Near-Zero Fluoroscopy Ablation Workflow With a Circular Multielectrode Pulsed-Field Ablation Catheter.

BACKGROUND: Pulmonary vein isolation (PVI) using pulsed field ablation (PFA) is effective for treating atrial fibrillation (AF) but often requires fluoroscopy, posing risks of radiation exposure. Using a circular multi-electrode PulseSelect catheter along with 3D electroanatomical mapping (3D-EAM) and intracardiac echocardiography (ICE) enables pulmonary vein isolation (PVI) with near-zero fluoroscopy. METHODS: All procedures were managed under general anesthesia. Following transseptal puncture, pre-mapping was conducted using a multi-electrode catheter. Based on these initial mappings, the P

Delivery of Cavitation Therapy With a Modified Clinical Scanner: In Vitro Evaluation.

In this study, we design and implement pulses [1.67 MHz, 20-1000 cycles, 0.8-2.5 MPa, and 5-100 ms pulse repetition time (PRT)] suitable for microbubble cavitation treatments with a phased array of a clinical ultrasound scanner. A range of acoustic parameters was evaluated in a tissue-mimicking phantom with suspended Sonazoid microbubbles. Hydrophone measurements were used to optimize the transmit beamforming. A passive cavitation detection (PCD) system was designed to measure the microbubble scattered signals over a 1 s exposure. Postprocessing of the scattered signals evaluated frequency con

Real-time cardiomyocyte contraction sensing via a neo-flexible magnetic sensor.

Assessing heart disease and evaluating drug-induced cardiotoxicity require a deep understanding of the contractile properties of cardiac tissue, particularly at the level of individual cardiomyocytes. Traditional methods for measuring cardiomyocyte contractility present several challenges, including limitations in real-time detection, complex and costly sensing platforms, and biocompatibility issues. To address these challenges, we introduce an innovative magnetic sensor that utilizes a flexible coil cantilever, pioneering the application of electromagnetic induction for detecting cardiomyocyt

An active electronic, high-density epidural paddle array for chronic spinal cord neuromodulation.

Objective. Epidural electrical stimulation (EES) has shown promise as both a clinical therapy and research tool for studying nervous system function. However, available clinical EES paddles are limited to using a small number of contacts due to the burden of wires necessary to connect each contact to the therapeutic delivery device, limiting the treatment area or density of epidural electrode arrays. We aimed to eliminate this burden using advanced on-paddle electronics.Approach. We developed a smart EES paddle with a 60-electrode programmable array, addressable using an active electronic mult

VIEshunt: towards a ventricular intelligent and electromechanical shunt for hydrocephalus therapy.

BACKGROUND: Shunt systems for hydrocephalus therapy are commonly based on passive mechanical pressure valves that are driven by the intracranial, intra-abdominal, and hydrostatic pressure. The differential pressure acting on the valve determines the drainage rate of cerebrospinal fluid (CSF) but is not a gauge of the physiological condition of the patient. Internal and external influences can cause over- or underdrainage and lead to pathological levels of intracranial pressure (ICP). METHODS: The first prototype of a ventricular intelligent and electromechanical shunt (VIEshunt) is developed,

Development of a Small, Low-Power, Real-Time Phase-Dependent Neuromodulation System.

Neurological diseases and neuropsychiatric disorders are often characterized by abnormal neural oscillations, such as exaggerated synchronization or suppression within a narrow frequency band and complex oscillation coupling which disrupt normal brain function and contribute to debilitating symptoms. Phase-dependent stimulation (PDS) offers a promising solution by synchronizing electrical stimulation with specific phases of neural oscillations, thereby enhancing therapeutic precision and efficacy. However, the widespread clinical adoption of PDS is hindered by technological challenges, includi

Effects of shortening distal shunt catheters in children as relates to patient age and valve type: a simulation study with clinical implications.

OBJECTIVE: In children treated with a ventriculoperitoneal shunt (VPS), the growing body length increases the negative hydrostatic pressure within the shunt. At initial shunt insertion in neonates, infants, and small children, some surgeons traditionally shorten the distal catheter, although full length is generally well tolerated. Authors of this study investigate the effect of shortening distal shunt catheters on intracranial pressure (ICP) and cerebrospinal fluid (CSF) drainage rates in a bench test model set for different ages and with particular valve types. METHODS: Using a hardware-in-

Can Simple Changes in Splinting Technique Reduce Posterior Heel Contact Pressure?

BACKGROUND: Lower extremity splints are commonly used for the treatment of foot and ankle injuries and carry the risk of posterior heel pressure injury. Assessing heel contact pressures in a splint may guide clinicians toward specific splint designs that are associated with lower contact pressures. QUESTIONS/PURPOSES: This biomechanical study tested multiple splint characteristics to answer: (1) Which combination of heel position, padding thickness, and padding type (brand) minimizes posterior heel contact pressure? (2) Are there factors while making a splint that are associated with higher c

Nanohole-in-microsphere array (NiMA): An ultrasensitive digital-SERS tool for serum diagnosis of rheumatoid arthritis.

Rheumatoid arthritis (RA) is a serious chronic systemic autoimmune disease leading to joint destruction for progressive disability. Accurate non-invasive diagnosis of RA in the early stages is vital to timely precision treatment. The advancements of surface-enhanced Raman spectroscopy (SERS) in label-free liquid biopsy for noninvasive diagnosis have been witnessed in the past decade. The recently-developed digital SERS (dSERS) further overcomes the spectral intensity fluctuation, strengthening reliability and robustness for quantitative analysis. Here we propose a high-throughput dSERS substra

Feasibility of a Customised Handgrip for a Rehabilitation Robot to Meet Specific Needs.

Stroke is a common healthcare problem, leading to disability and significantly impacting individuals' quality of life. A customised device was suggested to be used for roboticassisted therapy to meet different stroke survivors' needs. However, most existing upper limb rehabilitation robots only offer a generic handgrip, which fails to meet the various needs of stroke survivors with different spastic patterns and levels of upper limb weakness. This study investigates the technical and clinical feasibility of customised handgrips to meet stroke survivors' needs through an online questionnaire wi

Safer fluoroscopy guided femoral access using a passive robotic support with a 3D printed needle and guidewire inserter.

Radiation during fluoroscopy guided puncture remains a daily concern for staff. We have designed 11 degree of freedom passive robotic mechanism (7 DOF passive robot and 4 DOF 3D printed needle and guide wire insertion) that keeps manual feel and line of sight while moving hands away from the beam. We tested its effectiveness on operator dose in a controlled swine model. Twelve femoral punctures were completed by one experienced operator. Six were done without the passive robot and six with it. Imaging used a mobile C arm.A diagnostic catheter was used to select both femoral arteries, and the s

Establishing thresholds for swing transparency at the knee during gait to inform exoskeleton design.

Knee exoskeletons have been developed to assist, stabilize, or improve human movement or recovery. However, exoskeleton designers must implement transparency (i.e., get out of the way) modes during the swing phase of locomotor tasks to avoid impeding movement. The problem is that it is not understood how sensitive people are to small knee torques or what level of knee impedance is acceptable (sufficiently transparent) during swing phase. Here, we (i) characterized the biomechanical consequences of knee stiffness and damping during swing, and (ii) leveraged user perceptions of being impeded and

Mechatronic Design of a 2-DoF Wearable Robot for Wrist Stiffness Assessment.

Parkinson's Disease (PD) is a neurodegenerative disorder characterized by a variety of motor symptoms including joint rigidity, typically assessed through semi-quantitative clinical scales, which are subjective and low-reliable. To address the limitations of current methods, we introduce in this paper the PDMeter2, a portable, wearable device for objective, quantitative assessment of wrist rigidity along two coupled Degrees of Freedom (DoFs): Flexion-Extension (FE) and Radio-Ulnar Deviation (RUD). The system includes two actuators to deliver controlled and repeatable perturbations to the wrist

Case study on force compliant robot arm controller for nasopharyngeal swab insertion.

The nasopharyngeal (NP) swab sample test, commonly used to detect COVID-19 and other respiratory illnesses, involves moving a swab through the nasal cavity to collect samples from the nasopharynx. While typically this is done by human healthcare workers, there is a significant societal interest to enable robots to do this test to reduce exposure to patients and to free up human resources. The task is challenging from the robotics perspective because of the dexterity and safety requirements. While other works have implemented specific hardware solutions, our research differentiates itself by us

Endoscopically Implanted, Self-Powered, Vagal Nerve Stimulation Device: Experimental Feasibility Study.

Background: The prevalence of metabolic disorders and obesity is increasing worldwide. The underlying pathogenetic mechanisms include an imbalance of the autonomic nervous system secondary to a relative decrease of the parasympathetic vagal tone or increase of the sympathetic tone. Previous clinical experience with a surgically implanted gastric pacemaker for morbid obesity showed that augmenting the vagal tone effects satiety and weight control. The aim of this study was to assess the feasibility of endoscopic implantation of a brain-neuromodulator (BNM) within the proximal gastric wall. Meth

Wearable Vibration Device for Supporting Postural Stability based on Proprioceptive Stimulation.

In this study, we present a wearable device for local vibration stimulation aimed to support postural stability in balance-compromised persons. Experiments were conducted with 15 young and 9 middle-aged adults without lower limb disabilities in a standing posture with vibration stimulation. The results showed a decrease in the mean velocity of the center-of-pressure sway in the standing posture during vibration presentation. In middle-aged and older adults, the rate of increase in the postural stability index was higher than that in the absence of vibration when the standing balance test was r
