Shared pathophysiological mechanisms between diabetes and calcific aortic valve disease: an immunometabolic perspective.

This review synthesizes the immunometabolic mechanisms linking diabetes mellitus to accelerated calcific aortic valve disease (CAVD) and evaluates their therapeutic implications. Despite the absence of disease-modifying pharmacotherapy for CAVD, diabetes consistently increases incident aortic stenosis risk (HR 1.3-1.7), calcification burden, and disease severity, independent of traditional cardiovascular risk factors. Epidemiological, imaging, and histological evidence demonstrate that dysglycemia promotes earlier onset, denser valvular calcium deposits, and worse post-intervention outcomes. W

Recent Advances of the Role of Dl-3-n-Butylphthalide in the Treatment of Alzheimer's Disease.

Alzheimer's disease (AD) is a prevalent neurodegenerative disorder affecting approximately 55.2 million individuals globally, with complex pathogenesis involving amyloid-β (Aβ) aggregation, tau pathology, neuroinflammation, oxidative stress, and synaptic dysfunction. Current treatments offer only symptomatic relief without halting disease progression. Dl-3-n-butylphthalide (NBP), a small-molecule compound originally derived from celery seeds, has emerged as a promising multi-target therapeutic candidate for AD. Preclinical studies demonstrate that NBP exerts its therapeutic effects in AD by al

The gut mycobiome and metabolic dysfunction in type 2 diabetes mellitus.

Type 2 diabetes mellitus (T2DM) affects approximately 589 million adults globally, yet mechanisms linking intestinal microbial dysbiosis to metabolic dysfunction remain incompletely understood. Although bacterial alterations have been extensively characterised, the gut mycobiome has received comparatively limited attention despite growing evidence that fungi participate in immune, epithelial, and metabolic homeostasis. This review synthesises evidence linking gut fungal dysbiosis with T2DM from human and experimental studies. Across cohorts, T2DM has been associated with Candida enrichment and

Microglial advances in Parkinson's disease.

Microglia perform the function of CNS macrophages and act as the primary immune cells in the brain. They surveil the environment, phagocytose cellular debris, maintain homeostasis and respond to tissue damage. While under physiological conditions they play a role in supporting neurons, activated microglia participate directly in the degeneration of neurons in the substantia nigra, as the hallmark of the Parkinsons disease (PD). Their detrimental effects result from direct phagocytosis of dopaminergic neurons as well as via neuroinflammation and release of toxic reactive oxygen and nitrogen spe

Preemptive cardioprotection with a small molecule in rodents that suppresses genes predictive of heart failure.

Ischemic heart disease is a leading cause of death worldwide. While percutaneous coronary intervention (PCI) restores blood flow in acute coronary syndrome (ACS), reperfusion injury exacerbates myocardial damage, contributing to heart failure (HF). Preemptive administration of a cardioprotective agent could help counter the imminent proinflammatory insult of PCI and reperfusion. Administering BT2, a small-molecule MAPK kinase/extracellular signal-regulated kinase inhibitor, 24 hours before and during ischemia in rats before reperfusion reduced infarct size by ~70% and preserved cardiac functio

Bridging the gap: neuroinflammation and the dawn of precision medicine in amyotrophic lateral sclerosis.

Neuroinflammation is no longer a secondary feature of amyotrophic lateral sclerosis (ALS), but rather a disease-modifying process that actively shapes the motor neuron vulnerability from the earliest stages of pathology. Central and peripheral immune cells, including microglia, astrocytes, and infiltrating T lymphocytes, adopt context-dependent states that can be neuroprotective or neurotoxic depending on disease stage and genetic background. These states are driven by discrete molecular programs, such as cGAS-STING-mediated innate immune sensing, NLRP3 inflammasome activation, and RIPK1-depen

Sirtuin 1 deficiency mediates chronic kidney disease-induced inflammaging cardiovascular calcification.

Chronic kidney disease (CKD) markedly accelerates calcific aortic valve disease (CAVD), yet the underlying mechanisms and therapeutic targets remain poorly defined. Here, we integrate population-scale analyses, single-cell transcriptomics, genetic inference and functional experiments to identify a central role for Sirtuin 1 (SIRT1) in CKD-associated aortic valve calcification. Analysis of UK Biobank data linked CKD to accelerated ageing and increased aortic stenosis risk. Single-cell RNA sequencing of human aortic valves identified SIRT1 downregulation and NLRP3 pathway activation specifically

Renoprotective Effects of a Herbal Formulation Against Lipopolysaccharide-Induced Acute Kidney Injury Through Anti-Inflammatory and Antioxidant Activities.

Sepsis-associated acute kidney injury (AKI) is a severe inflammatory complication with limited therapeutic options. This study investigated the protective effects of Taiwan Chingguan Yihau (NRICM101), a herbal formulation, against lipopolysaccharide (LPS)-induced AKI. Male C57BL/6 mice were injected with LPS to induce AKI in vivo, followed by therapeutic administration of NRICM101. NRK-52E renal epithelial cells were stimulated with LPS to induce inflammatory responses and oxidative stress in vitro. Key bioactive compounds were identified via HPLC and analyzed using molecular docking. NRICM101

[Research progress on the regulatory mechanisms of inflammatory signaling pathways in acupuncture treatment for ischemic stroke].

The occurrence of ischemic stroke is closely related to multiple inflammatory pathways, and acupuncture can improve neurological function in ischemic stroke by regulating inflammatory signaling pathways. At the level of pattern recognition receptors, acupuncture can inhibit high mobility group box 1 (HMGB1)/Toll-like receptor 4 (TLR4), NOD-like receptor thermal protein domain associated protein 3 (NLRP3) inflammasome, and cyclic GMP-AMP synthase(cGAS)/stimulator of interferon genes (STING) pathways, thereby inhibiting excessive activation of innate immunity. At the level of transcription facto

Interplay of oxidative stress and inflammatory mechanisms in doxorubicin-induced cardiomyopathy.

Doxorubicin (DOX) is a potent chemotherapeutic drug used to treat a variety of cancer types, but its usage is limited due to major cardiovascular side effects. According to World Health Organization reports, by 2050, new cancer cases are projected to increase to >35 million, implying an enhanced risk of DOX-induced cardiomyopathy (DIC). Despite its cardiotoxic side effects, DOX has remained an effective therapeutic choice for cancer treatment. Current evidence suggests that multiple pathophysiological mechanisms are involved in DIC, including oxidative stress, mitochondrial dysfunction, endopl

Unveiling a novel pathway: Ginseng vanillic acid fights heart failure by coupling mitophagy to pyroptosis through the PINK1-BRCA1-NLRP3 axis.

BACKGROUND: Heart failure (HF) represents a frequent and serious complication of hypertensive diseases. Substantial evidence has implicated angiotensin II (Ang II) in the promotion of cardiac remodeling and inflammation associated with hypertension. Vanillic acid (VA), an organic acid derived from natural sources such as ginseng, demonstrates a spectrum of bioactivities including anti-inflammatory and antioxidant effects. PURPOSE: This study aimed to elucidate the potential of VA in attenuating Ang II-induced HF and to delineate its underlying mechanism of action. METHODS: In C57BL/6 mice, H

Targeting VDAC1-dependent mtDNA release attenuates fibroblast innate immune activation and vitiligo pathogenesis.

Vitiligo is a chronic depigmentary disorder initiated by oxidative stress, which activates inflammatory signaling and innate immunity, ultimately leading to melanocyte destruction. Although melanocyte defects have been widely studied, dermal fibroblasts-the predominant stromal regulators of cutaneous immunity-remain insufficiently characterized in vitiligo pathogenesis. Here, we demonstrate that subtoxic oxidative stress in normal human dermal fibroblasts (NHDFs) induces a VDAC1-dependent, non-apoptotic release of mitochondrial DNA (mtDNA), thereby linking redox imbalance to immune activation.

Deletion of phosphatidylethanolamine methyltransferase promotes the spontaneous development of hepatic steatosis, inflammation, and fibrosis in young mice.

Phosphatidylethanolamine N-methyltransferase (PEMT) catalyzes the transfer of methyl groups to phosphatidylethanolamine to generate phosphatidylcholine (PC). PC produced de novo through this pathway is preferentially used for very-low-density lipoprotein assembly and is essential for its normal hepatic secretion as well as for bile acid detoxification. While human PEMT loss-of-function polymorphisms are linked to increased metabolic dysfunction-associated steatotic liver disease risk, the enzyme's role as a primary driver of progressive liver disease remains underexplored. We utilized PEMT kno

Targeting USP47 reprograms macrophage polarization to ameliorate septic peritonitis and promote cutaneous wound healing.

OBJECTIVE: Dysregulation of M1/M2 macrophage polarization drives sepsis and impaired wound healing. Deubiquitinases (DUBs) are promisingdrug targets, but the role of ubiquitin-specific protease 47 (USP47) in macrophage plasticity remains unclear. This study investigated the regulatoryfunction of USP47 in macrophage polarization and its therapeutic potential for inflammatory disorders. METHODS: In vitro, human THP-1 and mouse bonemarrow-derived macrophages were polarized, with USP47 modulated via genetic manipulation or the inhibitor P22077. In vivo, LPS-induced septicperitonitis and cutaneous

Natural Products Targeting Airway Inflammation and Mucus Hypersecretion: Molecular Mechanisms and Therapeutic Potential for Respiratory Health.

Chronic respiratory diseases, including asthma, chronic obstructive pulmonary disease (COPD), bronchiectasis, cystic fibrosis, and chronic bronchitis, are characterized by persistent airway inflammation and mucus hypersecretion, leading to airway remodeling and progressive pulmonary dysfunction. Although current therapies improve disease control, they often fail to adequately target the complex molecular mechanisms underlying chronic airway diseases and may cause adverse effects during long-term use. Natural products have therefore emerged as promising multitarget therapeutic agents because th

Nanomedicine targeting neuroinflammatory pathways in Alzheimer's disease: a new frontier in inflammopharmacology.

Alzheimer's disease (AD) is a multifactorial neurodegenerative illness characterized by progressive cognitive impairment, synaptic compromise, and relentless neuroinflammation. Increasing evidence suggests that neuroinflammatory cascades orchestrated by microglial activation, astrocytic malfunction, cytokine hyperproduction, and inflammasome signalling are at the core of AD pathogenesis. Conventional anti-amyloid and cholinergic treatments are only symptomatic and neglect the inherent neuroimmune dysregulation. Nanomedicine is a revolutionary frontier in inflammopharmacology, which enables the

Chronic Aseptic Myometritis: A Mechanistic Framework Linking Sterile Myometrial Inflammation to Uterine Fibroid Initiation and a Roadmap for Primary Prevention.

Uterine fibroids, the most common tumors in reproductive-age women, remain without a defined precursor tissue state. Unlike cervical dysplasia preceding cervical cancer, or colonic polyps preceding colorectal malignancy, no equivalent "at-risk" tissue marker exists for fibroids, and diagnosis relies on radiological imaging only after tumors are already well-established and often symptomatic including excessive menstrual bleeding, pelvic pain, infertility and obstetric complications. In this narrative review, we propose that a subset of women with unexplained AUB may harbor a chronic, non-infec

Serum 14-3-3 eta levels in relation to attack status and amyloidosis in patients with Familial Mediterranean fever.

INTRODUCTION: Familial Mediterranean fever (FMF) is a hereditary autoinflammatory disorder characterized by recurrent febrile episodes and complications, including secondary amyloidosis. Protein 14-3-3η, part of a family of intracellular proteins, regulates pyrin activity, inhibiting inflammatory processes. Mutant pyrin disrupts this interaction, activating the inflammasome. Therefore, this study aimed to evaluate serum 14-3-3η levels in relation to attack status and amyloidosis in FMF patients. METHODS: A cross-sectional study was conducted with 104 FMF patients diagnosed via Tel Hashomer an

SIRT5 desuccinylates PRDX3 to promote its chaperone-mediated autophagy and mitigate mitochondrial dysfunction in MSU crystal-induced inflammation.

BACKGROUND: Gout is triggered by monosodium urate (MSU) crystals and is closely linked to mitochondrial dysfunction and oxidative stress. However, the role of the desuccinylase SIRT5 in this process remains unclear. Here, we identify SIRT5 as a key negative regulator of MSU crystal-induced inflammation. METHODS: Macrophages were challenged with MSU crystals to assess mitochondrial membrane potential (ΔΨm), reactive oxygen species (ROS), and IL-1β secretion. The interaction between SIRT5 and PRDX3, as well as PRDX3 succinylation, was examined by co-immunoprecipitation (co-IP) and anti-succinyl

The subgingival microbiome in periodontitis: from ecological dysbiosis and metabolic reprogramming to precision interventions.

Periodontitis is a dysbiosis-driven chronic inflammatory disease characterized by complex interactions among the subgingival microbiome, microbial metabolism, and host immune responses. Accumulating evidence indicates that disease progression is not solely determined by the enrichment of specific periodontal pathogens but is critically associated with ecological disruption and functional reprogramming of the subgingival microbial community. During the transition from periodontal health to disease, microbial metabolism shifts from carbohydrate utilization toward proteolysis and amino acid ferme
