Rhinosporidiosis is a chronic granulomatous condition caused by Rhinosporidium seeberi primarily affecting the mucosa of the nose, eyes and sinuses. The patients usually present with nasal symptoms and skin involvement is uncommon [1-3]. Here, a patient with nasal rhinosporidiosis having two different morphological types of cutaneous lesions of rhinosporidiosis, coexisting at the same time and at the same site, is reported. Sinonasal polyposis sometimes can masquerade malignant pathology of the paranasal sinuses and nose. We report the case of a 60 years old male diagnosed initially as solitary polyp and operated by endoscopic surgery with intrasurgical biopsy because a suspect of malignancy. The result was epidermoid carcinoma from left maxillary sinus that was confirmed after the definitive anatomopathologic exam. After Caldwell-Luc and complete removal of the tumoration, the patient evolutioned satisfactory being free of illness by clinical and radiological explorations after 1 year follow-up.

Three cases of Kearns-Sayre Syndrome are reported, in which some facial anomalies, including facial asymmetry, high forehead, wide nasal bridge, upturned nose, flat philtrum, low set ears and short neck were present. This syndrome comprises of bilateral femoral hypoplasia and unusual facies comprises of short nose with broad tip, elongated philtrum, thin upper lip, small lower jaw, cleft palate and antimongolian slant of palpebral fissure. Etiology is idiopathic, sporadic, few families have been reported to have autosomal dominant inheritance. We report a case with the additional finding of unilateral complete aplasia of femur, encephalocele, complete cleft lip, bilateral absence of 5th metatarsal and toe and hypospadias. The mode of inheritance appears to be sporadic mutation in this case.

We present a case of peri-orbital surgical emphysema following functional endoscopic sinus surgery (FESS), due to extubation with simultaneous manual inhalation with a closed expiratory valve. This complication has not been previously reported in association with general anaesthetic and FESS. Patients undergoing FESS are advised against nose blowing. However, we recommend that the anaesthetist working with a surgeon who performs FESS should also be warned not to use manual inhalation with a closed expiratory valve while extubating, as this can cause peri-orbital surgical emphysema.

Inverted papilloma (PI) is a benign neoplasm of nose and paranasal sinuses. IP's pathophysiology is characterized by its capacity to be locally destructive, its ability to grow rapidly and its tendency to recur. IP's association with carcinoma is about 13 percent of cases. The surgical treatment of IP has evolved over the years, with a progression toward more radical en bloc extirpation. The paper reports an inverted papilloma case of nasal cavities, with destruction of frontal sinus roof and intracranial extension aside frontal lobe displacement.

An unusual cutaneous angiosarcoma resembling rosacea clinically is described. A 66-year-old man presented with a red discoloration and a diffuse swelling on his nose over a 2-year period that was diagnosed as rosacea. Despite antiinflammatory treatment, the infiltration increased and, after repeated skin biopsies, the diagnosis of a well-differentiated cutaneous angiosarcoma associated with a prominent inflammatory infiltrate was considered. We report a case of endometrioma in the pelvic cavity. A 41-year-old woman presented with a peculiar periodic sciatica associated with her menses. Computed tomography showed an oval-shaped mass in the pelvic cavity compressing the right sciatic nerve. To confirm the diagnosis, hormonal therapy through nose dropping of buserelin acetate was given to stop menstruation. After treatment, her symptoms disappeared completely. When a sciatica is closely related to menses, orthopedic surgeons should consider cyclic sciatica resulting from endometrioma as a differential diagnosis.

A syndrome with the triad of abnormalities of mental retardation, wide mouth and intermittent overbreathing is described, which has all the features of the syndrome previously described by Pitt and Hopkins. Pitt and Hopkins published two cases that bear close resemblance to each other, each having an abnormal respiratory rhythm. Both had an unusually wide mouth and palate, thick fleshy lips, broad-beaked nose, finger clubbing and abnormal electroencephalograms. This paper discusses a client living in a hostel for intellectually handicapped people who has all these features.

A family is reported in which the 'acromegaloid facial appearance' (AFA) phenotype was segregating through two generations.
