Non-Newtonian blood rheology significantly alters hemodynamic predictions during cardiac looping: a computational study.

INTRODUCTION: Hemodynamic forces play a key role in early cardiac morphogenesis, yet many computational studies assume Newtonian blood behavior. Here, we evaluate the impact of non-Newtonian shear-thinning rheology on flow patterns, pressure distributions, and wall shear stress (WSS) during cardiac looping using idealized three-dimensional models of the embryonic heart tube. METHODS: Five geometries representing progressive looping stages, from a linear tube to an S-shaped configuration with ventricular ballooning, were analyzed under pulsatile flow using both Newtonian and power-law viscosit

Evaluation of Hemodynamic Data in an Ovine Model to Estimate Optimal Time for Compressor Change during Neonatal Resuscitation.

INTRODUCTION: Survival from cardiac arrest is directly and consistently linked to the quality of chest compressions (CC). Although compressor fatigue is well established, neonatal resuscitation guidelines (NRP) do not offer specific recommendations on rotation intervals beyond suggesting provider changes every 2-5 min. By comparison, Pediatric Advanced Life Support recommends a strict 2-min rotation to prevent fatigue and maintain high-quality CC. METHODS: Prospective observational data from resuscitation of asphyxiated cardiac-arrested neonatal lambs to evaluate changes in chest compression

Immunosenescence-Driven Hemodynamic Dysregulation and Cognitive Impairment in Myalgic Encephalomyelitis/Chronic Fatigue Syndrome: An Integrative Perspective.

Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a complex disorder marked by persistent fatigue and cognitive impairments, often termed "brain fog." Emerging evidence suggests that immunosenescence, age- or stress-related deterioration of immune function, plays a pivotal role in the pathogenesis of cognitive dysfunction in ME/CFS. Immunosenescence induces chronic low-grade inflammation (inflammaging); alters T-, NK-, and B-cell function; and promotes the release of senescence-associated secretory phenotype (SASP) factors. These changes are proposed to cerebral blood flow (CBF) r

When pregnancy tests the heart: voluntary wheel running attenuates HFHS-associated postpartum hemodynamic dysfunction.

Pregnancy and lactation increase cardiovascular demand and may unmask postpartum cardiac vulnerability under metabolic stress. We tested whether exposure to a high-fat/high-sucrose (HFHS) diet from premating through lactation induces postpartum hemodynamic dysfunction with preserved ejection fraction (EF) and whether voluntary wheel running mitigates these alterations. Virgin C57BL/6J female mice were assigned to control sedentary (CS), HFHS sedentary (HS), or HFHS with voluntary wheel running across the reproductive window. Left ventricular (LV) pressure-volume (P-V) analysis was performed 6-

Functional Hemodynamic Assessment in Intracranial Atherosclerotic Disease: From Anatomical Stenosis to μQFR-Guided Precision Medicine.

Intracranial atherosclerotic stenosis (ICAS) is the leading cause of ischemic stroke worldwide, especially in Asians. Current risk stratification depends mainly on the degree of anatomical narrowness. Growing evidence indicates a significant separation between lumen stenosis and lesion-specific hemodynamic impairment. This phenomenon highlights the limitations of anatomy-based assessment alone. In this context, researchers began to adopt a physiologically oriented assessment strategy. This strategy is seen as a promising approach to improve individualized ICAS diagnosis and guide treatment dec

Half of what? Number bisection, line bisection, number line bisection and estimation differ in their hemodynamic signatures.

Spatial and numerical processing are tightly connected capacities of human cognition. It is assumed that space and numbers draw on common neural networks. Here, we used functional near-infrared spectroscopy (fNIRS) during spatial-numerical bisection tasks to assess neural activity in a natural production paradigm. Thirty-six neurotypical participants used a touch pen during line bisection, number line bisection, number line estimation, number bisection, and an active neutral task (figure-copying). Accordingly, we systematically varied spatial and numerical requirements in a series of four bise

The Use of a Hemodynamic Monitor Perceptual and Adaptive Learning Module to Improve Clinical Interpretation Skills in Anesthesiology Residents.

BACKGROUND: Perceptual and adaptive learning modules (PALMs) are educational tools designed to enhance pattern recognition and long-term memory. We evaluated the impact of a hemodynamic monitor PALM on first-year anesthesiology residents' performance in diagnosing clinical abnormalities via waveform and vital sign interpretation on simulated monitors. METHODS: First-year residents were randomly assigned to a group (n = 20) that used the hemodynamic monitor PALM or a control group (n = 21) that did not. Both groups received a pretest that measured their accuracy and fluency (percentage of resp

Physiology-guided bilateral pulmonary artery banding in high-risk neonates: early hemodynamic and perfusion outcomes.

BACKGROUND: Balancing pulmonary and systemic blood flow remains one of the most critical challenges in the management of high-risk neonates with ductal-dependent circulation, particularly in the presence of low birth weight, sepsis, shock, restrictive interatrial communication, or complex congenital cardiac anatomy. Bilateral pulmonary artery banding (biPAB) is frequently employed as an initial stabilizing strategy to control pulmonary overcirculation; however, optimal band tightness remains largely experience-based, and uniform band sizing may be associated with variable physiological respons

Peripheral Artery Disease (P.A.D.): Vascular Hemodynamic Simulation Using a Printed Circuit Board (PCB) Design.

BACKGROUND: Arterial stenosis produces nonlinear changes in vascular impedance that are challenging to investigate in real time using either benchtop flow phantoms or high-fidelity computational fluid dynamics (CFD) models. OBJECTIVE: This study aimed to develop and evaluate a low-cost printed circuit board (PCB) analog capable of reproducing the hemodynamic effects of progressive arterial stenosis through an R-L-C mapping of vascular mechanics. METHODS: A lumped-parameter (0D) electrical network was constructed in which voltage represented pressure, current represented flow, resistance mode

Sex-Specific Cutoffs for High-Sensitivity Troponin I in the Risk Stratification of Hemodynamically Stable Pulmonary Embolism.

High-sensitivity cardiac troponin I (hs-cTnI) is frequently elevated in patients with pulmonary embolism (PE), although its prognostic value in hemodynamically stable cases and the optimal cutoff points for risk stratification remain unclear. This prospective observational study included consecutive patients with symptomatic, hemodynamically stable PE from 2018 to 2023 at a tertiary hospital. Hs-cTnI levels were measured at diagnosis, and sex-specific cutoff points were determined for a 30-day composite outcome, defined as PE-related mortality, non-PE-related mortality, hemodynamic instability,

Impaired atrioventricular time interaction contributes to tachycardia-induced hemodynamic deterioration: In vivo and in silico hybrid approach.

BACKGROUND: Heart rate (HR) is a major determinant of cardiac output (CO). During tachycardia, adequate left ventricular (LV) filling becomes important; however, excessive tachycardia reduces CO by shortening filling time. The impact of HR on hemodynamics differs according to the underlying cardiac pathological conditions, making HR management challenging. The atrioventricular (AV) time interaction, which represents the temporal coordination between atrial and ventricular functions for optimal LV filling, is key to effective HR management during tachycardia. This study aimed to elucidate tachy…

Prognostic Value of Hemodynamic Indices Prior to tMCS Explantation: An Analysis from the Cardiogenic Shock Working Group.

BACKGROUND: Standardized hemodynamic-guided weaning criteria are needed for patients with cardiogenic shock receiving temporary mechanical circulatory support (tMCS). We aimed to determine whether hemodynamic indices prior to tMCS explant predict mortality and native heart survival and whether performance varies across device platforms. METHODS: We analyzed 385 cardiogenic shock patients from the multicenter Cardiogenic Shock Working Group registry who received tMCS. Four indices were evaluated: blood pressure response index (BPRI), organ perfusion pressure (OPP), aortic pulsatility index (AP

Hypohydration Increases Heart Rate Despite Preserved Hemodynamic Compensation During Blood Flow Restricted Exercise.

During blood flow restriction (BFR) exercise, blood becomes sequestered in the active limbs due to impeded venous return, contributing to central hypovolemia. Dehydration also reduces blood volume and can exacerbate the ability of the cardiovascular (CV) system to defend against CV stressors, such as exercise and orthostatic stress. As BFR becomes more widely used, its application in individuals who may be suboptimally hydrated underscores the need to understand how hydration status influences cardiovascular strain. We tested whether hypohydration affects the CV response to BFR exercise and it

Comparison of the Hemodynamic Effects of Epinephrine on Blood Pressure Augmentation at 1-, 3-, and 5-Minute Dosing Intervals.

BACKGROUND: Although current cardiopulmonary resuscitation guidelines recommend administering epinephrine at 3- to 5-minute intervals during advanced life support (ALS), scientific evidence for the optimal dosing interval for enhancing hemodynamic parameters remains limited. Therefore, we compared the hemodynamic effects of 1-, 3-, and 5-minute epinephrine dosing intervals on blood pressure augmentation in a porcine ventricular fibrillation cardiac arrest model. METHODS: Forty-two pigs were randomly assigned to 1-, 3-, and 5-minute epinephrine dosing-interval groups. After ventricular fibrill

Incidence of Adverse Cardiac Events After Liver Transplantation in Patients With Different Hemodynamic Classifications of Pulmonary Hypertension.

INTRODUCTION: Pulmonary hypertension (PH) in liver transplantation candidates is associated with elevated morbidity and mortality. Few studies have investigated whether different categories of PH based off hemodynamic characteristics portend worse outcomes. Furthermore, there have been newly defined parameters for PH classifications. Therefore, we aimed to compare postoperative outcomes after liver transplantation in patients within different newly established hemodynamic categories of PH. METHODS: This was a 20-year single-center retrospective observational study of adult patients undergoing

Multidimensional feature-based assessment of microcirculatory hemodynamic dysfunction in patients with Wagner Grade 0 diabetic foot.

OBJECTIVE: This study characterizes the hemodynamic abnormalities of the microcirculation in the feet of patients with diabetic foot (DF) based on multidimensional features, including time-domain and frequency-domain metrics, rhythmicity, and symmetry. It further elucidates the relationship between these abnormalities and plantar pressure hotspots, specifically the forefoot first metatarsal head (MT1) and fifth metatarsal head (MT5), and constructs a cross-sectional discriminative model. METHODS: A total of 286 consecutive participants were included in the study (non-DF: 157, DF: 129). Microc

Comparison of the Effects of Dexmedetomidine Versus Lidocaine in Blunting the Hemodynamic Response to Tracheal Extubation in Patients Undergoing Craniotomy.

Background Tracheal extubation following neurosurgical procedures is commonly associated with sympathetic stimulation, leading to tachycardia and hypertension. Such hemodynamic fluctuations may increase intracranial pressure (ICP) and adversely affect cerebral perfusion in patients undergoing craniotomy. Various pharmacological agents have been used to attenuate these responses; however, the comparative effectiveness of dexmedetomidine and lidocaine during tracheal extubation remains an area of clinical interest. Objectives The objective of this study is to compare the effectiveness of dexmede

CD71+ erythroid cells as a potential early biomarker for hemodynamic significant patent ductus arteriosus in preterm infants.

INTRODUCTION: Hemodynamically significant patent ductus arteriosus (hsPDA) in preterm infants is associated with increased mortality and morbidity. Early and accurate identification of infants at risk for PDA is critical for improving outcome. Perinatal inflammation is a known risk factor of PDA. CD71+ erythroid cells (CECs) are immature red blood cells with immunomodulatory roles and show variable abundance in preterm infants. We aimed to examine the proportion of CECs in preterm infants and investigate association with PDA. METHODS: In this prospective study, we evaluated the association be
