A 27 year old man developed multiple ventricular septal defects and a coronary artery ventricular fistula after a blunt injury. These were treated successfully by surgery, which was carried out four months after the injury to allow some healing of the defect to occur.

Blunt chest trauma occurs in up to 50 per cent of all fatal motor vehicle accidents and is the primary cause of death in 12-25 per cent; yet only 15 per cent of patients with chest trauma arriving alive to the emergency department require early thoracotomy. Although pulmonary disability is usually minimal following total sternectomy, the potential for remote complications exists. We present a case of severe respiratory failure resulting from simple rib fractures occurring two years after sternectomy for sternal osteomyelitis. Respiratory failure resulted from flail chest due to the combination of acute rib fracture and the lack of anterior fixation of the thoracic cage.

Traumatic tricuspid insufficiency is a rare condition which raises the problem of when and how should its surgical correction be performed. Its diagnosis has been considerably facilitated by echocardiography. This is the third known reported case in the medical literature of complete paraplegia following acute trauma in a patient with tuberculous kyphosis. Early clinical evidence of neurological recovery and the absence of a radiological fresh lesion are important indications for conservative treatment. The patient made a complete recovery. A case of pulmonary and aortic punctures occurred during an attempted internal jugular venipuncture in a patient with a dilated aortic arch. Procedural errors contributing to the complications included an inexperienced operator, inappropriate medial and caudal angulation of the needle, and failure to use a "finder" needle or Seldinger technique. Use of proper procedural technique will reduce the risk of complications in performing this procedure.

A case of massive transfixion of the chest is described. It was sustained by an 18-year-old male during a high-speed motor accident. After initial treatment and operative management, complications included hemothorax and empyema. Following drainage and gentamicin the patient recovered uneventfully. Rapid transportation and correct initial management undoubtedly contributed to survival.

A metallic mediastinal foreign body, which eroded into the esophagus and resulted in a mediastinal abscess, was removed through the mediastinoscope. Tubes for drainage and irrigation were easily inserted after the pus had been entirely drained. Satisfactory low-risk treatment was accomplished without the need for major surgical intervention.

Of forty patients with unstable fractures and fracture-dislocations of the thoracic or lumbar spine treated with Harrington instrumentation and spine fusion, thirty-five had a neural deficit (twenty-three with incomplete or cauda equina lesions and twelve with complete lesions).
