estimated using computational simulations, explains the variability in tDCS

Vagus nerve afferent stimulation: Projection into the brain, reflexive physiological, perceptual, and behavioral responses, and clinical relevance.

The afferent vagus nerves project to diverse neural networks within the brainstem and forebrain, based on neuroanatomical, neurophysiological, and functional (fMRI) brain imaging evidence. In response to afferent vagal stimulation, multiple homeostatic visceral reflexes are elicited. Physiological stimuli and both invasive and non-invasive electrical stimulation that activate the afferent vagus elicit perceptual and behavioral responses that are of physiological and clinical significance. In the present review, we address these multiple roles of the afferent vagus under normal and pathological

Intraoperative aberrant bispectral index values due to facial nerve monitoring.

Bispectral index is an accepted depth of anaesthesia monitor for guiding intraoperative hypnotic agent administration. Frontalis EMG displayed on BIS monitor may increase due to twitching of frontalis muscle. EMG increases are also known to cause artefactual increases in BIS values. We report a case of artefactual increase of EMG and subsequently BIS values, due to electrical artefact from cranial nerve stimulator being used to identify the facial nerve. An explanation of the effect of stimulator signal on BIS EMG and BIS values has been provided.

Engineering evaluation of the energy-storing orthosis FES gait system.

A system to restore walking in the vicinity of a wheelchair for people with paraplegia resulting from spinal cord injury is under development. The approach combines single channel surface electrical stimulation with an orthosis. The orthosis is spring loaded and contains a pneumatic system that stores energy during knee extension caused by quadriceps stimulation and transfers it to hip joint for hip extension. A laboratory version of the prototype of the gait system has been fabricated and engineering bench tests were performed. The paper presents the design of the wearable prototype and resul

Artificial neural network prediction using accelerometers to control upper limb FES during reaching and grasping following stroke.

This work investigates arm acceleration as a control signal for functional electrical stimulation (FES) of the upper limb during reaching and grasping. We segment the reach and grasp motion into phases and present an artificial neural network (ANN) approach that estimates the phase of the reaching cycle from accelerometer signals. We then select the stimulator command that maximizes successful triggering without unnecessary risk to the patient's safety. Our results suggest that the algorithm successfully generalizes between sessions and patients but is less successful at generalizing between d

Efficacy of spinal cord stimulation for neuropathic pain following idiopathic acute transverse myelitis: a case report.

Spinal cord stimulation (SCS) involves the electrical stimulation of dorsal structures within the spinal cord, and is the most widespread application of neurostimulation for the relief of chronic pain. Idiopathic acute transverse myelitis (IATM) is an acute monophasic lesion of the spinal cord that presents with symptoms associated with loss of cord function. The incidence of chronic pain secondary to this condition is unknown, but is considered rare. We report the efficacy of SCS for relief of severe neuropathic pain in both lower limbs secondary to IATM, which had failed to respond to conven

Cardiac sympathetic nerve stimulation enhances cardiovascular performance during hyperkalaemia in the anaesthetized pig.

A rapid increase in arterial plasma potassium concentration to values seen during intense exercise depresses cardiac function at rest. Increasing the cardiac concentration of noradrenaline by right-sided sympathetic stimulation in eleven anaesthetized pigs significantly augmented cardiovascular performance during hyperkalaemia, while electrical pacing of the right atrium at equivalent rates to sympathetic stimulation afforded no protection against the deleterious effects of hyperkalaemia. We conclude that the inotropic effect of sympathetic activation may be important in sustaining cardiac fun

A technique for nerve reconstruction following resection of soft-tissue sarcoma.

Primary nerve grafting should be considered whenever tumor extirpation necessitates resection of an adjacent critical nerve. A technique of nerve grafting is presented which distinguishes motor from sensory fascicles, using intraoperative electrical stimulation. Stimulation of individual motor fascicles is performed proximal and distal to the lesion, and fascicles which produce the same muscle contraction are identified. Motor and silent sensory fascicles are tagged prior to tumor resection. This facilitates subsequent specific motor/sensory reconstruction of the defect, using intrafascicular

[Diazepam (valium) in premedication. Experimental findings].

The anxiolytic and analgesic effects of 10 mg of orally administered diazepam (Valium) were assessed on 10 volunteers. The electrodermal activity was recorded during two experimental sessions while cutaneous electrical stimulations were administered to the subjects. On one hand, the anxiolytic activity of diazepam could be observed by a decrease of both autonomic nervous system and anticipatory activities. On the other hand, an increase of the pain threshold could be shown under diazepam by lower responses to the stimulations. These results confirm that diazepam can be useful in anxious patien

Influence of clonidine upon sympathetic component of the carotid chemoreceptor reflex.

The central influence of clonidine on the sympathetic component of carotid chemoreflex was verified in chloraloseurethane anaesthetised cats. The electrical stimulation of sinus nerve evoked only the excitatory responses in the sympathetic cardiac and renal nerve in the full range of stimulus strength. The excitatory response in the sympathetic renal nerve was completely depressed during the central cardiodepressor action of clonidine. Taking into account that the evoked excitatory responses in the sympathetic nerves were mainly the result of chemoreceptor carotid afferent fibre stimulation, i
