Congenital pseudarthrosis of the tibia. Factors that affect results.

Congenital pseudarthrosis of the tibia is a rare disease of unknown etiology and variable natural history. Treatment offers a high percentage of poor results. The decision of when the limb will be cosmetically and functionally better than a below-knee prosthesis is a critical one that the orthopedic physician must reach with the patient. The presence or absence of neurofibromatosis apparently does not alter the result. Those cases which show a cystic radiographic appearance have a more favorable prognosis than those which become or present as dysplastic. The more times the patient is bone graf

Anterior tibial tendon transfer in idiopathic clubfoot: does the outcome differ with the initial treatment? Proposed classification to surgical indication.

PURPOSE: To establish parameters and propose a radiographic classification for foot adduction/supination deformities to assist the indication of Garceau procedure. Secondly, to investigate whether the outcome of Garceau surgery depends on the initial treatment used, peritalar release, or the Ponseti method, and verify the maintenance of correction until skeletal maturity. METHODS: Prospective cohort study, with follow-up evaluations in 2009 and 2019. Fifty-three consecutive patients (71 feet) with idiopathic congenital clubfoot (ICCF) were divided into two groups according to the initial trea

Troubleshooting in the Treatment of Pseudoarthrosis After a Humeral Diaphyseal Fracture: A Case Report.

We report a case of a patient with a history of surgery for insufficient pseudoarthrosis after a humeral diaphyseal fracture. Although most humeral nonunions are successfully treated with a single procedure, some humeral nonunions are more difficult to heal and require multiple procedures. A-61-year-old man presented at our hospital with pain and deformity in his left upper arm. The humeral diaphyseal fracture had been previously treated elsewhere with open reduction and internal fixation. Some days prior to his visit at our hospital, he had had pain in his left upper arm, without a particular

When it is not an infection: metal allergy after the Nuss procedure for repair of pectus excavatum.

PURPOSE: Increasing use of implantable bars for minimally invasive pectus excavatum repair has introduced metal allergy (nickel and chromium) to pediatric surgeons. Metal allergy is a well-recognized entity in neurologic, orthopedic, and craniofacial surgery. This study was performed to evaluate metal allergy and its effects on treatment with the Nuss procedure in 862 patients. METHODS: After institutional review board approval, we undertook a retrospective review of a prospectively gathered database of patients undergoing the Nuss procedure. Metal allergy was diagnosed either with the use of

Defining surgical strokes in lumbar laminectomy: toward objective skill assessment and robotic integration.

PURPOSE: To characterize discrete drilling stroke phenotypes during lumbar laminectomy using quantitative motion features, and to assess how phenotype usage varies by procedural stage, training level, and navigation assistance. METHODS: Eleven orthopedic trainees each performed 10 lumbar laminectomies (110 total) on a CT-based VR simulator (CAPTAiN), split between CAPTAiN-assisted and non-navigated conditions. Burr strokes were quantified by length, velocity, acceleration, jerk, bone removed, and scaled proximity to the dura. Stroke feature vectors were embedded using UMAP and clustered via k

Results of the Girdlestone Procedure for the Treatment of Refractory Hip Prosthesis Infections.

INTRODUCTION: Refractory prosthetic joint infections (PJI) of the hip remain a major challenge in orthopedic surgery. The Girdlestone procedure, also known as resection arthroplasty, was originally described for native hips but has since been adopted as a salvage option for complex and recurrent prosthetic infections. The main objective of this study was to evaluate the effectiveness of this procedure in treating prosthetic hip infections in this context. MATERIALS AND METHODS: Eighty-eight patients (91 hips) who underwent femoral resection arthroplasty for refractory hip prosthesis infection

Osmotic pump with potential for bone lengthening distracts continuously in vitro and in vivo.

BACKGROUND: In pediatric orthopedics, long bone lengthening procedures are routinely performed using manual, motorized or magnetically controlled implants. This study aims to prove expansion of a newly designed osmotic pump prior to long bone lengthening in living organisms and to rule out any complications related to in vivo conditions, such as congestion of the semipermeable membrane, local infection, or lack of water to drive the osmotic pump, as well as to compare in vivo and in vitro expansion data. METHODS: Osmotic pumps, which were designed to distract a plate osteosynthesis, were inse

Comparison of outcome of tibial plafond fractures managed by hybrid external fixation versus two-stage management with final plate fixation.

BACKGROUND: Tibial platfond fractures are usually associated with massive swelling of the foot and ankle, as well as with open wounds. This swelling may cause significant decrease of the blood flow, so the state of the soft tissue is determinant for the surgical indication and the type of implant. This retrospective study compares the union times in cases of tibial plafond fractures managed with a hybrid external fixation as a definitive procedure versus those managed with a two stage strategy with final plate fixation. MATERIALS AND METHODS: A retrospective study in a polytrauma referral hos

Assessment and management of the lower extremity in myelodysplasia.

The goal in managing the lower extremity in a child with spina bifida is to achieve a pattern of development as near normal as possible. Lower limb deformities are produced by muscle imbalance, weight-bearing, and the postural effects of gravity. At any point in time, the management of the lower extremities will depend on the child's general development. Lower limb paralysis, hydrocephalus, bladder infections, hydromyelia, and Arnold Chiari malformation, all contribute to developmental delay. A child's potential for mobility varies with the severity of the motor and sensory deficit. Surgery is

Unplanned Emergency Room Visits following Hip Arthroscopy.

PURPOSE: Hip arthroscopy is an increasingly common orthopedic procedure. Although the number of reported complications is low, emergency room visits in the early postoperative period do occur. This study aims to evaluate the impact of demographic, insurance, and clinical factors on the likelihood of postoperative emergency department (ED) visits following hip arthroscopy. Through the identification of key risk factors, including insurance status, comorbidities, and procedural specifics, the objective of this study is to inform clinical practices and enhance patient outcomes through targeted ri

Development and implementation of an automated micrometric bone distraction system based in a fuzzy logic controller for an Aybar-type external fixator.

Bone distraction is a widely applied orthopedic procedure based on Ilizarov's tension-stress principle, which requires high precision and accuracy to guarantee successful bone regeneration. In many low resource locations, the Aybar-type external fixator is the only viable treatment option due to its low cost and accessibility. However, its manual procedure introduces human error, reduces precision, and compromises treatment outcomes. To address this problem, this study develops and implements an automated micrometric distraction system for an Aybar-type fixator, aiming to provide a low cost ye

A Traumatic Neuroma Formation Following Fasciotomy for the Treatment of Tibialis Anterior Muscle Herniation: A Case Report.

Muscle herniation of the lower extremity, such as tibialis anterior muscle herniation (TAMH), is not a rare cause of leg pain in athletes. However, a few studies have reported surgical treatment for TAMH, and the optimal surgical procedure remains controversial. Fasciotomy was reported to be effective for patients with TAMH. However, this procedure would be associated with a risk of intraoperative injury to the superficial peroneal nerve (SPN), although no previous literature has reported this complication. This case report aimed to report a case of bilateral TAMHs in which a traumatic neuroma

Complication rates after proximal femoral nailing: does level of training matter?

BACKGROUND: Surgical treatment of pertrochanteric fractures is one of the most performed surgeries in orthogeriatrics. Proximal femoral nailing, the most performed procedure, is often used as a training surgery for young residents. The objective of this study was to evaluate the relevance of the resident's training level to complication rates. MATERIAL AND METHODS: This study was a retrospective cohort study. Surgeons were divided into four groups according to their training level. Complications included infection, cut-out, and revision surgery. The study was performed at a level 1 trauma cen

Umbilical cord mesenchymal stem cells combined with secretome for treating congenital pseudarthrosis of the Tibia: a case series.

INTRODUCTION: Most patients with congenital pseudoarthrosis of tibia (CPT) do not have promising outcomes despite multiple attempts using conventional surgeries. The combination of umbilical cord-derived mesenchymal stem cells and conditioned medium (secretome) contains major components pivotal for the enhancement of fracture healing. The purpose of this study was to address fracture healing in CPT cases that were treated using the combined implantation of umbilical-cord mesenchymal stem cells (UC-MSCs) and secretome. MATERIALS AND METHODS: From 2016 to 2017, six patients with CPT who were tr

Arthroscopic rotator cuff repair in fibromyalgia patients had comparable outcomes to a matched control group.

BACKGROUND: Although fibromyalgia is associated with poor outcomes following orthopedic surgeries, several studies show some benefit from surgical intervention and nevertheless recommend operative treatment when indicated. There is sparse evidence of the effect of fibromyalgia on the outcomes of shoulder surgery. The purpose of this study was to investigate the effect of fibromyalgia on patient-reported outcomes of arthroscopic rotator cuff repair (ARCR). METHODS: All patients with a confirmed diagnosis of fibromyalgia who underwent ARCR in one institution between 2010 and 2021 were included.

A novel comprehensive radiation shielding system eliminates need for personal lead aprons in the catheterization laboratory.

OBJECTIVES: This clinical study evaluated the efficacy of a novel radiation shielding system for the cardiac catheterization laboratory designed to provide comprehensive protection that obviates the need for personal lead aprons. BACKGROUND: Invasive Cardiologists are exposed to occupational health hazards related directly to radiation exposure (RE) and indirectly to the orthopedic burden of wearing only partially protective lead aprons. Innovations to reduce these risks are warranted. A novel comprehensive shielding system (ProtegoTM , Image Diagnostics Inc, Fitchburg, Ma) has been validated

Risk of Instability After Revision Total Knee Arthroplasty for Periprosthetic Joint Infection.

Periprosthetic joint infection (PJI) remains a major source of morbidity after total knee arthroplasty (TKA). The risk of recurrent infection has been more extensively studied than the risk of mechanical failure. We sought to define the incidence of instability after revision TKA for PJI and to compare this incidence with that for revision TKA for instability. We retrospectively reviewed patients treated by 4 arthroplasty surgeons at 1 institution. The primary outcome was a new diagnosis of clinical instability after index revision. We analyzed potential risk factors that may contribute to pos
