Treatment of fixed thoracolumbar kyphosis in immature achondroplastic patient: posterior column resection combined with segmental pedicle screw fixation and posterolateral fusion.

A 13-year-old male achondroplastic dwarf with fixed thoracolumbar kyphosis was treated by segmental pedicle screw fixation, posterolateral fusion combined with one stage two level posterior column resection. Preoperative and postoperative kyphosis angles were 97 and 32 degrees, respectively. Combination of segmental pedicle instrumentation with posterior column resection is a treatment option even in immature achondroplastic patients.

Mechanical conditions in the internal stabilization of proximal tibial defects.

OBJECTIVES: The goal was to design a method which would permit an assessment of the suitability of a newly developed implant under physiological-like loading conditions. Information obtained from such an analysis is expected to delineate more clearly the indications for a new device prior to clinical utilization. DESIGN: In vitro mechanical stiffness testing and finite element analysis. METHODS: From in vitro testing of proximal tibiae with defects, the stiffness of an internal stabilization system was determined. Using a finite element model, the loading of both the implant and bone was ana

The use of absorbable material in correction of pectus deformities.

Various techniques have been described for correction of the congenital anterior chest wall deformities. We described a new technique in which 33 patients were used in correction of those deformities. In this new technique we used the resorbable copolymer plaque and polymer screw for the fixation of the sternum. We have obtained excellent results in one-year follow up period. This technique was developed to avoid a second operation, which usually requires for removal of the metal support. Resorption of copolymer plaques and polymer screws are completed at the end of the first year.

Surgical treatment for concomitant fractures of the femoral neck and diaphysis: a treatment protocol.

Concomitant ipsilateral femoral shaft and neck fractures are difficult to treat. The standard protocol in our hospital is plates for diaphyseal fractures and lag screws or dynamic hip screws (DHS) fixations for the femoral neck fractures. We treated 21 patients with these complex fractures between 1988 and 1998 with this protocol. All patients were injured from high-energy trauma. There were 15 males and 6 females with an average age of 44 years. The average follow-up period is 50 months. All except one patient united well. During the follow-up period, neither non-union of the femoral neck fra

Recognition and management of Tillaux fractures in adolescents.

Tillaux fractures are relatively uncommon Salter Harris III fractures of the tibia. The importance of recognizing this fracture is that a residual deformity in the joint surface can lead to premature degenerative arthritis. For this reason, it is important that accurate imaging to assess the congruity of the joint, as well as adequate reduction, is obtained. These fractures can occur in adolescents in the 18-month period during which the distal tibial epiphysis is closing. These injuries occur either by lateral rotation of the foot or by medial rotation of the leg on the fixed foot. Closed red

The surgical treatment of spondylolisthesis with transpedicular stabilization: a review of 25 cases.

The present study concerns a series of 25 patients submitted to vertebral stabilization and fusion for the treatment of spondylolisthesis. In all of the cases a transpedicular screw system was used. The Steffee plates and screws system was used in 22 cases; the Cotrel-Dubousset (CD) system was used in 3. The listhetic vertebra was L5 in 14 cases, it was L4 in 11 cases. Based on Meyerding radiographic criteria there were: grade I: 8 cases, grade II: 13 cases, grade III: 4 cases. Instrumented fusion was performed at only 1 level in 20 cases, at 2 levels in 5 cases. Mean follow-up was 29 months.

[Direct screwing in spondylolysis: results and observations on indications].

Ten patients suffering from persisting low-back pain underwent direct repair of spondylolysis L5 with hook screws. At a follow-up three to four years after the operation, four patients showed good and six unsatisfactory results. Four patients had a secondary procedure. Average age was relatively high in the study group compared with the literature, and pain tended to be chronic. In five of the six patients with a poor clinical result, radiological consolidation of the lytic defect could not be demonstrated. We are discussing indications based on a literature review.

Biomechanical analysis of single- versus double-screw fixation in slipped capital femoral epiphysis at physiological load levels.

Slipped capital femoral epiphyses in 12 pairs of bovine femora were pinned with a single screw on one side and two screws on the other. The specimens were subjected to physiological shear loads across the epiphysis (100 cycles at 1.1 Hz; 400 N for slow walking, and 900 N for fast walking). The rates of creep were decreased 23% with double screws compared to single screws at slow walking, and 30% at fast walking; this was not statistically significant. Single-screw fixation in slipped epiphyses is thus recommended, because the small gains in resistance to cyclic creep at physiological loading w

Current methods of dorsal spine fusion with pedical screws and presentation of a new method for treatment of fractures and low back instability.

A new instrumentation system, the H-frame system, has been developed for dorsal fusion in scoliosis, spinal fractures and degenerative low back problems. The basis is a prelinked slotted H-frame with two rods of 6-mm diameter with 5-cm threaded ends. There are 4 different lengths, solid in the middle and threaded at the ends. Connection with hooks and/or pedical screws is easy and stable. A slotted system using special conical nuts to grasp these hooks or screws tightly prevents loosening. For low back fusion in degenerative disease the slightly lordotic H-frame is connected to 4 screws. If ne

[Muscle-flap plasty in fractures of the tibial pilon].

It is not rare in fractures of the distal lower leg, that precarious soft-tissue conditions compromise the outcome of early osteosynthesis. Because of oedema and haematoma the wound is widely split open and its primarily closure is perilous. In such a situation the stabilized fracture can be covered primarily with a local muscle-flap, such as extensor digitorum communis or peronaeus brevis. In the last 2 years we have chosen this solution in 6 cases.

Irreducible dislocation of the distal radioulnar joint.

Although dislocation of the distal radioulnar joint is commonplace in association with fractures of the radial shaft, irreducible dislocation has previously been considered to be rare. In the only three previously reported cases the tendon of the extensor carpi ulnaris blocked reduction of the distal radioulnar joint in Galeazzi injuries. The cases presented in this report show that other tendons may be involved, and that the injury may occur even when the ulna is broken. It is likely that the injury described is more common than is realized and is usually overlooked.

[Dislocation of the head of the fibula--diagnostic and therapeutic problems].

Luxation of the head of the fibula in the proximal tibiofibular joint is rare and is often overlooked. Pointers in this direction should be a swelling above the head of the fibula that is sensitive to pressure, an inexplicable pattern of signs involving the fibula, as well as an undefined insecurity of gait associated with the absence of other injuries in the genicular area. Diagnosis is best confirmed in the hospital by means of luxation or repositioning manoeuvres at the joint. The experienced physician can draw further conclusions from x-ray films of the forced extreme joint position. Thera

Bilateral posterior dislocation of the shoulder combined with fracture of the proximal humerus. A case report.

A 28-year-old turner presented with bilateral posterior fracture-dislocation of the shoulders following an epileptic seizure. He was treated by open reduction of the humeral heads and fixation of the oblique fractures with screws. At the follow-up examination 2 years postoperatively shoulder mobility was satisfactory, and the patient could return to his previous work.

Experimental scoliosis by epiphysiodesis in pigs.

Using two-month-old pigs selective epiphysiodesis of the neurocentral cartilage was performed using cancellous screws inserted to achieve effective compression. The operation was performed on eight experimental animals, on the right side, involving four or five vertebrae at the mid thoracic level. The procedure consistently caused structural scoliosis, convex on the side of operation. The radiographic, macroscopic and microscopic findings in the experimental animals are described. In other animals so treated at ages when the neurocentral cartilage is not active rotational deformity did not occ

Spinal instrumentation in the treatment of severe progressive spondylolisthesis.

By using A-frame instrumentation and direct distraction force, an exceptional correction can be obtained in patients with severe spondylolisthesis. Adolescent progressive spondylolisthesis should be considered as an entity separate from the lumbosacral anomalies such as spondylolysis and grades 0 and 1 spondylolisthesis without progression. An aplastic relationship appears to exist between the sacrum and the posterior superior wing of the ilium in the adolescent with progressive spondylolisthesis. Complications have been minimal but long-term observations of patients are necessary to evaluate
