Retinal microvascular changes on retinal photographs for predicting stroke recurrence: a systematic review and meta-analysis.

BACKGROUND/OBJECTIVES: Changes in retinal microvasculature have been associated with the stroke severity and incident stroke risk. However, its role in predicting stroke recurrence, which results in higher mortality and poorer functional recovery, remains uncertain. This study aims to determine the association of retinal microvascular changes with recurrent ischemic stroke. SUBJECTS/METHODS: We conducted a systematic search of the PubMed and Embase databases up to June 2026. Prospective studies that investigated the use of retinal biomarkers as predictors of recurrent stroke risk were include

Pre-eclampsia.

Pre-eclampsia is a multisystem disorder that arises in at least 2-4% of pregnancies and is characterised by hypertension and new-onset organ dysfunction. The pathophysiology of pre-eclampsia is diverse but mostly involves a mismatch between fetoplacental demands and uteroplacental supply, combined with a maternal predisposition to cardiometabolic disease. While our understanding of the mechanisms underlying pre-eclampsia has advanced in recent years, maternal and perinatal mortality remains high in low-income and middle-income countries (LMICs), where pre-eclampsia incidence is higher. Risk fa

The Anatomical Connections of Nucleus Accumbens with Brainstem and Cerebellum in the Rat and Human Subjects.

The nucleus accumbens (NAc) plays a key role in reward-related behaviors and is involved in several neurological and psychiatric disorders. While the cortical and subcortical connections of the NAc are well-documented, its anatomical connectivity with brainstem and cerebellar structures is comparatively poorly understood. This study aims to investigate the anatomical connections of the NAc with the underexplored brainstem and cerebellar connections, which may have implications for understanding the neural mechanisms underlying many neurological conditions. The brainstem and cerebellar connecti

Elecoglipron, an oral small molecule GLP-1 receptor agonist in adults with obesity or overweight (VISTA): a multicentre, phase 2, randomised, placebo-controlled clinical trial.

BACKGROUND: Elecoglipron (AZD5004) is an oral small-molecule glucagon-like peptide-1 (GLP-1) receptor agonist administered once daily without food or fluid restriction, in development for weight management in people living with obesity or overweight and type 2 diabetes. We assessed the efficacy, safety, and tolerability of elecoglipron versus placebo in participants with obesity or overweight and at least one weight-related condition without diabetes. METHODS: In this double-blind, randomised, controlled, phase 2 dose-ranging study with a total treatment duration of 36 weeks, adult participan

Trends and Disparities in Mortality from Hereditary Ataxia in United States, 2000-2020: A Retrospective Analysis with Projections to 2050.

Hereditary ataxias are rare cerebellar disorders. Population level mortality patterns in the United States in the previous decades remain insufficiently described. We quantified national mortality trends associated with hereditary ataxias and described stratified trends by sex, race, census region, and urbanization. We analyzed U.S. Multiple Cause of Death data from CDC WONDER for 2000 to 2020. Decedents were identified using ICD-10 code G11 for hereditary ataxias. Age-adjusted mortality rates (AAMRs) per 100,000 were computed using the 2000 U.S. standard population, with 95% confidence interv

Atrial fibrillation.

Atrial fibrillation affects approximately 37·6 million people worldwide, with the prevalence predicted to double over the next 35 years. The ubiquitous use of wearable devices and other technologies with inbuilt diagnostic algorithms allows greater detection of atrial fibrillation among the general public than previously. Atrial fibrillation increases the risk of stroke and thromboembolism, heart failure, and death, and is associated with reductions in quality of life. Patients with atrial fibrillation frequently have comorbidities, and the accumulation of risk factors, including lifestyle fac

Secondary extraocular extension of choroidal melanoma following primary photodynamic therapy.

BACKGROUND/OBJECTIVES: Secondary extraocular extension (SEOE) following conservative treatment of choroidal melanoma (CM) is extremely rare, and its occurrence in patients treated with primary photodynamic therapy (PDT) has not been documented in the literature so far. We present three cases from our combined experience in the United Kingdom (UK). METHOD: Retrospective, observational case series. All cases of patients treated with PDT, based on the TAP protocol with some deviations, for CM who experienced SEOE were included, from two of the four national ocular oncology centres in the UK. Low-Titer Anti-GAD Cerebellar Ataxia: Evidence That Diagnosis Should Extend Beyond Serum Antibody Levels - A Case Report and Systematic Review.

BACKGROUND: Anti-glutamic acid decarboxylase (anti-GAD) antibodies are associated with cerebellar ataxia. Although high antibody titers are considered a key diagnostic feature, the significance of low titers remains uncertain. We describe a patient with low-titer anti-GAD cerebellar ataxia and systematically review the literature. METHODS: We report the case of a 38-year-old woman with autoimmune cerebellar ataxia associated with low-titer anti-GAD antibodies. A systematic review of PubMed/MEDLINE and SciELO identified published cases of cerebellar ataxia associated with low anti-GAD antibody

Deferral of percutaneous coronary intervention in patients undergoing transcatheter aortic valve implantation (PRO-TAVI): an investigator-initiated, multicentre, open-label, non-inferiority, randomised controlled trial.

BACKGROUND: Coronary artery disease is common in patients undergoing transcatheter aortic valve implantation (TAVI). We aimed to assess whether deferral of percutaneous coronary intervention (PCI) is non-inferior to routine PCI before TAVI in patients with coronary artery disease. METHODS: In this investigator-initiated, open-label, randomised controlled trial, done at 12 hospitals in the Netherlands, TAVI patients with coronary artery disease were randomly assigned in a 1:1 ratio to deferral of PCI or PCI before TAVI. Randomisation was done by use of a web-based system with random block size

CRISPR-Cas9‑based lipid nanocarriers for advanced therapy of urinary bladder cancer.

Bladder cancer (BCa) exhibits significant genetic and phenotypic variability. This variability suggests that various tumor subtypes could be influenced by several biomarkers and signaling pathways, which presents a problem for monotherapy strategies. Despite the initial effectiveness of traditional therapies, BCa's high rates of progression and recurrence, and the eventual development of drug resistance in many patients, continue to be major concerns. Because of the potential to transform the genetic modifications linked to the disease, genome editing using CRISPR/Cas9 has become a transformat

Comprehensive Characterization of Clinical Phenotypes and CTG Repeat-Associated Manifestations in Spinocerebellar Ataxia Type 8.

Spinocerebellar ataxia type 8 (SCA8) is traditionally characterized as a cerebellar syndrome, yet emerging evidence suggests significant phenotypic heterogeneity. The complex non-linear relationship between genetic burden and clinical severity remains understudied. We analyzed 172 SCA8 patients using advanced data mining techniques. Complete clinical demographic data were available for the cohort, with precise genetic repeat numbers available for 171 individuals. K-means clustering identified patient subtypes, while Gaussian Graphical Models (GGM) visualized symptom topology. We developed expl

Predicting progression to proliferative diabetic retinopathy using automated versus manual quantification of retinal haemorrhages.

PURPOSE: To compare automated and manual quantification of retinal haemorrhages in eyes with diabetic retinopathy (DR) and to analyse the risk of progression to proliferative DR (PDR). METHODS: Retinal haemorrhages on ultra-widefield (UWF) pseudocolor images in eyes with non-proliferative diabetic retinopathy (NPDR) were manually segmented. DR severity was assessed within the seven ETDRS fields at baseline and 1-year follow-up. Lesions were also automatically segmented using EyeRead UWF software (Eyenuk) and the frequency and area of retinal haemorrhages and the average distance of haemorrhag

OphthoEvidence: a model for rapid and trustworthy ophthalmology guidelines.

INTRODUCTION: The World Health Organization estimates that 2.2 billion people worldwide suffer from vision impairment, representing an annual productivity loss of US$411 billion. Meanwhile, ophthalmology advances at a fast-moving pace. As in every discipline, optimal ophthalmologic care depends on trustworthy clinical practice guidelines that promote patient-centred care and support consistent, evidence-informed decision-making. Existing guidelines, however, may not always be up to date and their adherence to internationally accepted standards for trustworthy development is variable. METHODS:
