The experience with treatment of 104 patients with burn cicatricial strictures of the esophagus and stomach is analyzed. Postoperative complications were seen in 28 (26.9%) patients. Hospital lethality was 12.5%. It is concluded that method of esophagoplasty in these patients should be selected individually.

BACKGROUND: There has been discussion about the value of laparoscopic pyloromyotomy (LP) for the treatment of hypertrophic pyloric stenosis (HPS). In their initial small series, the authors reported a relatively high complication rate. BACKGROUND: Drainage methods for the gastric conduit after esophagectomy for carcinoma have been controversial.
STUDY DESIGN: In a randomized controlled trial, 92 patients with esophageal carcinoma were randomized to have pyloroplasty or pyloromyotomy as a drainage procedure for the gastric conduit used for esophageal replacement. Nerve supporting cells were examined immunohistochemically for the expression of S-100 protein and D7 antigen (markers of peripheral Schwann cells) and GFAP (a marker of central-type nerve supporting cells) in pyloric tissue from 18 patients with infantile hypertrophic pyloric stenosis (IHPS) and 10 age-matched controls. The pathomorphological changes in the gastric mucosa and acid production were studied in 118 patients with ulcer disease of a pyloric portion of the stomach and duodenum before and after selective proximal vagotomy in combination with drainage operation. Fifty patients were randomized in a consecutive series of parietal cell vagotomy (PCV) and selective vagotomy with pyloroplasty (SV +P) in the treatment of duodenal ulcer. There were no operative deaths, and the length of the hospital stay and time off from work were the same in both groups. The clinical results were evaluated at 1 and 2 years after operation. Perforation of gastro-duodenal ulcers is followed by a 10% mortality rate. Surgery remains the first choice treatment. A series of 27 cases treated as emergencies by a gastrostomy with a "T" tube of Kehr is reported. The technique is simple and operative time minimal. It is especially indicated in perforations of more than 8 hours or happening in old or diseased patients. It is safer than simple sutures and makes an efficient postoperative intensive care possible. Its purpose is to save the patient while postponing elective surgery.
