R-wave peak time at lead II in Chinese healthy adults.

BACKGROUND: Wide QRS complex tachycardia (WCT) is a common arrhythmia. How to differentiate between WCTs is a challenge in clinical practice. Recently R-wave peak time (RWPT) at lead II was reported to be a helpful and simple tool for differentiating WCTs. However, it has remained unknown about the reference range of RWPT at lead II. In present study, we aimed to investigate the reference range of RWPT at lead II in Chinese healthy adults. METHODS: A retrospective study was conducted in the First Affiliated Hospital of Shantou University Medical College in Southern China. Two thousand four hu

Feasibility and Accuracy of Cardiac Magnetic Resonance Imaging-Based Whole-Heart Inverse Potential Mapping of Sinus Rhythm and Idiopathic Ventricular Foci.

BACKGROUND: Inverse potential mapping (IPM) noninvasively reconstructs cardiac surface potentials using body surface potentials. This requires a volume conductor model (VCM), usually constructed from computed tomography; however, computed tomography exposes the patient to harmful radiation and lacks information about tissue structure. Magnetic resonance imaging (MRI) is not associated with this limitation and might have advantages for mapping purposes. This feasibility study investigated a magnetic resonance imaging-based IPM approach. In addition, the impact of incorporating the lungs and the

β1-Adrenergic and M2 Muscarinic Autoantibodies and Thyroid Hormone Facilitate Induction of Atrial Fibrillation in Male Rabbits.

Activating autoantibodies to the β1-adrenergic and M2 muscarinic receptors are present in a very high percentage of patients with Graves' disease and atrial fibrillation (AF). The objective of this study was to develop a reproducible animal model and thereby to examine the impact of these endocrine-like autoantibodies alone and with thyroid hormone on induction of thyroid-associated atrial tachyarrhythmias. Five New Zealand white rabbits were coimmunized with peptides from the second extracellular loops of the β1-adrenergic and M2 muscarinic receptors to produce both sympathomimetic and parasy

Transdifferentiation of cardiac fibroblasts, a fetal factor in anti-SSA/Ro-SSB/La antibody-mediated congenital heart block.

The signature lesion of autoantibody-associated congenital heart block (CHB) is fibrosis of the conducting tissue. To date, participation of myofibroblasts in the cascade to injury has been unexplored. The importance of myofibroblast/macrophage cross-talk is demonstrated by the novel finding of these cell types in the heart of a neonate dying of CHB. This clue to pathogenesis prompted consideration of the mechanism by which maternal anti-SSA/Ro-SSB/La Abs initiate an inflammatory response and promote fibrosis. Isolated cardiocytes from 16-24 wk abortuses were rendered apoptotic by exposure to

Transthyretin Cardiac Amyloidosis: An Evolution in Diagnosis and Management of an "Old" Disease.

Heart failure with preserved ejection fraction (HFpEF) is a heterogeneous syndrome, and cardiac amyloidosis (CA) is one of the causes of HFpEF, that has established and emerging treatment options. However, it remains an underdiagnosed and often overlooked cause of HFpEF. The importance of early diagnosis cannot be emphasized enough, as emerging therapies are more effective early in the course of the disease. Further, because of the unique physiologic and hemodynamic features of CA, patients poorly tolerate traditional heart failure medications and experience worse outcomes compared with other

Understanding and managing dextrocardia with situs inversus: A case report.

Dextrocardia with situs inversus is an uncommon congenital disorder in which the heart and other organs in the chest and abdomen are reversed. Although it rarely causes symptoms, it can be linked to heart or systemic disorders and is challenging to diagnose due to its unique architecture. The present report describes the case of a 27-year-old man who presented with sudden breathing problems. A physical examination revealed no heartbeat on the left side and odd cardiac noises. Further electrocardiogram tests revealed classic symptoms of dextrocardia, including right axis deviation and inverted

The Influence of Exercise and Physical Activity on Autonomic Nervous System Function Measured by Heart Rate Variability in Individuals with Type 1 Diabetes Mellitus-A Systematic Review.

Non-pharmacological interventions, such as physical activity and exercise, are essential in managing type 1 diabetes mellitus by improving glycemic control, cardiovascular health and autonomic function. Given the chronic nature and long-term complications associated with type 1 diabetes, strategies beyond pharmacotherapy are essential. This review examines the effects of exercise on heart rate variability, a key indicator of autonomic nervous system activity. A systematic search was conducted in March 2024 across PubMed, Embase, Cochrane and CINAHL databases. Studies evaluating the retrospecti

Overview of bile acid signaling in the cardiovascular system.

Bile acids (BAs) are classically known to play a vital role in the metabolism of lipids and in absorption. It is now well established that BAs act as signaling molecules, activating different receptors (such as farnesoid X receptor, vitamin D receptor, Takeda G-protein-coupled receptor 5, sphingosine-1-phosphate, muscarinic receptors, and big potassium channels) and participating in the regulation of energy homeostasis and lipid and glucose metabolism. In addition, increased BAs can impair cardiovascular function in liver cirrhosis. Approximately 50% of patients with cirrhosis develop cirrhoti

Late results of surgery for congenital heart defects.

Surgery for congenital heart defects started 50 years ago with "closed" procedures, and open heart surgery is in its forth decade. Thus, long-term results are now available. Although a majority of patients lead normal lives, problems do exist. Hemodynamic anomalies can be related to residual lesions or to persistent systemic hypertension (after coarctation repair) or pulmonary hypertension (after late repair of left to right shunt lesions). Right or left ventricular dysfunction may be observed, due to longstanding overload, hypoxia or to the open heart procedure itself. Rhythm disturbances hav

Effects of mu-opioid receptor agonists on circulatory responses to simulated haemorrhage in conscious rabbits.

1. Cardiac output, arterial pressure, heart rate, systemic vascular conductance, respiratory rate and arterial blood PO2 and PCO2 were measured in unanaesthetized rabbits. Haemorrhage was simulated by inflating a cuff placed around the inferior vena cava so that cardiac output fell at a constant rate of about 8% of its resting value per min. 2. The effects of drug treatments on resting haemodynamic and respiratory variables, and on the haemodynamic response to simulated haemorrhage, were tested. The treatments were; 4th ventricular (-)-naloxone HCl (10-100 nmol), 4th ventricular H-Tyr-D-Ala-Gl

Timing Ultrafast Charge Transfer via Fano Interference beyond the Core-Hole Clock.

Ultrafast charge transfer (CT) lies at the heart of molecular and electronic functionality. We develop a Fano-based core-hole clock (FCHC) method that captures coherent coupling between localized excitons and the directly populated delocalized CT continua in resonant Auger scattering. Applied to sulfur KLL Auger spectra of a conductive organic polymer, FCHC reveals clear Fano interference and a CT time of 27±1.8  fs, demonstrating that the conventional core-hole clock model underestimates the CT dynamics timescale. We establish FCHC as a robust method for probing ultrafast CT in complex molecu

Lesion area, ablation time, fluoroscopic time, and short-term outcomes among 3 pulsed field ablation systems.

BACKGROUND: Three pulsed field ablation (PFA) systems are approved in Japan: a pentaspline catheter (FARAPULSE [FP]) and 2 loop circular catheters (VARIPULSE [VP] and PulseSelect [PS]). However, the differences in the lesion characteristics, procedural parameters, complications, and short-term outcomes remain unclear. OBJECTIVE: This study aimed to compare the PFA-related features among the FP, VP, and PS systems. METHODS: Overall, 75 patients with atrial fibrillation (paroxysmal: 54; persistent: 21) underwent PFA using FP (n = 25), VP (n = 25), or PS (n = 25). Post-ablation, low-voltage are
