Functional assessment of tibial-nerve recovery in the cat using gait analysis: preliminary study.

The purpose of this study was to investigate gait-pattern changes after complete tibial nerve lesion in the cat, and to observe whether nerve repair could reverse some of the changes. In six cats, a 5-cm segment of the tibial nerve was transected. The nerve gap was then repaired with nerve autograft in three animals and was unrepaired in three as controls. The walking patterns of the cats were videotaped, and the hip, knee, ankle, and metatarsophalangeal joint angles were measured at the beginnings of the F, E1, E2, and E3 phases of the step cycle. Two weeks after surgery, abnormal gait patter

Wrist tremor: investigation of agonist-antagonist interaction by means of long-term EMG recording and cross-spectral analysis.

We have developed a method to observe and quantify the phasic relationship between the burst patterns of antagonistic muscles in any roughly periodic movement, such as tremor, for up to 24 h. This paper describes our approach, in which long-term EMG recordings are evaluated by a procedure based on cross-spectral analysis. The method is then illustrated by data from 6 patients with essential tremor. Our data confirm earlier observations that a particular patient may show different burst patterns at different times. However, as opposed to tremor analysis based on short-term recording, our method

[Contribution of the electromyogram in the diagnosis of infantile spinal muscular atrophy in the neonatal period].

BACKGROUND: The acute form of Werdnig-Hoffman disease, infantile spinal muscular atrophy type I (SMA I), is characterized by severe paralytic hypotonia with neurogenic electromyographic (EMG) pattern and specific histologic features. PATIENTS: Four cases of very severe SMA I suffering from generalized muscle weakness at birth were included in the study. RESULTS: The neurogenic EMG pattern was observed at the first exam performed between D2 and D46. The muscular biopsy performed between D18 and D45 showed only a mild decrease of the muscle fiber size without grouping of fiber types. CONCLUSI

Fasciculation frequency changes at different length of relaxed muscle.

We have studied, in chronically denervated muscles, fasciculation potentials (FPs) which were clearly identified when the muscles were either relaxed or passively stretched. The frequency of 10 FPs was determined in both positions in seven patients. In stretched muscles, two FPs were no longer recorded, and the mean FP frequency decreased significantly (P < .01). There was also a decrease of the FP amplitude. As FPs are frequently associated with ordinary muscle cramps, stretching of the muscle may lessen the frequency of FPs by the same mechanism which stops cramps.

Lumbrical-interossei motor studies localize ulnar neuropathy at the wrist.

Ulnar nerve entrapment at the wrist (UNW) is uncommon and often difficult to localize electrophysiologically. The difference between the motor latencies to the median-innervated second lumbrical (2L) and ulnar-innervated palmar interosseous (Pl) (Diff 2L-Pl) has been shown to be of localizing value in patients with median neuropathy at the wrist. In the last year, we evaluated 2 patients with clinically definite ulnar neuropathy at the wrist. We performed motor studies to the 2L-Pl on the 2 patients and 12 disease controls with ulnar neuropathy at the elbow as follows: Using the same electrode

[Simultaneous bilateral facial paralysis].

Simultaneous bilateral facial paralysis (SBFP) is very rare, but it can present itself as the first complaint in a wide spectrum of diseases and a comprehensive evaluation must be completed. Bilateral Bell's Palsy can be diagnosed only when all other possible causes have been excluded. The prognosis and treatment for SBFP is dependent upon the underlying etiology. We present four cases of SBFP seen in our department which were finally diagnosed as leukemic infiltration, sarcoidosis, Bell's Palsy and demyelinating polyneuropathy (probably Guillain-Barré syndrome). The most important etiologies

Early features of zidovudine-associated myopathy: histopathological findings and clinical correlations.

Zidovudine-induced myopathy is characterized by reversible muscle weakness, wasting, myalgia, fatigue, and elevated creatine kinase (CK). Some zidovudine-treated patients with normal muscle strength experience excessive fatigue, myalgia, or transient mild CK elevations that improve when zidovudine is stopped. To determine the cause of these symptoms, we studied 13 physically fit, HIV-infected men who developed fatigue, myalgia, and reduced endurance, while taking zidovudine for a mean period of 20 months (2-39 months), with neurological evaluation and muscle biopsy processed for enzyme histoch

Blepharospasm and related facial movement disorders.

The variable clinical features and the relatively good response of blepharospasm to botulinum-toxin type A are now well established. The etiology and pathophysiology of blepharospasm and related facial movement disorders are still poorly understood. Genetic and histopathologic studies over the last year have contributed to our understanding of this disease. The most significant progress has been made in the electromyographic studies of the the levator palpebrae and orbicularis oculi muscles. Subclassification based on the electromyographic abnormalities of these two muscles have begun to impro

Effect of vibration on antagonist muscle coactivation during progressive fatigue in humans.

1. Biceps femoris antagonist coactivation increases during progressive fatigue. Our purpose was to determine if the mechanism that increases coactivation during fatigue is susceptible to vibration. Vibration drives alpha-motoneurons via the Ia loop, producing force without descending motor drive, and thus uncoupling antagonist and agonist activation. Evidence that vibration increases coactivation disproportionately from its 'common drive' would suggest the possibility that some of the effects of fatigue are mediated through a segmental reflex loop. 2. Ten male subjects performed repeated maxim

Joint dependent passive stiffness in paretic and contralateral limbs of spastic patients with hemiparetic stroke.

Torque-angle relations at the elbow and ankle joints of relaxed normal controls and patients with hemiparetic stroke were compared. Low velocity flexion/hold/extension angular perturbations were applied to the joint under examination. The resulting torque-angle profiles described a hysteresis loop with similar slopes during the extension and flexion stages but separated by a vertical torque offset. Torque-angle responses obtained in the absence of significant muscle activation, as recorded by surface electromyographic activity, were designated as passive. Elbow passive stiffness estimates were

Contraction of knee flexors and extensors in skiing related to the backward fall mechanism of injury to the anterior cruciate ligament.

The purpose of this study was to examine the motion pattern and muscle activity during jumping and landing in downhill skiing to estimate the muscular involvement during the jump-landing-backward fall-anterior cruciate ligament (ACL) injury. A digitized 2-D video motion analysis and a synchronous 8-channel electromyography registration was performed over a jump on a downhill slope during 3 runs by 2 skiers and during 6 control jumps in the laboratory. The knee flexors (lateral gastrocnemius and biceps femoris) were recruited before touchdown and mean 60 ms earlier than the extensors (vastus la…

Rehabilitation of the shoulder in tennis players.

The tennis player places unique demands on the shoulder by creating a high risk for overuse and overloading of the soft tissues. Tennis requires concentric work to position and move the arm, eccentric work to stabilize the shoulder, effective depression of the humeral head to avoid impingement in the overhead position, and normal stability to prevent secondary impingement. The tennis serve produces enormous angular velocities about the shoulder joint. A comprehensive rehabilitation program has been described in which the therapist, trainer, player, and physician alike need to have an understan

[Clinical investigation for polymyositis and related disorders].

The clinical investigation for inflammatory myopathies, which include polymyositis (PM), dermatomyositis (DM) and others, was outlined. The serum creatine kinase (CK) activity increases in the majority of cases of inflammatory myopathies. However, the cases of myositis associated with connective tissue diseases tend to show normal or moderately elevated CK activity. Among the isoenzymes of CK, the MB fraction can increase in the course of treatment as it can originate from regenerating muscle fibres. The macro CK type 1 was reported to appear in association with myositides. Varieties of autoan

Rapid wrist movements in patients with essential tremor. The critical role of the second agonist burst.

Ballistic wrist flexion movements towards 15, 30 and 60 degrees visual targets were studied in a group of 17 patients with hereditary essential tremor. Compared with age-matched normal subjects (n = 16), there were three main kinematic differences: patients overshot the target a little more; the kinematic profile of their movements was more 'asymmetric' due to higher peak decelerations; and their movements initiated tremor. Ballistic movements performed by patients with essential tremor were associated with a triphasic pattern of agonist-antagonist-agonist muscle activity similar to that of no

Muscle fiber conduction velocity in the diagnosis of familial hypokalemic periodic paralysis--invasive versus surface determination.

Muscle fiber conduction velocity (MFCV) in the brachial biceps muscle was determined in a large family of patients with hypokalemic periodic paralysis (HOPP) by both a surface and an invasive method. Other surface EMG parameters and the muscle force were also determined. Both the surface and the invasive method showed a significantly lower mean MFCV in the proven gene carriers but only the invasive method showed a lower MFCV in all proven carriers. It can be concluded that MFCV determination is a reliable method to detect the membrane defect in HOPP carriers and that the invasive method is not
