[Static and dynamic changes of the cervical spine after laminectomy for cervical spondylotic myelopathy].

PURPOSE OF THE STUDY: Extensive cervical laminectomy has been widely used in the treatment of progressive myelopathies secondary to stenotic … spinal instability. RESULTS: 18 patients (31 per cent) developed postoperative changes in cervical spine curvature. 15 patients (25 per cent) had one or more destabilized levels. Deformities of the cervical spine occurring after surgery do not appear to cause any symptom or neurologic abnormalities. Destabilization required repeat surgery in 3 patients …

Septic and aseptic post-operative discitis in the lumbar spine--evaluation by MR imaging.

PURPOSE: The aim of this study was to determine whether it was possible to differentiate septic from aseptic post-operative discitis in the lumbar spine by means of MR imaging. MATERIAL AND METHODS: The study was a retrospective evaluation of 12 patients with prior lumbar discectomy and suspected post-operative discitis displaying low-back pain and typical MR findings. Six patients had elevated serum C-reactive protein (CRP) (septic) and 6 had normal CRP (aseptic). We used MR imaging to assess the distribution and degree of changes in the disc, in adjacent bone marrow, and in surrounding soft…

The relationship between vertebral body deformity and disc degeneration in lumbar spine of the senile.

This study provides an investigation of the relationship between vertebral deformities and disc degeneration in patients with senile osteoporosis using biomechanical and medical imaging methods. The finite element analysis showed that stress concentration in the central area of the vertebral body is much decreased with disc degeneration, indicating that load transmission has been altered. Radiography and MRI suggested that vertebral deformities are related to the height and degeneration of the disc just below this vertebral body. When a disc has decreased height or degeneration, the vertebral

[Ventral interbody spondylodesis in injuries of the cervical spine. Indications, surgical technique and results].

Lower cervical spine injuries with instability of the anterior and/or posterior column can be treated by anterior interbody fusion and plate fixation. Plates available for anterior instrumentation of the lower cervical spine can be divided into locking or non-locking systems with uni- or bicortical screw purchase. Our biomechanical comparative testing of different screw fixation systems demonstrates improved stability with the use of bicortical purchase. Clinical studies, however, have proven high fusion rates without loss of correction and a low implant related morbidity with the use of unico…

Magnetic resonance imaging findings in ossification of the posterior longitudinal ligament of the cervical spine.

OBJECT: Because of the lack of magnetic resonance (MR) signal from cortical bones, MR imaging is inadequate for diagnosing ossified lesions in the spinal canal. However, MR imaging provides important information on spinal cord morphology and associated soft-tissue abnormality. The purpose of this study is to determine the role of MR imaging in the diagnosis and treatment of patients with ossification of the posterior longitudinal ligament (OPLL) of the cervical spine. METHODS: The authors reviewed MR imaging findings in 42 patients with cervical OPLL who were examined with a superconducting M…

Abnormal dendritic spines in fragile X knockout mice: maturation and pruning deficits.

Fragile X syndrome arises from blocked expression of the fragile X mental retardation protein (FMRP). Golgi-impregnated mature cerebral cortex from fragile X patients exhibits long, thin, tortuous postsynaptic spines resembling spines observed during normal early neocortical development. Here we describe dendritic spines in Golgi-impregnated cerebral cortex of transgenic fragile X gene (Fmr1) knockout mice that lack expression of the protein. Dendritic spines on apical dendrites of layer V pyramidal cells in occipital cortex of fragile X knockout mice were longer than those in wild-type mice a…

CT and MRI of sellar spine with upward extension of the pituitary gland: case report.

This is a report of CT and MRI findings in a patient with a sellar spine which caused deformity of the pituitary gland. The sellar spine is an infrequent anatomical variant characterized by an osseous spine which arises in the midline from the anterior aspect of the dorsum sellae and extends into the pituitary fossa. The CT and MRI findings of sellar spine have been described in previous reports; however, only one investigator reported deformity of the pituitary gland as revealed by CT. This is the first report of the MRI finding of the sellar spine associated with a deformity and superior ext…

The effects of simple trauma on patients with cervical spine neurofibromatosis: two case reports.

Von Recklinghausen's disease of the cervical spine is rare. Spinal deformities appear to occur only in peripheral neurofibromatosis as opposed to central neurofibromatosis, and such deformities include non-dystrophic and dystrophic changes. We describe two patients with neurofibromatosis of the cervical spine who were subjects of simple trauma, one of whom demonstrated dystrophic and the other non-dystrophic changes. The first presented with acute upper cervical spine instability and subluxation, the second with cervical myelopathy. The purpose of this article is to heighten the physician's aw…

Osteomyelitis of the spine due to Salmonella infection--conservative treatment with quinolone: a case report.

Although osteomyelitis due to Salmonella infection is known to be associated with sickle cell anemia, various hemoglobinopathies and immune suppressive states, it may also occur in normal hosts. A 16-year-old Chinese boy without sickle cell disease or any other condition that would compromise the immune system had osteomyelitis of the lumbar spine caused by Salmonella enteritidis. The condition was treated conservatively with ciprofloxacin (quinolone group). This may be the first reported case in which a patient with spinal osteomyelitis due to Salmonella infection, who was otherwise healthy, …

Cervical spine degenerative diseases: an evaluation of clinical and imaging features in surgical decisions.

In clinically severe cervical spondylosis, imaging plays a vital role in surgical decisions. A prime factor is acquired canal stenosis with cord compression. To validate this concept, the clinical and imaging features of 20 patients with spondylotic myelopathy and 24 with radiculopathy were retrospectively reviewed. All had computed tomographic myelography (CTM) as part of their clinical work-up. The patients' clinical severity was graded as mild, moderate and severe; the age, length of illness and a history of eventual surgery or otherwise were recorded. At the level of maximum compression th

The vertebral column--a phylogenetic failure? A theory explaining the function and vulnerability of the human spine.

The increase in degenerative diseases of the vertebral column is often attributed to an inadequate adaptation to the upright posture in the human. On the basis of a precise analysis of the motion segments, an attempt has here been made to demonstrate that the design of the vertebrae in both the larger mammals and in humans has resulted in no qualitative differences between the stresses to which either is subjected. In the course of evolution there has certainly been an obvious conflict in aims between the need for essential stability and the desired or necessary mobility. These mutually self-l

[Biomechanical changes in and lesions of the lumbosacral spine in horse riding. Magnetic resonance assessment].

Spinal injuries account for 5-15% of sport-related traumas. Equestrian sports are considered, together with rugby, motor sport and diving, the riskiest sport for severe spinal injuries. We investigated the biomechanical changes and repeated microtraumas in equitation. We examined with MR the lumbar spines of 12 professional horsewomen and horsemen, 18-51 years old (mean: 33.4 years), belonging to F.I.S.E. (the Italian Federation for Equestrian Sports) and classified as Olympic riders, Senior and Young European riders. An 0.3-T resistive and an 0.2-T permanent magnets with dedicated surface coi…

Experimental impact injury to the cervical spine: relating motion of the head and the mechanism of injury.

The purpose of this study was to analyze, with use of an impact model, the relationships among motion of the head, local deformations of the cervical spine, and the mechanisms of injury; the model consisted of the head and neck of a cadaver. Traditionally, the mechanisms of injury to the cervical spine have been associated with flexion and extension motions of the head and neck. However, the classification of the mechanisms is not always in agreement with the patient's account of the injury or with lacerations and contusions of the scalp, which indicate the site of the impact of the head. Elev…
