[Slowly progressive aphemia--a case report].

A 69-year-old women was admitted to Tokyo Medical and Dental University Hospital because of slowly progressive difficulty in speech production over 8-years. On admission, her spontaneous speech was non-fluent, limited to one-syllable utterance, and severely efforty. But her visual and auditory comprehension was preserved. There was no significant general intellectual deterioration. Severe buccofacial apraxia, but no swallowing was observed. So we considered her difficulty in speech as aphemia. Three-dimensionally reconstructed surface MR image clearly showed severe atrophy in the posterior par

Gray matter heterotopia and acute necrotizing encephalopathy in trichothiodystrophy.

Trichothiodystrophy was diagnosed in a 3-year-old male presenting with speech delay, brittle hair, chronic neutropenia, and a history of febrile convulsions. Cranial magnetic resonance imaging revealed a focal subcortical and periventricular gray matter heterotopia. An acute encephalopathy with status epilepticus and coma occurred when he was 4 years of age during an upper respiratory tract infection. Magnetic resonance imaging revealed multifocal T2-weighted hypersignal lesions involving mainly the thalami, hippocampi, midbrain, and pons. Analysis of cerebrospinal fluid revealed hyperproteino

[Frontal type adrenoleukodystrophy: the progress of the white matter lesion--a neuroradiological and pathological study].

We studied the correlation between neuroradiological findings and pathological observations of white matter lesions in a patient with frontal type adrenoleukodystrophy. A 41-year-old man developed schizophrenic symptoms and generalized convulsions at the age of 40. Examination revealed baldness, loss of the axillary hair, stereotypical behavior, mutism, dysphagia echographia, right hemiparesis, and brisk reflexes in all four limbs with bilateral extensor plantar responses. Blood examination revealed a high concentration of very-long-chain fatty acids in plasma; the patient was diagnosed as hav

[Involvement of the basal ganglia in obsessive compulsive disorder: a review].

This article reviews the main clinical indicators linking the basal nuclei to obsessive compulsive disorder (OCD). Various pathologies associated with lesions of the basal nuclei are examined, followed by a review of neuropharmacological, brain imaging and psychosurgical studies. Once the role of the neostriatum in ritualistic behaviours has been clarified, the symptoms of OCD are interpreted based on the hypothesis of a lesion of the striato-orbito-frontal loop.

The origin of myoclonus and periodic synchronous discharges in subacute sclerosing panencephalitis.

We report here a case of a patient with subacute sclerosing panencephalitis (SSPE) and we have analyzed periodic events using dipole tracing methods to clarify the origin of periodic synchronous discharges and myoclonus. Both source generators were located in the subcortical part of the cerebrum, an area adjacent to the thalamus. Although the pathophysiology of periodic events in SSPE has been controversial, dipole tracing methods may contribute to clarify the origin of periodic events in SSPE.

Primary intracranial actinomycosis. Report of a case and review of the literature.

Actinomycosis is a bacterial infection that affects chiefly the cervico-oro-facial area, the abdomen and lungs. From these sites it may sometimes spread to the central nervous system. Extremely rare, and controversial, is the possibility of a primary infection of the central nervous system. We report an odd case of apparently primary brain abscess caused by Actinomyces israeli and review the literature.

Endogenous oxytocin inhibits morphine tolerance through limbic forebrain oxytocin receptors.

It has previously been shown that endogenous oxytocin (OXT) inhibits the development of acute morphine tolerance. The role of OXT receptors in the central nervous system (CNS) was therefore studied by using a specific OXT receptor antagonist, N-acetyl-(2-O-methyltyrosin)-OXT (ACME-OXT). ACME-OXT (1 pg) was injected into the posterior olfactory nucleus, central amygdaloid nucleus, ventral hippocampus, caudate nucleus or lateral cerebral ventricle. The antagonist facilitated the development of tolerance to morphine when injected into the posterior olfactory nucleus, central amygdaloid nucleus or

Grooming reflexes and Brown-Séquard epilepsy in cats with pontile lesions.

The literature on the relation between the scratch reflex and Brown-Séquard epilepsy in the guinea pig indicates that the scratch reflex is a stimulus-induced myoclonus that is the first component of a complete seizure. Cats with pontile lesions and cats with frontal neocortical lesions exhibit the scratch reflex and other grooming reflexes. The grooming reflexes in cats with pontile lesions develop over a period of a year or more into complete seizures that are similar to Brown-Séquard epilepsy as described for the guinea pig by Brown-Séquard and others. Cinematographic analyses revealed that

[Systems organization of cerebral processes in the cat during learning while undergoing electrical stimulation of the neocortex].

A delayed spatial choice (DSC) was elaborated in five cats during electrical stimulation (ES) of the frontal cortical parts with a frequency of 80 imp/s. Then the task fulfillment was tested following cessation of the electrostimulation or under the action of other current frequencies (35,3 and 120 imp/s). The number of erroneous choices increased in the absence of ES or during 3 imp/s ES of the frontal parts. The effects of frequencies of 35,80 and 120 imp/s did not differ. Three cats were trained to DSC without ES. The conditioned reflexes were tested during ES of the frontal cortical parts

[Many-year course of systemic lupus erythematosus with cerebral symptoms].

During 20 years of disease duration in the reported woman aged 47 years at the time of death articular involvement was followed by cardiac involvement and after labour renal signs appeared. Cerebral signs developed about 11 years before her death and manifested themselves as several typical strokes. The observed systemic affections, including the neurological signs, responded favourably to immunosuppression. Postmortem examination of the brain demonstrated numerous ischaemic foci, including a large cerebellar infarction, due to proliferative changes in the walls of cerebral and meningeal vesse

[Capillary telangiectasia of the pons].

Three observations of teleangioectases of the Varolian bridge verified at a histological examination are presented. In one of the cases the telangioectases were concurrent with an astrocytoma of the left hemisphere, and were a casual finding. In another case the teleangioectases have led to a massive hemorrhage to the Varolian bridge and the patient's death. In the third case the teleangioectases were revealed at the histological examination of the Varolian bridge affected by an astrocytoma. The development of the tumour was, possibly, due to reactive gliosis withing the zone of a vascular dev

Organization of afferent input to subdivisions of area 8 in the rhesus monkey.

The sources of ipsilateral afferents to subdivisions of one frontal eye field (Walker, '40a area 8) were studied with horseradish peroxidase (HRP) in macaque monkeys. There were major differences in the distribution of cells projecting to the caudal and rostral parts of area 8. The majority (53%) of labeled cortical cells projecting to caudal regions were in visual association areas, and an additional 23% were in the ventral bank of the intraparietal sulcus, where neurons may have predominantly visual and visuomotor properties. In contrast, rostral area 8 had a much lower percentage of its cor

Thalamocortical mechanisms in odor-guided behavior. I. Effects of lesions of the mediodorsal thalamic nucleus and frontal cortex on olfactory discrimination in the rat.

The "core" subnucleus of the mediodorsal thalamic nucleus (MD) receives direct input from olfactory cortex in the rat. This part of MD projects to the frontal neocortex of the rhinal sulcus (RS), while other parts of the MD project to the anterior medial wall of the neocortex (MW). In this study, rats were tested on odor threshold, detection of specific odors, and three odor discriminations both before and after lesions of MD, MW, RS, or sham lesions. Olfactory threshold and detection ability remained unaffected by any of these lesions; however, subjects with MD and RS lesions were markedly im

Acute ascending polyradiculoneuritis in a case of Creutzfeldt-Jakob disease.

A patient is described who showed subacute dementia and ataxia and, finally, acute asending polyradiculoneuritis. At necropsy, together with the cerebral and cerebellar lesions that are typical of Creutzfeldt-Jakob disease, the lesions of the spinal motor neurons, the spinal ganglia, and of the roots and peripheral nerves were analogous with those of the Guillain-Barré-Landry syndrome. The aetiological possibility is discussed either that this is a simple and unique association, or that the disorder is related to the virus nature of Creutzfeldt-Jakob disease.

Central nervous system aspergillosis in two young adults.

Unusual and unique features were present in the cases of two patients with central nervous system aspergillosis: One had a previously unreported complication of aspergillosis, extracranial internal carotid artery thrombosis documented by angiography, and in the other patient, cerebral granulomas developed from Aspergillus fumigatus in association with intravenous narcotic abuse. The latter etiologic mechanism was suggested once before. Neither of these patients showed other underlying disease. A detailed neuropathologic examination correlated the salient clinical findings with formation of asp
