Bridging the Gap between Literature and Practice: Nationwide Outcomes of Endoscopic Ultrasound-guided Hepatico-gastrostomy.

Background Endoscopic ultrasound-guided hepatico-gastrostomy (EUS-HGS) enables internal biliary drainage via the stomach in patients with altered anatomy, inadequate left hepatic duct drainage, or duodenal obstruction. The technically demanding procedure is only performed at expert centers in the Netherlands. Most available evidence originates from selected cohorts, overestimating real-world outcomes. We evaluated outcomes of EUS-HGS in a nationwide cohort. Methods This nationwide retrospective cohort study included all patients in the Netherlands who underwent EUS-HGS attempts between 2009 an

A Multi-Subject Tractography Atlas of Human Cerebellar Connections.

The cerebellum is a highly interconnected structure involved in motor coordination and balance as well as cognitive, sensory, autonomic, and emotional functions. Its connectivity relies on complex afferent and efferent white matter pathways linking it with the brainstem, spinal cord, thalamus, cerebral cortex, vestibular system, and limbic-related structures. Recent advances in diffusion MRI (dMRI) and tractography have enabled the in vivo reconstruction of these pathways, offering new opportunities to study cerebellar anatomy and connectivity. However, compared with supratentorial white matte

Transcatheter Aortic Valve Implantation in Low-Risk and Younger Patients with Porcelain Aorta: A State-of-the-Art Narrative Review.

Background and Objectives: Porcelain aorta is an anatomy-driven high-risk phenotype characterized by extensive calcification of the ascending aorta, which complicates surgical aortic valve replacement by increasing embolic and technical hazards during cannulation and cross-clamping. As transcatheter aortic valve implantation (TAVI) expands into younger and low-surgical-risk populations, porcelain aorta creates a distinct clinical dilemma: optimizing short-term procedural safety while ensuring durable long-term outcomes and preserving future treatment options. Materials and Methods: We performe

Safety of endovascular shunting for normal pressure hydrocephalus from a prospective, multicenter, single-arm study.

OBJECTIVE: Normal pressure hydrocephalus (NPH) is a neurological condition characterized by impaired gait, cognitive decline, and urinary incontinence. Endovascular shunting using the eShunt Implant diverts cerebrospinal fluid (CSF) from basal cisterns to the internal jugular vein via a transvenous, transfemoral procedure. We report initial eShunt safety data for treatment of NPH through 90 days in a prospective, multicenter, single-arm trial. METHODS: NPH participants indicated for endovascular shunt placement were included after exhibiting positive gait response to lumbar CSF withdrawal and

Surgical vs. trans-catheter aortic valve replacement in patients With bicuspid aortic valve stenosis.

BACKGROUND: Aortic valve stenosis can be treated surgically or with transcatheter aortic valve implantation (TAVI). However, bicuspid aortic valve (BAV) anatomy presents unique challenges that may affect procedural outcomes. We aimed to compare outcomes between the two approaches. METHODS: a retrospective cohort of 480 patients with BAV stenosis: 75 (16%) underwent TAVI and 405 (84%) underwent SAVR. Median age was 76 (Q1-Q3: 72-82) in the TAVI group and 61 (Q1-Q3: 52-70) in the SAVR group. Patients with isolated aortic insufficiency were excluded. Primary analyses were adjusted for age, sex,

An eikonal model with re-excitability for fast simulations in cardiac electrophysiology.

Precision cardiology based on cardiac digital twins relies on accurate yet computationally efficient simulations of cardiac electrophysiology and arrhythmias. Detailed biophysical models, such as the monodomain equation, are computationally demanding and therefore have limited applicability in clinical settings. It is thus desirable to develop reduced models that preserve the main physiological features of arrhythmic propagation while enabling faster computation. The standard eikonal model, an approximation of the monodomain model, effectively predicts the arrival times of the electrical wavef

What do ALS motor phenotypes measure? A population-based decomposition of their prognostic dimensions.

BACKGROUND: The ALS-OPM 3.3 classification stratifies incident ALS through three axes: onset region (O), propagation time (P) and motor-neuron pattern (M). We assessed whether OPM 3.3 improves prognostic discrimination over established classifications and characterised how it decomposes their prognostic signal in a population-based cohort. METHODS: In the prospective Piedmont and Aosta Valley ALS Register (2000-2022), patients were classified on the three axes; M was operationalised as M_3class (M0 balanced, M1d UMN-predominant, M2d LMN-predominant), with the PLS-spectrum M1p analysed separat

Hemodynamic analysis of the aortic root based on differences in outflow graft angle in patients with implantable left ventricular assist devices.

Aortic insufficiency (AI) that develops during long-term support with continuous-flow left ventricular assist devices (LVADs) remains a significant clinical problem. Abnormal flow patterns and altered wall shear stress (WSS) on the aortic valve are believed to contribute to leaflet remodeling, yet the hemodynamic influence of outflow graft orientation in patient-specific anatomies is not fully understood. This study investigated how different outflow graft angles affect aortic valve WSS and coronary perfusion using computational fluid dynamics (CFD). Three LVAD recipients who underwent preoper

The Natural History of Snoring in Children With Sickle Cell Anemia in Relation to Upper Airway Anatomy and OSA.

BACKGROUND: Though children with sickle cell anemia (SCA) possess an elevated risk for sleep-disordered breathing (SDB), progression of obstructive sleep apnea (OSA) across childhood and optimal screening approach remain unclear. Improved understanding of SDB symptomatology and relationship to OSA may simplify screening. This study longitudinally describes SDB symptom profile in children with SCA and examines how they evolve in relation to upper-airway anatomy. METHODS: Patients with SCA aged 2-18 years were prospectively recruited. Demographic data, clinical assessments, and upper airway exa

Transperineal Coccygeal Muscle-Guided Endoscopic Approach for Early Distal Control of the Internal Iliac Vein in High-Complexity Pelvic Exenteration: Technical Tricks and Initial Experiences.

BACKGROUND: High-complexity pelvic exenteration requires extensive lateral dissection and often en bloc resection of the internal iliac vessels to secure margins. However, achieving safe, precise access to the distal pelvic sidewall through a confined operative field is technically demanding and hazardous. IMPACT OF INNOVATION: We developed a transperineal coccygeal muscle-guided endoscopic approach primarily to establish a reliable, anatomically guided pathway to the lateral pelvic compartments. Using the coccygeal muscle as a landmark facilitates precise identification of the distal interna
