Severe Nonischemic Dilated Cardiomyopathy Associated With Multi-Ingredient Herbal Supplement Use.

BACKGROUND: Herbal supplements marketed for cognitive and cardiovascular enhancement are widely used, yet safety data remain scarce. Supplement-associated cardiomyopathy is a rare but life-threatening complication that remains underrecognized. CASE SUMMARY: A 39-year-old previously healthy man developed progressive dyspnea, and bilateral leg edema 2 months after initiating a multi-ingredient herbal supplement. Echocardiography revealed severe left ventricular dysfunction (ejection fraction 10%-15%). Comprehensive evaluation excluded ischemic, infectious, autoimmune, infiltrative, and toxicolo

Identifying the Oxidative Stress-related Hub Genes in Dilated Cardiomyopathy by Bioinformatics Analysis.

INTRODUCTION: Characterized as an idiopathic primary myocardial disorder, dilated cardiomyopathy (DCM) predominantly affects children and elderly adults. However, a lack of specific clinical symptoms and reliable biomarkers impedes timely diagnosis and rational clinical management of DCM. METHODS: Whole-genome expression profiles (GSE120895, GSE9800) were retrieved from the Gene Expression Omnibus database via the GEOquery R package. A series of bioinformatic methods were employed, including DEG, GSVA, WGCNA, GO/KEGG enrichment, PPI, and immune infiltration analysis. Key biomarkers were valid

Dysregulated Ribonucleoprotein Granules Impair Mitochondrial Function in RBM20-Related Dilated Cardiomyopathy.

BACKGROUND: Pathogenic variants in RBM20 cause severe dilated cardiomyopathy. Loss-of-function variants disrupt splicing; neomorphic gain-of-function (GoF) variants also mislocalize RBM20 to cytoplasmic ribonucleoprotein granules and are associated with more aggressive disease. The mechanism by which RBM20 mislocalization drives cardiac dysfunction remains unknown. METHODS: We investigated the effects of Rbm20 GoF and loss-of-function (LoF) variants using proteomic profiling, protein solubility assays, mitochondrial respiration and calcium flux analyses, and ultrastructural imaging in mouse m

Body surface potential driven personalisation of electrophysiological digital twins in hypertrophic cardiomyopathy.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is associated with marked inter-patient heterogeneity in ventricular electrophysiology, contributing to arrhythmic risk that is insufficiently captured by current clinical methods. Electrocardiographic imaging (ECGI) provides high-density body surface potential (BSP) measurements but remains largely descriptive. Computational modelling offers a mechanistic framework to interpret BSP signals in terms of underlying tissue-level properties. METHODS AND FINDINGS: We developed a BSP-driven workflow to construct patient-specific electrophysiology (EP) m

Trapped in the Trabeculae: A Rare Case of Noncompaction Cardiomyopathy and Large Left Ventricular Thrombus.

Left ventricular noncompaction (LVNC) is a rare primary cardiomyopathy characterized by a failure of myocardial morphogenesis, resulting in a prominent, characteristic "sponge-like" trabeculated endocardial layer. Clinical presentation is classical for heart failure, ventricular arrhythmias, and systemic thromboembolism, which defines its severity. Here, we describe a 34-year-old male with no prior medical history who presented with progressive dyspnea and volume overload. On initial evaluation, severe biventricular failure, stage 3b chronic kidney disease (type 2 cardiorenal syndrome), and si

Novel Truncating Variant c.1222DupC in RBM20 Causes Cardiomyopathy Consistent With Haploinsufficiency.

BACKGROUND: RBM20 (RNA binding motif protein 20) is a cardiac splicing factor responsible for the splicing of several cardiac genes such as titin (TTN), triadin (TRDN), ryanodine receptor 2 (RYR2), PDZ and LIM domain protein 1 (PDLIM1), and calcium/calmodulin-dependent protein kinase II (CAMK2D). Pathogenic variants in RBM20 are a major cause of familial dilated cardiomyopathy, and lead to missplicing of RBM20 target genes. METHODS: We identified a patient with a novel RBM20 variant, and expressed the human and mouse-equivalent variant in neonatal rat cardiomyocytes and HEK293 cells. We perfo

Mavacamten and left ventricular dysfunction in hypertrophic cardiomyopathy with left bundle branch block.

BACKGROUND: Mavacamten is a cardiac myosin inhibitor used for patients with obstructive hypertrophic cardiomyopathy (oHCM). The efficacy and safety of this negative inotropic agent in oHCM and concomitant left bundle branch block (LBBB) remains largely unknown. This study aimed to explore the interaction between mavacamten treatment and LBBB on longitudinal changes in left ventricular (LV) systolic function and mechanical dyssynchrony. METHODS: This multicenter observational study included 127 patients with oHCM treated with mavacamten. Echocardiographic assessment was performed at 4, 8, 12 a

Myosin Inhibition in Non-obstructive Hypertrophic Cardiomyopathy: Lessons from ODYSSEY-HCM.

The Phase III ODYSSEY-HCM trial evaluating the cardiac myosin inhibitor mavacamten in symptomatic non-obstructive hypertrophic cardiomyopathy did not demonstrate significant improvements in clinical endpoints, including the Kansas City Cardiomyopathy Questionnaire score and peak oxygen consumption. However, mavacamten significantly reduced pro-B-type natriuretic peptide levels and improved echocardiographic indices of diastolic and left atrial function, suggesting a potential biological effect. This apparent dissociation between biomarker improvement and clinical outcomes raises questions rega

Improvement of Microvascular Ischemia by Atrioventricular Sequential Pacing in Non-Obstructive Hypertrophic Cardiomyopathy.

BACKGROUND: Therapeutic options for symptomatic non-obstructive hypertrophic cardiomyopathy (nHCM) remain limited. We investigated whether right ventricular apical (RVA) pacing with optimal atrioventricular delay (AVD) can improve myocardial oxygen supply-demand mismatch, assessed using coronary sinus (CS) lactate as a marker of microvascular ischemia. METHODS: Patients underwent atrial pacing at 70% of target heart rate followed by AV sequential pacing at individually optimized AVD. CS lactate levels were measured at baseline, during atrial pacing, after washout, and during AV sequential pac

[Burden, trends and gender disparities of alcoholic cardiomyopathy in China from 1990 to 2021].

Objectives: To analyze the burden level, temporal trends, and gender and age disparities of alcoholic cardiomyopathy (ACM) in China from 1990 to 2021. Methods: Data were extracted from the Global Burden of Disease 2021 study database. Core metrics for ACM in China-including prevalent cases, deaths, and disability-adjusted life years (DALYs)-were extracted. The absolute numbers, rates (per 100 000 population), and variation characteristics of the above mentioned disease burden parameters were analyzed by stratification according to age and gender. The Joinpoint regression model was employed to

Circulating Organic Acid Profiles in Non-Ischaemic Cardiomyopathy: A Case-Control Study.

Metabolic remodeling is increasingly recognized as a key component of cardiomyopathy, yet the circulating organic acid profile associated with this condition remains incompletely characterized. In this case-control study, we investigated differences in circulating organic acid profiles between patients with cardiomyopathy and control participants using targeted liquid chromatography-tandem mass spectrometry (LC-MS/MS). A total of 160 participants were enrolled, including 80 patients with cardiomyopathy and 80 age- and sex-comparable control participants. Circulating organic acids were quantita

Complete Recovery of Thyrotoxicosis Cardiomyopathy After Antithyroid and Corticosteroid Therapy: A Case Report.

Thyroid storm (TS) is an extreme hypermetabolic state due to thyrotoxicosis. TS may manifest as cardiovascular symptoms, including atrial fibrillation (AF) (9 - 23%) and heart failure (HF). Dilated cardiomyopathy (DCM) and HF develop at a later stage in approximately 1% of thyrotoxicosis patients. A 46-year-old man presented to the emergency department with a chief complaint of severe dyspnea and rapid AF. Blood test examination showed overt thyrotoxicosis. The Burch-Wartofsky point scale is 60 (highly suggestive of TS). Echocardiography showed severely reduced left ventricular (LV) systolic f

NAD+ precursor treatment prevents cardiomyopathy but disrupts erythroid maturation in mitochondrial progeria.

Nicotinamide adenine dinucleotide (NAD+) plays a central role in energy metabolism, and its decline is linked to various degenerative diseases. While NAD+ restoration holds therapeutic promise, its long term, tissue-specific consequences remain poorly understood. We investigated effects of nicotinamide riboside (NR) supplementation for "mutator" mice manifesting mitochondrial progeria. Our results reveal strikingly divergent outcomes: in proliferative bone marrow, NR-treated mutators show reductive stress with accumulation of NADH/NADPH, altered amino acid, nucleotide, folate levels, and impai
