We report our experience with 10 cases of sphenoidal aspergillomas treated by endoscopic sinus surgery (ESS). Chronic symptoms such as cough … been evaluated to confirm the diagnosis: histology, direct smear, fungal cultures, and serology for Aspergillus. ESS has been successfully carried out without morbidity in all cases. No recurrence of the disease is seen after a mean follow-up of 27 months.

A commercial latex agglutination (LA) test for the detection of circulating Aspergillus galactomannan was evaluated in sera obtained from 121 immunocompromised patients, including 19 with proven invasive pulmonary aspergillosis. Patient urine (the specimen of choice for detection of galactomannan) was not tested with the LA test as 34 of 81 specimens from controls gave false-positive results. The sensitivity (95%), specificity (90%) and negative predictive value (99%) of the LA test were similar to previously published results obtained with two EIAs. However, the positive predictive value was …

Infections caused by fungi of the genus Aspergillus have become a major and dangerous problem in the modern treatments of blood diseases using cytotoxic drugs. Invasive pulmonary aspergillosis is the usual clinical manifestation of the disease. The clinical signs, not very suggestive, are those of an infective pneumonia and localized radiological forms are initially more frequent in most cases. The extra pulmonary forms are frequent: nose and sinus, skin, bone, digestive, central nervous system are principal localisations; but all organs may be affected by the processus of dissemination. The p…

Children suffering from kwashiorkor could be grouped as those in whom aflatoxin was detectable in both serum and urine, and those in whom th … gs suggest that the consumption of a staple food such as maize that is contaminated with the fungus Aspergillus flavus exposes susceptible kwashiorkor children to the metabolic hazards of aflatoxins, resulting in a greater risk of frequent infections.

We report a case of Lennert's lymphoma complicated by the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) and autoimmune … course of COP-BLAM therapy. Then she was treated with IMV-triple P therapy. Eventually, she died of aspergillus pneumonia. The etiology of her SIADH was suggested to be an abnormal feedback mechanism of ADH secretion due to the infiltration of lymphoma cells into the diencephalic-hypophysial system.

Fungal infections in the immunocompromised cancer patient may threaten survival and represent a major therapeutic challenge. Granulocytopenia, especially, increases the risk of infection with the two most common fungal species, Candida and Aspergillus. In cancer patients, mucocutaneous disruptions due to administration of chemotherapeutic agents and use of IV lines, Hickman catheters, and nasogastric tubes provide portals of entry for Candida species. Although cryptococcosis, histoplasmosis, and coccidioidomycosis have gained recent attention because of their increasing prevalence in patients …

In summary, AFS is a newly recognized form of sinusitis, appearing in otherwise healthy young adults with a history of chronic bacterial or polypoid rhinosinusitis refractory to conventional therapy. Radiologic study may show patchy opacification or calcifications of the sinuses on CT. The patients have an elevated total IgE, peripheral eosinophilia, and positive skin tests for fungal antigens. They may also have elevated serum fungal allergen-specific IgE and IgG and precipitating antibodies to Aspergillus, Curvularia, or other fungi. Diagnostic and therapeutic surgical drainage of the sinuse…

A case report is presented of a female worker of a tee packing factory, with a 5 years history of tea dust exposition and symptoms of reversible airway obstruction and early allergic alveolitis. The results of spirometric and lung mechanics demonstrated reversible airway obstruction with co-existent restrictive changes, lowering of CO diffusing capacity (DLc0). Bronchial provocation tests with tea dust were carried out thrice and all were positive. The authors believe that the etiological factor of above mentioned changes were Aspergillus niger spores found in tea dust. Tea dust and the patien…

30 cases of autopsies of patients who died after human renal allografts were studied. The incidence of infection was 60% and the lethal infectious complications amounted to 43%. The most common infectious agents were bacteria, including mycobacterium tuberculosis. In addition, some opportunistic agents, such as Cryptococcus, Aspergillus, mucormycosis, Pneumocystis carinii and cytomegalovirus were also found. All these patients had a history of receiving large dosage of immunosuppressive drugs such as cyclophosphamide, azathioprine, prednisone and dexamethasone after renal allografts. Pathologi…

In addition to prolonged granulocytopenia high contamination of air with aspergilli is necessery for an epidemic of invasive pulmonary aspergillosis.

Over 4000 cases of fungal nail, sole and palm infection recorded during a three-year study period were investigated to determine the prevale … l infections. The most common species were Scopulariopsis brevicaulis, Hendersonula toruloidea, and Aspergillus sydowii. Scytalidium hyalinum was recorded for the first time from Canada, and an instance of nail infection by Gymnascella dankaliensis was recorded. Only H. toruloidea and Sc. hyalinum caused palm and sole infections.

Precipitating antibodies towards antigens associated with farmer's lung disease were studied in Finnish dairy farming and non-farming rural populations and the results were compared to those of serum samples of patients suspected of having a fungal allergy. The antigen panel consisted of Aspergillus fumigatus, Micropolyspora faeni and Thermoactinomyces vulgaris. All three microbes seemed to be important environmental inducers of antibody formation. The prevalence of antibodies towards all of them was higher in the patient group. The presence of A. fumigatus and T. vulgaris antibodies correlate…

This study was designed to evaluate the frequency of respiratory allergens in different age groups of asthmatic atopic children in the Chieti-Pescara area. We examined a pediatric population (507 children) aged between 1 to 17 years (mean 6.62 +/- 2.9). All the children were submitted to a panel of skin prick tests (SPT) for 12 common aeroallergens: Grass Pollens (G.), Parietaria (P.), Olive (O.), Artemisia (A.), Ragweed (R.), Dermatophagoides Pteronyssinus and Dermatophagoides Farinae (D.P. & D.F.), Cat and Dog dander (C.D.), Feathers (F.), Alternaria (Al.), Aspergillus (As.). Acute invasive fungal rhinosinusitis in immunocompromised children may rapidly progress beyond the sinonasal cavity and produce extensive facial and intraoral necrosis. We report an 11-year-old girl with acute myeloblastic leukemia who underwent haploidentical peripheral blood stem cell transplantation (PBSCT) and developed invasive Aspergillus infection involving the right cheek and oral cavity during profound neutropenia. Despite aggressive antimicrobial therapy, serial extirpative procedures, including endoscopic sinus surgery, turbinectomy, maxillectomy, parotidectomy, tracheostomy, and wi…
